Case history:

Patient: YL. W, a 37 years old female

Occupation: private company’s administrative assistant

Medical history: non-contributary

Drug history: no known allergy or side effects to medications or food

Dental history: had many different dental treatments in the past and was told to have severe periodontal problems over the pas few years.

Chief complaint: My gums bleed when brushing and sometimes they feel discomfort in the past few months. Now, I like to have my gums and teeth fixed and get back to the normal state and condition whatever the costs might be!!
