Abstract
The patient described here is a 75-year-old asymptomatic woman who on a routine evaluation was found to have striking leukocytosis. Her complete blood count revealed a white blood count of 166.7 x 10~3/mu L, a hemoglobin of 11.9 g/dL, and a platelet count of 277 x 10~3/muL. The differential included 5% neutrophils and 93% atypical,
lymphocytes, virtually all of which had very scanty cytoplasm. Some of the atypical lymphocytes had the appearance of classical "smudge" cells, and the nuclei of most of the cells exhibited a condensed chromatin pattern with markedly irregular nuclear contours and deep nuclear grooves, thereby giving the classic appearance of "buttock cells" (see Image 1).
