高血壓與慢性腎臟病治療新進展

摘 要

慢性腎臟病是重要的慢性非傳染性疾病,可能會進展至須長期透析治療的末期腎病 變,且有較高的心血管疾病罹病率與死亡率。慢性腎臟病所致之高血壓,主要是由於腎素 - 血管收縮素 - 醛固酮系統的活化所形成,因此降血壓藥物首選為血管收縮素轉化酶抑制劑 (angiotensin-converting enzyme inhibitor;ACEI) 或血管收縮素受體阻斷劑 (angiotensin receptor blocker;ARB),當血壓控制未達目標時可合併鈣離子通道阻斷劑或利尿劑的使用。慢性腎 臟病患者不具糖尿病且沒有尿蛋白時,血壓控制的目標為 140/90 mmHg 以下;而具蛋白尿之 慢性腎臟病患者原則上以血壓 130/80 mmHg 以下為控制目標。腎動脈狹窄會導致續發性高血 壓,以 ACEI 或 ARB 作為首選治療;當降血壓藥物控制不良時,才進行經皮腎動脈氣球擴張 術或外科手術。高血壓與慢性腎臟病息息相關,良好的血壓控制可以延緩腎功能退化並降低 心血管疾病的死亡率。

引言

慢性腎臟病是重要的慢性非傳染性疾病,

世界各國盛行率約在 8% 至 16% 間,因此在全 球衛生議題中占有重要的角色1。慢性腎臟病患 者可能會進展至須長期透析治療的末期腎病變,

且有較高的心血管疾病罹病率與死亡率2。常見

的慢性腎臟病起始因子包括糖尿病、高血壓、年 齡、藥物、尿路結石等2,3。造成慢性腎臟病進 展的惡化因子,除了原本的起始原因外,血壓 過高所導致的腎絲球內高壓也占了相當重要的 角色,因此血壓控制與降血壓藥物的使用是延 緩慢性腎臟病進展的關鍵之一1,3。茲於此介紹 高血壓與慢性腎臟病最新治療之原則。

慢性腎臟病患者降血壓藥物的選擇

一、腎性高血壓的機轉

慢 性 腎 臟 病 所 致 之 高 血 壓, 是 由 於 腎 素 - 血 管 收 縮 素 - 醛 固 酮 (renin-angiotensin- aldosterone;RAA) 系統的活化、鈉的滯留、自 主神經系統的作用等多重因素所形成。在腎臟 灌流量減少與腎絲球過濾率開始下降時,腎臟 會先藉由自主神經調控,減少入球小動脈的收 縮,使全身的血壓更易傳入腎絲球內,以維持 足夠的腎絲球灌流量4。當腎絲球過濾率下降更 明顯時,RAA 系統的啟動會增加體內血管收縮 素的濃度,血管收縮素可引起血管收縮與血壓 升高,於腎絲球內同時收縮入球小動脈與出球 小動脈,並選擇性在出球小動脈有更好的收縮 效果,使得腎絲球內壓力上升,以維持足夠之 腎絲球過濾率4-6。然而,在 RAA 系統長期過度 活化下,會導致慢性腎臟病患者的蛋白尿上升 及腎功能惡化7

二、RAA系統之抑制藥物

慢 性 腎 臟 病 患 者 的 降 血 壓 藥 物 首 選 為 血 管收縮素轉化酶抑制劑 (angiotensin-converting enzyme inhibitor;ACEI) 或血管收縮素受體阻 斷劑 (angiotensin receptor blocker;ARB),因為 ACEI 與 ARB 類 降 血 壓 藥 物 可 抑 制 RAA 系 統 的活化,選擇性對出球小動脈有較佳的舒張效 果,減輕腎絲球內的壓力並降低蛋白尿,故可 提供患者長期之腎臟保護效果7-9

在血管收縮素轉化酶被抑制的狀況下,血 管收縮素仍可藉由 chymase 等酵素的作用催化 而來,因此 ARB 可提供比 ACEI 更完全的血管 收縮素系統之阻斷。然而 ACEI 可藉由增加體 內 kinin 濃度的機轉,提供額外之出球小動脈舒 張作用及其他之心血管保護效果,同時也導致 了較易咳嗽的現象10,11。不同於 ARB 專一性的 阻斷 AT1 受體,ACEI 藉由抑制血管收縮素的生 成同時會阻絕 AT1 與 AT2 受體的活化。此外亦 有研究發現 ACEI 可能有抑制腎絲球肥大或纖 維化的保護效果12-14。僅管在藥理機轉上有這 些不同,ACEI 與 ARB 並沒有在降低血壓、減

輕尿蛋白、或臨床的腎臟保護作用上表現出差 8,15,16

由於 ACEI、ARB 可降低腎絲球內壓力的 機轉,使用初期時可能導致腎絲球過濾率的下 降與血中肌酸酐的上升;若開始使用 ACEI、

ARB 的前 2-4 個月期間,血中肌酸酐上升幅度 少 於 30-35%, 則 仍 可 繼 續 使 用 本 類 藥 物17 這 種 早 期 血 中 肌 酸 酐 輕 微 上 升 的 現 象, 代 表 ACEI、ARB 在本類病人可產生良好的腎臟保 護效果,一般建議在開始使用 ACEI、ARB 治 療的前 3-5 天要監測血中肌酸酐與鉀離子的濃 17。比起單獨使用,ACEI 與 ARB 的合併使 用,有更高的風險會發生暈厥、低血壓、高血 鉀、腎功能的下降等副作用;因此不建議合併 使用 ACEI 與 ARB18,19

腎 素 直 接 抑 制 劑 ( 如 aliskiren) 藉 由 對 腎 素的直接阻斷,可達到降低血壓的效果。臨床 試驗中,在已使用 ACEI、ARB 的糖尿病患者 若再合併使用腎素直接抑制劑時,可進一步再 降低尿蛋白,但是並未達到更理想的腎臟預後 或心血管預後,反而在合併使用後低血壓或高 血鉀的風險升高20,21。因此並不建議在已使用 ACEI 或 ARB 的病人,再合併使用腎素直接抑 制劑;僅在病人無法耐受使用 ACEI 或 ARB 的 情 況 下, 可 考 慮 改 用 腎 素 直 接 抑 制 劑 去 抑 制 RAA 系統的活化20,21。此外,不可在孕婦或準 備懷孕之女性使用 ACEI、ARB、或腎素直接抑 制劑,因這些藥物有致畸胎性之風險22

三、鈣離子通道阻斷劑與利尿劑

Dihydropyridine 鈣 離 子 通 道 阻 斷 劑 (calcium channel blocker; CCB) 包括 nifedipine,

amlodipine 等,有很好的降血壓療效,對入球 小 動 脈 有 很 好 的 血 管 擴 張 效 果, 在 全 身 血 壓 降 低 時, 仍 可 能 升 高 腎 絲 球 內 壓 力, 因 此 本 類藥物對於腎臟保護與尿蛋白排出量的影響 尚 無 定 論23-26。Non-dihydropyridine CCB ( 如 diltiazem、verapamil) 的 降 血 壓 療 效 雖 然 沒 有 dihydropyridine CCB 那麼強,但對入球小動脈 的血管擴張效果亦較弱,且同時有減輕出球小 動脈收縮的效果,故具有降低尿蛋白的效果,

因此 non-dihydropyridine CCB 可作為合併 ACEI 或 ARB 使用之降血壓用藥23,25,26

當 ACEI 或 ARB 無法達到良好之血壓控制 時,亦可合併使用低劑量的 thiazide 類利尿劑 ( 如 hydrochlorothiazide、indapamide、chlortha- lidone);合併使用可降低 RAA 系統抑制藥物所 致高血鉀、或利尿劑所致低血鉀的風險27,28。鹽 皮質激素受體拮抗劑 ( 如 spironolactone、eplere- none) 具有利尿與降低血壓的效果,當與 ACEI 或 ARB 併用時,可進一步降低尿蛋白,但是高 血鉀的可能性會增加,因此通常作為高血壓難 以控制時的第四線用藥29

許多慢性腎臟病患者的血壓在睡眠期間沒 有正常的下沉 (dipping,平均血壓比清醒期間下 降 15%),而此類患者有較高的心血管併發症之 風險,故慢性腎臟病患者在睡前可使用至少一 項非利尿劑之降血壓藥物,以形成睡眠期間之 正常血壓下沉30,31

慢性腎臟病患者高血壓與尿蛋白的治 療目標

一、非糖尿病之慢性腎臟病

在非糖尿病之慢性腎臟病患者,越高的尿 蛋白排出量與越快的腎功能惡化極為相關;良 好的血壓控制可延緩腎功能的衰退,而血壓控 制的腎臟保護效果在高量尿蛋白患者 ( 每日尿 蛋白排出量 500-1000 mg 以上 ) 特別明顯1,7,32 高量尿蛋白之高血壓患者首選之降血壓用藥為 ACEI 或 ARB, 以 血 壓 130/80 mmHg 以 下,

以及每日尿蛋白排出量 1000 mg 以下為治療目 標;若起始尿蛋白已達腎病症候群之水準 ( 每 日尿蛋白排出量 3500 mg 以上 ),則以尿蛋白 下 降 至 少 40-50%, 至 每 日 3500 mg 以 下 為 目

7,33,34。若須常規監測尿蛋白以評估療效,則

建議首次以 24 小時尿液送檢,與隨機尿液之蛋 白質 / 肌酸酐比值作比對,以便之後以隨機尿 液之蛋白質 / 肌酸酐比值追蹤時能有準確的估 35,36

高 量 尿 蛋 白 之 高 血 壓 患 者 合 併 有 水 腫 症 狀 時, 除 了 ACEI 或 ARB 的 使 用, 可 合 併 環 利尿劑 ( 如 furosemide、bumetanide) 的使用,

以達較佳之血壓控制;若血壓控制仍不理想,

可 再 併 用 non-dihydropyridine CCB。 若 飲 食 中 的 鈉 攝 取 過 量, 會 減 弱 ACEI、ARB、non- dihydropyridine CCB 等藥物的降尿蛋白效果;

此類患者應限制飲食中鈉的攝取量在每日 90 mEq 以下,並考慮併用利尿劑,方可達到良好 的降尿蛋白效果27,37,38

當 慢 性 腎 臟 病 患 者 不 具 糖 尿 病 且 沒 有 尿 蛋 白 時, 血 壓 控 制 的 目 標 為 140/90 mmHg 以

7,34,39。若此類患者有水腫症狀,可先以利尿

劑作為起始治療,待水腫改善後再加上 ACEI、

ARB、或 dihydropyridine CCB;若此類患者沒 有水腫症狀,則以 ACEI、ARB 為第一線降血 壓治療藥物,並以 dihydropyridine CCB、利尿 劑為第二、三線的治療藥物7,40

二、糖尿病腎病變

糖 尿 病 所 致 慢 性 腎 臟 病, 或 稱 糖 尿 病 腎 病 變, 疾 病 的 控 制 與 進 展 亦 與 高 血 壓 息 息 相 9,41。白蛋白尿 (albuminuria) 是糖尿病腎病變 典型的表現,與腎臟預後有很強之相關性,通 常可用隨機尿液之白蛋白 / 肌酸酐比值作為每 日排出量的估計42,43。每日白蛋白尿排出量在 30-300 mg 間稱為中度上升白蛋白尿 (moderately increased albuminuria,舊稱 microalbuminuria),

每日白蛋白尿排出量超過 300 mg 稱為重度上升 白蛋白尿 (severely increased albuminuria,舊稱 macroalbuminuria)7,42

第一型糖尿病患者合併有中度上升白蛋白 尿及高血壓時,以 ACEI 為首選用藥,並以血 壓 130/80 mmHg 以下為治療目標;若病人因咳 嗽或血管性水腫 (angioedema) 等副作用而無法 耐 受 ACEI 時, 可 改 用 ARB 治 療7,33,41。 第 二 型糖尿病患者合併有中度上升白蛋白尿及高血 壓時,ACEI 或 ARB 皆可作為首選用藥,並以 血壓 125-130/90 mmHg 以下為治療目標7,33,41 糖尿病腎病變患者以每日尿蛋白排出量在 500- 1000 mg 以 下、 或 尿 蛋 白 下 降 60% 為 治 療 目 標;在 ACEI 或 ARB 使用下血壓仍未達理想目 標時,可合併使用 non-dihydropyridine CCB;在 有水腫症狀時可合併使用利尿劑7,33

腎動脈狹窄的高血壓治療

腎 動 脈 狹 窄 主 要 的 原 因 為 動 脈 粥 狀 硬 化 (atherosclerosis) 與纖維肌性發育不全 (fibromus- cular dysplasia);動脈粥狀硬化好發於 45 歲以上 男性患者,影響近端腎動脈為主;而纖維肌性 發育不全好發於 50 歲以下女性患者,影響遠端 腎動脈為主44,45。腎動脈狹窄會活化 RAA 系統 導致續發性高血壓,大部份患者為單側之腎動 脈狹窄,而對側腎臟相對正常;少部份患者為 雙側腎動脈狹窄,或單側腎動脈狹窄合併對側 腎功能缺失44

腎動脈狹窄患者之高血壓,以降血壓藥物 作為起始治療46-48。使用 ACEI 或 ARB 通常有 很好的降血壓效果,但通常須合併其他類型降 血壓藥物,如利尿劑、CCB 等,方能達到理想 的血壓目標47。大部份的腎動脈狹窄患者 ( 包 括雙側腎動脈狹窄患者 ) 在使用 ACEI 或 ARB 時,腎絲球過濾率的下降情形並不嚴重;若血 中肌酸酐上升幅度少於 30% 時,仍可繼續使用 ACEI 或 ARB17。然而,腎動脈狹窄患者在血壓 已藉由藥物良好控制之下,腎絲球過濾率仍有 可能因為以下原因而逐漸下降:1. 全身血壓下 降使狹窄後端的腎臟灌流不足;2. 因動脈粥狀 硬化而使狹窄惡化;此腎功能下降之狀況尤以 雙側腎動脈狹窄患者較常發生49,50

當有以下任一情況時可考慮腎動脈血管重 新灌流治療 (revascularization):1. 診斷為腎動脈 狹窄不久前才發生血壓上升的現象,此類患者 對血管重新灌流治療的反應較佳;2. 在降血壓 藥物的使用下無法達到良好的血壓控制;3. 無 法耐受降血壓藥物的治療;4. 反覆的突發性肺 水腫 (flash pulmonary edema) 或嚴重的心臟衰 竭。此外,雙側腎動脈狹窄、或單側腎動脈狹 窄合併對側腎功能缺失的患者,若有無法解釋 的腎功能衰退時,亦可考慮血管重新灌流治療

47,51。腎動脈的血管重新灌流治療,以經皮腎

動脈氣球擴張術 (percutaneous transluminal renal angioplasty) 為首選療法,動脈粥狀硬化導致之 腎動脈狹窄患者宜合併支架置放術 (stenting),

纖維肌性發育不全導致之腎動脈狹窄患者則在

氣球擴張術發生動脈剝離或穿孔時才建議併用 支架置放術45,47,51。當患者有以下複雜的解剖學 構造或病變時才考慮以外科手術進行血管重新 灌流治療:1. 多條之小型腎動脈;2. 腎動脈之 早期主要分支;3. 近腎動脈分支之腹主動脈須 手術治療 ( 如主動脈瘤 );4. 其他支架置放困難 或置放失敗之情況47

結論

慢性腎臟病患者的降血壓藥物首選為 ACEI 或 ARB,血壓控制未達目標時可合併 CCB 或利 尿劑的使用。慢性腎臟病患者不具糖尿病且沒 有尿蛋白時,血壓控制的目標為 140/90 mmHg 以下;而具蛋白尿之慢性腎臟病患者原則上以 血壓 130/80 mmHg 以下為控制目標。腎動脈狹 窄會導致續發性高血壓,以 ACEI 或 ARB 作為 首選治療;當降血壓藥物控制不良或患者無法 耐受降血壓藥物時,才進行腎動脈血管重新灌 流治療,包括經皮腎動脈氣球擴張術及外科手 術。高血壓與慢性腎臟病息息相關,高血壓是 慢性腎臟病的起始因子及惡化因子,腎功能的 下降也會導致血壓的上升;因此良好的血壓控 制可以延緩腎功能退化並降低心血管疾病的死 亡率。

Chronic kidney disease (CKD) is a major noncommunicable disease, which leads to higher risks of end-stage renal disease, cardiovascular morbidity, and death. CKD related hypertension is mainly due to activation of the renin-angiotensin-aldosterone system, therefore the first line antihypertesives are angiotensin-converting enzyme inhibitors (ACEI) or angiotensin receptor blockers (ARB). Calcium channel blockers or diuretics can be added if the blood pressure has not achieved the optimal target in the patient. In nondiabetic CKD patients without proteinuria, the blood pressure target is 140/90 mmHg or lower; while CKD patients with proteinuria should keep their blood pressure below 130/80 mmHg. Renal artery stenosis can lead to secondary hypertension, and the first line therapy is the antihypertensive treatment with ACEI or ARB; percutaneous transluminal renal angioplasty or surgery can be considered if resistant hypertension under medical treatment. Hypertension and CKD are closely interrelated, and optimal blood pressure control is able to decrease the rissk of decline in renal function and cardiovascular mortality.
