摘要

蜘蛛膜下腔出血經常合併發生各種心律不整,其中包含致命性的心室性心律不 整。本文報告一位 56 歲男性因頭痛至急診室求診,在接受進一步檢查前突然喪 失意識及呼吸。開始急救時發現病患呈現無脈搏心室頻脈,在病患恢復心跳血壓 後,隨即安排腦部電腦斷層掃瞄,發現顱內有蜘蛛膜下腔出血的變化。本文將回

顧蜘蛛膜下腔出血可能合併的心律不整與其可能的機轉。

Abstract

Subarachnoid hemorrhage (SAH) is often associated with cardiac arrhythmias including serious ventricular arrhythmias. We describe a male patient who presented with headache and sudden cardiac arrest in our emergency department.

During resuscitation the initial electrocardiographic (EKG) recording showed pulseless ventricular tachycardia (VT). Computed tomography scans of his brain after stabilization revealed SAH. Previous literatures were reviewed and the possible mechanisms were discussed.
