糖尿病合併冠狀動脈疾病 — 從藥物治療至血管重建之綜論

摘 要

冠狀動脈疾病 (coronary heart disease) 是糖尿病患者死亡的主要原因,而糖尿病患 者亦常併發心血管疾病,等同於冠狀動脈心臟病的高危險群 (coronary heart disease risk equivalent),故愈早將糖化血色素控制在理想值內,就愈能減少相關併發症的發生。近年 來糖尿病藥物蓬勃發展,許多藥物不僅可改善血糖控制,更可減少不良心血管事件的發 生。而冠狀動脈疾病治療的血管重建,包括經皮冠狀動脈介入治療 (percutaneous coronary intervention,PCI) 及冠狀動脈繞道手術 (coronary artery bypass graft,CABG) 其預後於糖尿病 患者皆較差。本文回顧糖尿病藥物之許多指標性的心臟血管結果試驗 (cardiovascular outcomes trial),並回顧相關文獻,比較經皮冠狀動脈介入治療及冠狀動脈繞道手術於糖尿病患者之優 劣,以期能增加內科醫師於糖尿病合併冠狀動脈疾病治療之認識。

前言

隨着人口的日益老年化,糖尿病合併冠狀 動脈疾病患者患者也逐漸增多,冠狀動脈疾病

coronary heart disease)是糖尿病患者死亡的 主要原因。與非糖尿病患者相比,糖尿病病患 罹患冠狀動脈疾病之機率較高,其通常是多支 血管病灶 (multi-vessel coronary artery disease ),

並且為無症狀之心肌缺氧。許多研究顯示,糖 尿病是冠狀動脈疾病的獨立危險因素1HbA1C 亦與心血管疾病及死亡率呈線性正相關2。因 此,罹患冠狀動脈疾病之糖尿病患者的長期存 活率低於非糖尿病的冠狀動脈疾病患者。

及 早 且 長 期 將 糖 化 血 色 素 控 制 在 理 想 值 內,就能產生”餘蔭效應”(legacy effect),根 據英國糖尿病前瞻性研究(the UK Prospective

Diabetes Study, UKPDS),糖化血色素每下降 1%,病患於 10 年內中風機率將減少 12%,心 肌梗塞減少14%。近年來糖尿病藥物不斷推陳 出新,許多心臟血管結果試驗(cardiovascular outcomes trial) 數據研究結果亦相繼發表,美 國、歐洲及台灣心臟學會與糖尿病學會皆發表 了有關糖尿病的最新臨床指引及共識。

在介入治療方面,經皮冠狀動脈介入治療 (percutaneous coronary intervention,PCI) 或冠 狀動脈繞道手術 (coronary artery bypass graft,

CABG) 的短期和長期預後在糖尿病患者均較 差,隨著冠狀動脈介入性治療的進步,包括技 術及輔助器械的突破和藥物支架的發明,心臟 內科醫師對於冠狀動脈的介入治療也有許多的 進展,大幅提升了接受藥物支架治療的病患術 後之預後。

本文參考近年發表之重要指標性研究,介 紹糖尿病合併冠狀動脈疾病患者接受藥物治療 及血管重建治療 (revascularization) 之最新治療 建議。

藥物治療

一、藥物介紹

(一) 雙胍類 (Biguanides)

UKPDS 研究中,Metformin 針對初診斷 2 型過重的糖尿病人,後續長期追蹤也顯示 較佳的血糖控制可以減少死亡率、心血管事件 以及心肌梗塞3-5。除非有禁忌症,Metformin 單一藥物治療應在第二型糖尿病被診斷時就開 始。Metformin 有效且安全、價格低廉,然而在 臨床使用上,Metformin 應該避免使用於嚴重心 臟衰竭、腎功能不全、肝功能異常、代謝性酸 中毒和急性疾病,避免引起乳酸性酸中毒。

(二) Thiazolidinedione (TZD)

2005 年 發 表 的 PROACTIVE 研 究6 針 對 第2 型糖尿病合併心血管疾病的患者,

比 起 對 照 組 能 減 少16% 次 要 終 點 (secondary endpoint) 事 件 ( 總 死 亡 率、 非 致 死 性 心 肌 梗 塞、 腦 中 風) 的 發 生。 在 2007 年 發 表 的 PROACTIVE 之次分析,對於有心肌梗塞病史的

第二型糖尿病病患7,亦顯示Pioglitazone 可以 顯著減少28% 的心肌梗塞、37% 的急性冠心症 以及19% 的大血管併發症,但也顯著增加 41%

的心衰竭住院。因此Pioglitazone 可適用於心血 管疾病患者的糖尿病治療,但對於有鬱血性心 臟衰竭的風險之族群須小心使用。

(三) 磺醯尿素類 (Sulfonylurea)

1970 年代的 The University Group Diabe- tes Program (UGDP) 發 現 服 用 Tobutamide 相 較安慰組會增加心血管的死亡率8。加拿大的 回 溯 性 研 究 發 現9, 第 一 代 磺 醯 尿 素 類 藥 物 Glyburide 劑量增加後,心血管事件相關的死亡 會增加。另一個研究指出,除了Gliclazide 之外 的磺醯尿素類藥物都可能增加心血管風險10 第二代Sulfonylurea 藥物如 Glimepiride、Glipi- zide、Gliclazide,與其他藥物交互作用較少,目 前較常使用,在ADVANCE study 中,顯示使用 Gliclazide 不會增加 3-point MACE (HR:0.94,95%

CI:0.84-1.06) 及 non-fatal MI (HR 0.98, 95%

CI:0.77-1.22)11

目前認為Sulfonylurea 會增加心肌梗塞風 險之機轉,可能與Sulfonylurea 會關閉存在於 心 肌 之ATP 依賴型鉀離子通道 (ATP-sensitive potassium channel),進而阻斷心肌於缺氧時之預 適應(ischemic pre-conditioning) 有關。Ischemic pre-conditioning 為心肌細胞之自我保護機制,

當 心 肌 缺 氧 時 會 使ATP-sensitive potassium channel 打開,減少鈣離子流入細胞,使心肌收 縮減少而降低心肌對氧氣的需求,故可在心肌 缺氧時保護心臟。

(四) 非磺醯脲類促胰島素分泌劑 (Meglitinides 類 )

包括repaglinide 以及 nateglinide,與 Sulfo- nylurea 類相較,其吸收速率較快且作用時間較 短,因此建議隨餐服用。丹麥的回溯性研究指 出,磺醯尿素類的gliclazide 以及 Meglitinides 類的repaglinide,相較於 Metformin,無論病人 有無心肌梗塞病史,都不會增加總死亡率、心 因性死亡率或主要心血管事件12

(五) 阿爾發葡萄糖苷酶抑制劑 (α-glucosidase inhibitors)

1999 年 UKPDS 對 於 acarbose 的 前 瞻 性 研 究13, 以 及2005 年 一 篇 關 於 單 獨 使 用 α-glucosidase inhibitors 的整合分析中顯示14 對 於 第 二 型 糖 尿 病 病 患, 並 沒 有 證 據 顯 示 α-glucosidase inhibitors 對心血管事件的發生或 致死率有顯著影響。The acarbose cardiovascular evaluation (ACE) trial 亦顯示 α-glucosidase inhibi- tors 無法減少心血管事件的發生15。因此對於冠 狀動脈疾病預後的影響,acarbose 是一個 neutral effect 的藥物。

(六) 二肽基酶 -4 抑制劑 (Dipeptidyl peptidase 4 inhibitors, DPP 4 inhibitors)

腸 泌 素 (incretin), 包 括 類 升 糖 素 肽 -1 (glucagon-like peptide-1,GLP-1) 和葡萄糖依賴 性胰島素刺激多肽(glucose-dependent insulino- tropic polypeptide, GIP) 兩種。DPP 4 抑制劑可藉 由對Dipeptidyl peptidase -4 的抑制,提高活性 腸泌素的濃度,促進胰島素的釋出和抑制升糖 素的分泌,達到降低血糖的作用16,17。根據近期 研究顯示,對於本身已有心血管風險的病人使 用大多數類型的DPP-4 抑制劑並不會影響心血 管事件的預後及心臟衰竭的風險18-21,但要注 意的是,Saxagliptin 在 SAVOR-TIMI 53 中比起 對照組 (Placebo) 有較高的心衰竭住院的發生率 (3.5% vs 2.8%)22

(七) 似 升 糖 素 胜 肽 -1 受 體 促 進 劑 與 類 似 物 (glucagon-like peptide 1 (GLP-1) receptor agonist and analogue)

GLP-1 是人體內主要的腸泌素 (incretin) 之 一,是從遠端迴腸及部分結腸所分泌的腸道荷 爾蒙。GLP-1 會促進胰臟 β 細胞分泌胰島素並 抑制胰臟α 細胞分泌升糖素,而使血中葡萄糖 濃度降低。Liraglutide 於 LEADER 試驗中,能 減少13% 主要不良心血管事件 ( 心因性死亡,

非 致 死 性 心 肌 梗 塞, 非 致 死 性 腦 中 風) 的發 23Semaglutide 於 SUSTAIN-6 試驗中,能減 26% 的主要不良心血管事件 ( 心因性死亡,

非致死性心肌梗塞,非致死性腦中風)24

(八) 第 2 型鈉 - 葡萄糖轉運蛋白抑制劑 (sodium glucose transporter 2 inhibitors)

SGLT2 抑制劑阻斷近端腎小管中的葡萄 糖重吸收,導致血糖的降低。這些藥物在第二 型糖尿病中可提供適度的體重減輕和血壓降 低。在指標性的SGLT2 抑制劑 Cardiovascular Outcomes Trials 中 (the EMPA-REG OUTCOME trial、The CANVAS program、DECLARE-TIMI 58 trial),使用藥物半年至一年之間,即呈現出 風險下降的差異,有別於GLP1 類似物在實驗 的後期才逐漸呈現下降風險的差異。SGLT2 抑 制劑最大特色在於心衰竭風險明顯減少,這暗 示著SGLT2 抑制劑的保護心血管好處,可能是 出自於心衰竭的部分,與GLP1 受體促進劑來 自於不同機轉。

EMPA-REG OUTCOME 總共有 7,020 名患 者參與試驗,其中99% 有心血管疾病,Empa- gliflozin 能減少 14% (hazard ratio, 0.86; 95% CI, 0.74 to 0.99, P = 0.038) 主要不良心血管事件 ( 心 因性死亡,非致死性心肌梗塞,非致死性腦中 )、減少 38% (hazard ratio,0.62; 95% CI, 0.49 to 0.77, P < 0.001) 心因性死亡、減少 35% (hazard ratio, 0.65; 95% CI, 0.50 to 0.85; P=0.002) 心衰竭 住院率25

CANVAS Program 總共有 10,142 名患者參 與試驗,其中65.6% 合併有心血管疾病,Cana- gliflozin 能減少 14% (hazard ratio, 0.86; 95% CI, 0.75 to 0.97; P<0.001 for non-inferiority; P=0.02 for superiority) 主要不良心血管事件 ( 心因性死 亡,非致死性心肌梗塞,非致死性腦中風),減 33% (hazard ratio, 0.67; 95% CI, 0.52 to 0.87) 心衰竭住院率26

2018 年 11 月 於 美 國 心 臟 學 會 (AHA) 年 會 發 表 之DECLARE-TIMI 58 trial27, 總 共 17,160 名患者完成研究,其中 40.6% 有心 血 管 疾 病,Dapagliflozin 相 較 於 安 慰 劑 組,

減少了17% 的心血管死亡或心衰竭住院風險 (4.9% vs. 5.8%; hazard ratio, 0.83; 95% CI, 0.73 to 0.95; P=0.005),這部分主要貢獻於心衰竭住

(hazard ratio, 0.73; 95% CI, 0.61 to 0.88) 而不 是心血管死亡(hazard ratio, 0.98; 95% CI, 0.82 to 1.17)27

此外,近期整合分析研究結果亦顯示類似 結 果,SGLT2 抑制劑可顯著降低總死亡率、

主要心血管事件死亡、心肌梗塞及心衰竭的風 28。在2018 年台灣心臟學會及糖尿病學會 共同發表的共識中,即建議糖尿病合併冠狀動 脈病人,使用Metformin 第一線治療後,第二 線 依 序 可 考 慮 使 用Pioglitazone、SGLT2 抑制 劑與GLP-1 作用劑,以減少不良心血管事件發 29

二、HbA1C 治療目標

2018 年台灣心臟學會及糖尿病學會共同發 表的共識,建議以HbA1c 低於 7.0% 作為糖尿 病合併冠狀動脈疾病患者的治療目標。但是,

對 於 較 年 輕、 糖 尿 病 罹 病 時 間 短、 共 病 症 較 少、且低血糖風險較低的族群,若沒有其他副 作用,則嚴格控制HbA1c 小於 6.5% 是可被考 慮的。

對於糖尿病合併心臟衰竭患者,許多研究 皆顯示HbA1c 和死亡率的關係可能存在 U 形曲 線,亦即過於積極的血糖控制反而會使死亡率 上升,因此2018 年台灣心臟學會及糖尿病學會 共同發表的共識,建議以HbA1c 低於 8.0% 作 為糖尿病合併心臟衰竭患者的治療目標。

三、藥物的選擇順序

對於糖尿病合併冠狀動脈疾病患者,2018 年台灣心臟學會及糖尿病學會共同發表的共識 建議,第一線治療為Metformin,若血糖仍控制 不良,需考慮dual therapy 時,第二線藥物可優 先考慮使用Pioglitazone,接下來可依序考慮使 SGLT2 抑制劑與 GLP-1 受體促進劑,若需考 慮第四種藥物,則可使用DPP-4 抑制劑,因為 其具有中性效果和安全性( 表一 )。

若為糖尿病合併心臟衰竭患者,第一線治 療 為Metformin 或 SGLT2 抑 制 劑,Metformin 不應該使用於急性心臟衰竭、休克或敗血症之 低血氧症患者,,以避免產生乳酸中毒。若需 考慮dual therapy 時,則建議使用 Metformin 加 SGLT2 抑制劑合併使用,第三線藥物可考 慮使用GLP-1 受體促進劑,接下來第四線藥物 可考慮使用DPP-4 抑制劑,但是 Saxagliptin、

alogliptin 及 vildagliptin 則應避免使用 ( 表二 )。

血管重建治療 (Revascularization)

糖尿病合併冠動脈疾病,其冠狀動脈病灶 特徵為多支血管病變 (multi-vessel disease),血 管管徑小,多瀰漫性 (diffuse) 病灶,多慢性阻 塞 病 灶 (chronic total occlusion) 且缺乏側枝循 環,多左主幹(left main) 病灶,此外病灶出血、

潰瘍、栓塞和鈣化的程度均較嚴重。

糖尿病患者合併冠狀動脈疾病的治療策略

證據之一為BARI 2D trial30。在BARI 2D trial 中, 招 募 了2368 名患有 2 型糖尿病和穩定性 缺血性心臟病( 冠狀動脈狹窄≥ 50%合併心臟 壓力測試陽性或≥ 70% 狹窄合併典型心絞痛 ) 的 患 者。 在 四 周 內 隨 機 接 受 血 管 重 建 術(PCI CABG) 合併強化藥物治療 (intensive medical therapy,IMT) 或單獨接受強化藥物治療,患者 接受CABG 或 PCI 治療的決定是依據主治醫生 的臨床決策。

在各亞組的心因性死亡和心肌梗塞 (cardiac death and MI) 的次要終點分析中31,血管重建 術合併強化藥物治療和單獨使用強化藥物治療 相比,五年心因性死亡率無顯著差異( 分別為 5.9% vs 5.7%)。然而,在 CABG 族群 (n = 768) 中,CABG 合併強化藥物治療的心肌梗塞發生 率明顯低於單獨使用強化藥物治療族群(10%

vs 17.6%),心因性死亡和心肌梗塞的複合終點 (15.8% vs 21.9%) 亦較低。

為了進一步了解哪些患者可能從CABG 中 受益,由血管造影和臨床變量(Framingham 風 險評分) 計算出風險評分。在最高血管造影風險 三分位數的患者中,複合終點 ( 死亡,心肌梗 塞,腦中風) 在 CABG 層中的發生率明顯低於 強化藥物治療組(24.8% vs 36.8%),這種影響在 高血管造影合併高臨床風險的患者中更為明顯 (27.1% vs 47.3%; HR: 2.10; p = 0.0051)32。這項 研究顯示,在糖尿病患者中,愈高血管造影和 臨床風險的患者愈能從CABG 的血管重建中受 益。

一、糖尿病患與非糖尿病患者相較

與 非 糖 尿 病 患 者 相 比, 糖 尿 病 患 者 經 皮 冠狀動脈介入治療(PCI) 或冠狀動脈繞道手術 (CABG) 血管重建後的結果較差。

在 接 受elective PCI 的患者中,糖尿病患 者與非糖尿病患者相比33-35,糖尿病患者的再 狹窄率較高,心肌梗塞率較高,死亡率較高,

血管重建率(revascularization) 較高,event-free survival 比率較低。大多數的心血管事件在做完 PCI 後的第一年發生。糖尿病患者再狹窄的預測 因子包括較小的血管口徑,較長的支架長度和

較低的體重指數 (BMI)。生長因子於血管平滑肌 的刺激作用導致內膜增生加速,以及加速的纖 維化反應是可能的再狹窄機轉36。病灶再重建 (target lesion revascularization),心肌梗塞及存活 率等結果皆會受支架類型以及糖尿病相關因素 的影響。

在接受elective CABG 的患者中,糖尿病患 者和非糖尿病患者的手術成功率相似。然而,

在追蹤期間,CABG 後死亡和不良非致命性結 (adverse non-fatal outcomes) 在糖尿病患者中 較高。糖尿病與腎衰竭和傷口感染相關37,手 術期維持嚴格的血糖控制可以降低其風險38,39 糖尿病也與動脈粥狀硬化或症狀復發的風險為 正相關40,在non-bypassed 及 bypassed 的原生 冠狀動脈中均可見到疾病進展41。儘管糖尿病 CABG 後不會影響院內死亡37,42,但糖尿病 患 者 接 受CABG 後的短期和長期存活率皆顯

著降低40,43-45。在許多大型研究中發現,糖尿

病患者在30 天死亡率皆較高 (5% vs 2.5%; p <

0.001)435 年及 10 年的長期死亡率亦較高 (5 年:22% vs 12%,10 年:50% vs 29%; both, p

< or =0.05)44。 因 此, 相 較 與 非 糖 尿 病 患 者,

CABG 在糖尿病患者中的長期預後較差。

二、PCI 與 CABG 相較

目 前 證 據 認 為 對 於 病 人 存 活 有 益 處 的 冠 狀動脈繞道手術治療對象為左主幹(left main disease) 狹窄超過 50% 的病人或者三條冠狀動脈 疾病(three-vessel disease)、兩條冠狀動脈 (two- vessel disease) 其中包含左前降支近端以及單獨 左前降支近端狹窄超過70% 的病人46,47PCI 與顯著較高的不良心血管事件及重複血管重建 治療 (repeat revascularization) 相關48,而CABG 與更高的中風率相關49

SYNTAX 試 驗 以 左 主 幹 疾 病 (left main disease) 或 三 條 冠 狀 動 脈 疾 病 (three-vessel disease) 的病人接受 PCI ( 第一代 paclitaxel 塗 藥支架) 與 CABG 做比較,該試驗包括 1,800 個 病 患, 其 中452 名 為 糖 尿 病 患 者。2013 年 SYNTAX trial 的五年追蹤分析中顯示,在糖尿 病患者中接受PCI 治療 (n = 231) 後的主要不良

心血管事件及重複血管重建治療(repeat revas- cularization) 高於接受 CABG,此趨勢在 syntax score 高的患者更為顯著有意義48,50,51。以冠狀 動脈病灶複雜度作分組 (SYTAX score 越高代表 病灶越複雜),在左主幹疾病 (left main disease) SYNTAX score 小 於 33 分 的 病 人,PCI 及 CABG 兩組的主要不良心血管事件 ( 包含死亡、

心肌梗塞、中風、重複血管重建治療) 沒有顯 著 差 異, 但 若SYNTAX score 在 33 分以上,

CABG 組的主要不良心血管事件會比 PCI 組 少。在三條冠狀動脈疾病 (three-vessel disease) 病變且SYNTAX score 小於 23 分的病人當中,

PCI 及 CABG 兩組的主要不良心血管事件沒有 顯著差異,但若SYNTAX score 在 23 分以上,

CABG 的主要不良心血管事件會比較少51 因此,依據SYNTAX trial,在第二型糖尿 病 患 者 族 群CABG 與 PCI 相 較 時, 左 主 幹 疾 (left main disease) 且 SYNTAX score >32 分的 病人,CABG 優於 PCI,但對於 SYNTAX score

<32 分之族群,兩者無顯著差異 ; 在三條冠狀 動脈疾病 (three-vessel disease) 病變且 SYNTAX score >23 分的族群,CABG 優於 PCI,但對於 SYNTAX score <22 分之族群,兩者無顯著差 異。

2013 年的一篇薈萃分析 (meta-analysis) 涵 3052 名糖尿病合併 multivessel disease ( 七至 八成病患為three-vessel disease) 之患者,使用第 一代塗藥支架與CABG 相比較,顯示使用第一 代塗藥支架的病患,死亡和心肌梗塞的風險皆 較高(RR: 1.51, 95%CI 1.09-2.10; P <0.01),但中 風風險較低 (2.3% vs.3.8%; RR 0.59,95% CI 0.39- 0.90; P <0.01)52

2015 年發表於 NEJM 的 BEST 試驗,糖尿 病患者使用新一代everolimus 塗藥支架 (EES) 進行PCI 治療相較於 CABG,在包含了 death,

MI,或 TVR 的主要複合終點 (primary endpoint) 之發生率較高 (EES vs CABG:19.2% vs 9.1%;

P = 0.007)53。然而2015 年另一個網絡薈萃分析 (network meta-analysis) 則顯示相反結果 : 在糖尿 病患者中,若使用新一代everolimus 塗藥支架 (EES),CABG 相較於 PCI 的生存獲益可能會消

另一個2018 年發表於 lancet 的數據匯總分 (pooled analysis),包含 11,518 例 multivessel LM disease 患 者, 隨 機 分 配 接 受 CABG 或 PCI 治療,顯示於五年之全因死亡率有顯 著 差 異 (CABG vs PCI:9.2% vs 11.2 % ; hazard ratio [HR] 1.20; P = 0.0038)。在次族群分析中,

顯示在糖尿病subgroup 中有顯著差異 (CABG vs PCI:10.7% vs 15.7%; P = 0.0001),但在沒有 糖尿病的患者中則沒有 (CABG vs PCI:8.4% vs 8.7%; P = 0.81)(P for interaction = 0.0077)。 在 multivessel disease 之 subgroup 中 ( 其中約六成 病患為three-vessel disease),其與糖尿病的相 互作用顯著(P for interaction = 0.0453); 而在 LM disease subgroup,與糖尿病的相互作用則不顯著 (P for interaction = 0.13)55。因此,2018 年歐洲 心臟學會 (ESC) 發表之 guideline 即建議於糖尿 病合併多支血管病變 (multivessel disease) 患者,

較有利的血管重建方式為施行CABG56

三、支架間之相較

藥物支架(drug-eluting stent) 的發明使得冠 狀動脈的治療有了革命性的突破57。支架置放 後發生的再狹窄是因為動脈內壁對治療所產生 的機械性傷害的痊癒反應(healing response),包 括血管重塑(vessel remodeling) 與新生內皮層的 過度增生(neointima hyperplasia) 等。支架的支 撐力可有效改善血管重塑的問題58,而新生內 皮層的過度增生 (neointima hyperplasia) 則是由 於動脈壁受傷後的發炎反應,引起白血球及免 疫蛋白活化,最終使得平滑肌細胞增生而減少 了血管內徑,造成再狹窄59。藥物支架乃是在 血管支架上塗布藥物,穩定釋放以達成抑制血 管內壁平滑肌增生,進而預防支架再狹窄。

藥物支架(DES) 優於裸金屬支架 (BMS),

因 為DES 與有較低的再狹窄 (restenosis)、病 灶再重建(target lesion revascularization,TLR) 及血管再重建(target vessel revascularization,

TVR)發生率。第一代 DES (sirolimus-eluting stents (SES) and paclitaxel-eluting stents (PES) 與 新 一 代DES 相 比, 新 一 代 DES (zotarolimus-

eluting, everolimus-eluting, and ridaforolimus- eluting stents (ZES, EES, and RES)) 提供更低的 TVR 和 TLR 率。新一代 DES 的 platform 厚度 較第一代DES 薄,且比第一代 DES 更具生物 相容性: 較少的炎症反應且血管內皮化較快速。

此生物相容性增加是由於聚合物技術(polymer technology) 的改進,也因此導致較低的心肌 梗塞和支架血栓形成機率。在2012 年發表,

涵蓋42 個 randomized trials 的 meta-analysis 顯 示:EES 相 較 其 他 DES 及 BMS, 可 顯 著 降 TVR,並且沒有增加任何安全性結果 ( 死 亡、心肌梗塞或支架內血栓形成) 的風險,因 EES 成為最佳塗藥支架的選擇60。 於 同 年 (2012) 發表的一篇 meta-analysis,涵蓋了 49 個 randomized trials 及 50,844 個病患,亦顯示 EES 相較其他種類塗藥支架 (ZES、SES、PES) 及裸 金屬支架,有較低的支架血栓形成機率61

糖尿病合併冠狀動脈疾病患者選擇冠狀動 脈重建時,應該以病人為中心 (patient-centered approach),著重個別化的治療 (individualization of treatment),並應成立心臟團隊 (heart team) 共 同評估冠狀動脈的解剖構造,且盡可能讓病患 參與所有的處理決定(shared decision making),

尊重病患的喜好、需求與價值觀,共同擬訂可 用的治療策略及選擇最佳的重建方式。

結論

因人類的生活飲食型態改變,全球糖尿病 盛行率正以驚人的速度攀升,台灣亦不例外,

因此糖尿病儼然成為醫界之一大挑戰。根據衛 生福利部國民健康署2018 年的資料顯示,台灣 18 歲以上糖尿病的盛行率為 10.12%,2017 年 國人十大死因排行榜上,糖尿病若加總計算其 所引發的腎臟、腦心血管疾病、高血壓等併發 症,死亡率超過死因第一名的癌症。

血 糖 的 控 制 可 避 免 併 發 症 的 發 生, 傳 統 降 血 糖 藥 物 如metformin、TZD 皆 有 證 據 顯 示 對 心 血 管 疾 病 有 所 幫 助; 而 較 新 型 的 降 血 糖 藥 物 包 括GLP-1 受體促進劑或類似物以及 SGLT2 抑制劑,對第 2 型糖尿病患者除了降低 血糖的作用外,亦有大型的心臟血管結果試驗

(cardiovascular outcomes trial) 顯示其亦能減少不 良心血管事件。

糖尿病是冠狀動脈疾病非常重要的危險因 子之一,罹患糖尿病的冠狀動脈疾病患者死亡 率較非糖尿病患者高,同時罹患糖尿病與冠心 症的患者接受血管重建治療(revascularization) 後,無論是接受經皮冠狀動脈介入治療(percu- taneous coronary intervention,PCI) 或 冠 狀 動 脈血管繞道手術(coronary artery bypass graft,

CABG),其預後皆比沒有糖尿病患者較差。雖 然接受CABG 的患者中風的機率有些微增加,

但在SYNTAX 研究中顯示,糖尿病合併複雜冠 狀動脈病灶患者接受CABG,在不良心血管事 件及血管再狹窄重建治療 (repeat revasculariza- tion) 等方面顯著優於接受 PCI 治療,但是還是 需要更多與更長期的追蹤時間才能證實CABG 的優勢是否一直存在。另一方面,SYNTAX 研 究是CABG 與第一代的塗藥支架 (paclitaxel 塗 藥 支 架) 的 比 較, 結 果 雖 然 顯 示 CABG 優 於 PCI,但是在與新一代塗藥支架的比較仍然需要 進一步的研究加以探討。

隨著糖尿病藥物的蓬勃發展以及冠狀動脈 介入治療技術與支架的突飛猛進,相信內科醫 師在糖尿病合併冠狀動脈疾病的治療將扮演愈 來愈重要的角色。

Coronary heart disease is the leading cause of death in diabetic patients, and diabetic patients often have cardiovascular disease. Diabetes mellitus is equivalent to high risk of coronary heart disease, so the sooner glycosylated hemoglobin keeps within the ideal value, the more the related complications can be reduced. In recent years, diabetes drugs have vigorous development, and many drugs can not only improve blood sugar control, but also reduce the incidence of adverse cardiovascular events. Revascularization in the treatment of coronary artery disease, including percutaneous coronary intervention (PCI) and coronary artery bypass graft (CABG), has a poorer prognosis in patients with diabetes. This review article provides an exhaustive overview of many landmark cardiovascular outcomes trials of diabetes drugs and relevant literature to compare the advantages and disadvantages of PCI and CABG in patients with diabetes, in order to increase the understanding of the treatment for diabetes mellitus combined with coronary heart disease.
