前言
近年來對於糖尿病治療的藥物研發有長足 的進步 ,然而也有一些有爭議的地方 ,例如是 否需將糖尿病病人的糖化血色素(HbA1c)積極 地 降 到6.5%以下及糖尿病病人服用降血糖藥 物是否會增加心血管事件等 。流行病學的資料
顯示 ,台灣地區糖尿病的盛行率逐年上升 ,目 前約為6% ,而在40歲以下以及80歲以上的族 群其增加的幅度比其他年齡層顯著 ,至於發生 率方面 ,在40歲以下的男性族群有顯著增加 的趨勢1,2。糖尿病患者若血糖控制不佳 ,會引 發大血管病變 ,例如缺血性心臟病 、冠狀動脈 疾病 、急性冠心症 、缺血性腦梗塞和周邊血管
以心臟科醫師的角度來看糖尿病藥物的使用

摘 要
至今只有有限的研究顯示降血糖藥物能減少冠狀動脈疾病 ,其中最經典的研究是英國的 UKPDS研究,使用雙胍類(Biguanides),磺醯尿素類(Sulfonylurea)與胰島素(Insulin)在長期 追蹤的研究中發現可降低心血管疾病發生率 。新一代藥物中Thiazolidinedione (TZD)類衍生物 在心血管疾病的角色最受爭議 ,上市後Rosiglitazone即因會伴隨增加心血管疾病而下市。阿 爾發葡萄糖苷酶抑制劑(α-glucosidase inhibitors)對於心血管的臨床預後尚無一致性的結論,
需要更多資料來佐證 。DPP-4抑制劑(Dipeptidyl Peptidase-IV Inhibitors)與第1型類昇糖素胜 肽(GLP-1)是最近研發的藥物,近期小型的研究顯示DPP-4與GLP-1可能能夠改善一些心血 管疾病的監測指標(surrogate markers)。近期ACCORD與VADT研究都不支持要將HbA1c降至 6.5%以下,這些積極降血糖的研究都發現,病人心血管疾病沒下降,但低血糖的危險性與 死亡率會上升 。血糖控制應該以病人為中心的方式(A Patient-Centered Approach),著重個別 化的治療(individualization of treatment);因此以心臟科醫師的立場處方降血糖藥物,就降低 心血管疾病的目的來看 ,對高心血管風險或已經確診心血管疾病的糖尿病病人 ,採取傳統標 準治療至HbA1C < 7%即可。藥物的建議使用仍然是以雙胍類為首選,至於DPP-4抑制劑與 GLP-1類對心血管疾病是否有所助益,可能須看未來幾年的研究而定了。

疾病等;以及小血管病變 ,包括視網膜病變 、 腎病變和神經相關病變等 。心血管疾病是糖尿 病患者最常見的死因3,相較於正常族群 ,會 增加2到4倍的風險4,還有研究發現 ,有糖尿 病但沒有過去心肌梗塞或冠狀動脈心臟病病史 的病人 ,未來死於冠狀動脈心臟病的風險 ,並 不小於先前有心肌梗塞或冠狀動脈心臟病病史 但沒有糖尿病的病人 ,特別是在女性方面5,6。 另 一 方 面 來 說 , 嚴 格 血 糖 控 制 , 經 由DCCT/
DEIT研究證實,對於第一型糖尿病可以減少 小血管病變的進展並且有持久效果7-9;經由日 本的Kumamoto研究以及英國的UKPDS研究也 證實對第二型糖尿病有類似效果 ,稱之為“延 續效應”(legacy effect)10-13,亦即所謂代謝記憶 (metabolic memory)的效應。在大血管病變方 面 , 嚴 格 血 糖 控 制 , 經 由DCCT/DEIT後續研 究顯示在第一型糖尿病可以顯著減少心血管事 件並具有“延續效應”14,15;然而在第二型糖尿 病方面 ,UKPDS早期研究雖顯示不能顯著減少 大血管病變11,但後續長期追蹤研究顯示較佳 的血糖控制仍然可以減少心血管事件以及心肌 梗 塞13, 不 過 隨 後 的ACCORD, ADVANCE, 以 及VADT研究卻顯示嚴格血糖控制並不能顯著 減少主要心血管事件16-19,甚至可能因為低血糖 而造成死亡率的增加16。至於在糖尿病的監控方 面 ,HbA1c提供了三個月平均血糖值的估計,
每 增 加1%的HbA1c會加18%的心血管事件風 險;而每減少1%的HbA1c會減少14%的心肌梗 塞 、21%的糖尿病死亡以及37%的小血管併發
症風險20,21。但HbA1c正常並不能完全排除糖尿
病或糖尿病前期(Prediabetes)4,22。從加拿大的整 合分析的資料顯示23,空腹血糖值與口服葡萄 糖耐受試驗2小時血糖值,都是發生心血管風 險的危險因子 ,而來自歐洲的DECODE研究也
顯示24,25,口服葡萄糖耐受試驗2小時血糖值越
高 ,死亡率越高 ,比空腹血糖值更能有效預測 心血管疾病死亡率 。日本的Funagata糖尿病研 究顯示 ,葡萄糖耐受不良患者死於心血管疾病 的風險較高 ,但空腹血糖值正常或偏高族群則 相當;而國人的金山社區心臟血管疾病追蹤研究 則顯示26,空腹血糖值和口服葡萄糖耐受試驗
2小時血糖值,均與心血管疾病有顯著相關且預 測能力相當 。綜合上述資料 ,不論是HbA1c 、 空腹血糖值、葡萄糖耐受試驗2小時血糖值或餐 後高血糖 ,臨床上都能用來評估糖尿病患者的 心血管疾病風險 。以心臟科醫師的立場來看 , 糖尿病藥物治療目的主要是希望能降低冠狀動 脈疾病的致死率或是罹病率 。若糖尿病病人以 飲食控制及運動仍無法有效的控制血糖時 ,應 早日考慮使用藥物治療 。接下來的內容將討論 各類型的降血糖藥物對心血管的相關風險 。
一 、磺醯尿素類(Sulfonylurea)
至 今 只 有 有 限 的 研 究 顯 示 降 血 糖 藥 物 能 降冠狀動脈疾病 。其中最經典的研究是英國的 UKPDS研究11,12,27,28。針對第二型糖尿病患者 , 使用磺醯尿素類降血糖藥物或胰島素嚴格控制 血糖 ,相對於飲食控制 ,在長期追蹤10年之 後 ,發現可顯著降低12%糖尿病相關併發症以 及小血管病變 ,但在大血管病變方面的改善卻 沒達到顯著水準 。不過源自丹麥的回溯性研究 發現 ,因為心肌梗塞住院但未接受冠狀動脈介 入治療的第二型糖尿病患者 ,相較於單純服用 Metformin ,除了Gliclazide之外的磺醯尿素類 藥物都可能增加心血管風險29;另一個源自加拿 大的回溯性研究發現 ,第一代磺醯尿素類藥物 或Glyburide劑量增加後,心血管事件相關的死 亡會增加 ,但Metformin劑量增加並不會30。值 得一提的是近期磺醯尿素類藥物的ADVANCE 研究與胰島素藥物的ORIGIN研究都沒顯示他們 可以顯著降低大血管病變17,31。在ADVANCE研 究中 ,針對第二型糖尿病的臨床試驗顯示 ,使 用Gliclazide併用其他降血糖藥物,嚴格控制血 糖使HbA1c降到6.5% ,在追蹤5年之後,相對 於一般控制(平均HbA1c7.3%) ,並不能顯著下 降主要心血管事件 、心因性死亡率以及總死亡 率 ,但顯著增加了低血糖的危險 。然而也許須 更久的追蹤來顯示這些新一代藥物的心血管保 護作用 。
二 、雙胍類(Biguanides)
源自UKPDS研究顯示12,27,對於過胖的第
二型糖尿病患者 ,使用Metformin追蹤10.7年之 後 ,相較於飲食控制 ,可以顯著減少32%的糖 尿病相關病變、42%的糖尿病相關死亡以及36%
的總死亡率;相較於磺醯尿素類或胰島素 ,可 以顯著減少糖尿病相關病變 、糖尿病相關死亡 以及中風的風險;當磺醯尿素類與Metformin併 用治療時 ,不會增加糖尿病相關死亡風險 。由 於雙胍類便宜又有效 ,比較沒有增加體重或低 血糖的情形 ,因此多數的臨床治療指引都推薦 其為糖尿病藥物的首選治療 。然而在臨床使用 上 ,Metformin應該避免使用於心臟衰竭、腎功 能不全 、肝功能異常 、代謝性酸中毒和急性疾 病 ,因為它可能會引起乳酸性酸中毒 。
三 、Meglitinides 類
主 要 是 包 括repaglinide以 及nateglinide , 目 前 沒 有 長 期 的 臨 床 試 驗 證 實Meglitinides是 否會增加心血管事件 。從丹麥的回溯性研究顯 示 ,磺醯尿素類的gliclazide以及Meglitinides類 的repaglinide,相較於Metformin用於治療第二 型糖尿病時 ,不管病人有無先前的心肌梗塞病 史 ,都不會增加總死亡率 、心因性死亡率或主 要心血管事件32。
四 、Thiazolidinedione (TZD) 類衍生物
新 一 代 藥 物 中Thiazolidinedione (TZD) 類 衍 生 物 在 心 血 管 疾 病 的 角 色 最 受 爭 議。Dr.
Nissen 在 2007 年 針 對 Rosiglitazone 所 做 的 整 合 分 析, 顯 示 其 會 顯 著 增 加43% 的心肌梗塞 以 及64% 的 心 因 性 死 亡 風 險33。Dr. Mannucci 在2010 年分析 164 個臨床試驗後的整合分析 發現,Rosiglitazone 會顯著減少 10% 的總死亡 率、不顯著減少6% 的心血管死亡率、不顯著 增加14% 的心肌梗塞和顯著增加 69% 的心臟 衰 竭34。 美 國 藥 物 食 品 管 理 局(U.S. Food and Drug Administration) 在 2010 年的 Rosiglitazone Cardiovascular Safety Meta-Analysis 中 顯 示,
Rosiglitazone 會顯著增加 80% 的心肌梗塞和 50%
的心血管死亡率35,而Dr. Nissen 在 2010 年更新 整合分析後顯示,Rosiglitazone 會顯著增加 28%
的心肌梗塞,但不會顯著增加心血管死亡率或
總死亡率。因為上述的整合分析並沒有一致性 的結論,造成Rosiglitazone 在 2010 年自歐洲下 架,但在美國以及部分國家的臨床適應症被限制 的情形。關於另一藥物Pioglitazone 的安全性,
在2005 年發表的 PROACTIVE 研究36,是一個 專門用來評估心血管事件以及死亡率的前瞻性 臨床試驗,顯示Pioglitazone 能顯著減少過去有 心血管病史的第二型糖尿病患者16% 的血管病 變,且不會增加整體死亡及事件率。在2007 年 發表的PROACTIVE 次分析顯示37,對於有心 肌梗塞病史的第二型糖尿病病患,Pioglitazone 可以顯著減少28% 的心肌梗塞、37% 的急性冠 心症以及19% 的大血管併發症,但也顯著增加 41% 的心衰竭住院,不過致死的心衰竭比率則與 安慰劑相當。在2007 年 Dr. Lincoff 的整合分析 中,Pioglitazone 可以顯著下降 8% 至 20% 的主 要心血管事件,但也顯著提高了41% 的心衰竭 危險38。在2008 年 Dr. Mannucci 發表的整合分 析中,當排除了PROACTIVE 研究之後顯示,
Pioglitazone 可以顯著下降 70% 的總死亡率,但 不顯著增加了38% 的非致死心衰竭39,也因為 具有一致性,Pioglitazone 目前仍可適用於糖尿 病治療。其實Thiazolidinedione 類的藥物都會增 加心衰竭的危險,因此臨床使用於心衰竭患者身 上是絕對禁忌症,也因為Thiazolidinedione 類衍 生物的事件,目前美國藥物食品管理局對最近新 上市的糖尿病藥物,都要求須有心血管疾病危險 性評估的縱向性研究。
五 、 阿爾發葡萄糖苷酶抑制劑(α-glucosidase inhibitors)
此類藥物主要是包括acarbose 、miglitol和 voglibose。α-glucosidase inhibitors在近端小腸抑 制澱粉和雙醣類的分解 ,進而延緩其吸收 ,可 降低餐後高血糖 ,但降血糖的效果不如磺醯尿 素類和雙胍類 ,單獨使用並不會發生低血糖 , 卻 可 能 會 有 脹 氣 或 腹 瀉 等 腸 胃 道 副 作 用 。 在 2003年STOP-NIDDM研 究 中40, 使 用acarbose 來治療葡萄糖耐受不良患者 ,相較於安慰劑 , 減少餐後高血糖可以顯著降低53%的主要心血 管事件風險 ,而這其中主要是來自於心肌梗塞

的顯著減少 。在2004年針對第二型糖尿病病患 的一篇整合分析中41,從7個臨床隨機試驗的 資料顯示 ,相較於安慰劑 ,使用acarbose可以 顯著減少64%的心肌梗塞以及35%的心血管事 件 。 然 而 在1999年UKPDS對 於acarbose的 前 瞻性研究42,以及在2005年一篇關於單獨使用 α-glucosidase inhibitors的整合分析中43,對於第 二型糖尿病病患 ,並沒有證據顯示對罹病率或 致死率有顯著影響 。總括來說 ,α-glucosidase inhibitors對於心血管的臨床預後尚無一致性的 結論 ,需要更多資料來佐證44。
六、 DPP-4 抑 制 劑 (Dipeptidyl Peptidase-IV Inhibitors) 與第1型類昇糖素胜肽(Glucagon- like peptide-1 analogues, GLP-1類)
這兩類都是近期研發的口服降血糖藥物 。 單獨使用GLP-1類並不會發失低血糖的危險而 且會減少體重 ,但可能會有腸胃道的症狀 ,不 過通常是發生在使用初期且後來症狀會緩解 。 2012年Dr. Sun發 表 的 整 合 分 析 中45, 從45個 GLP-1類臨床試驗的資料顯示並不會增加心血 管風險 。單獨使用DPP-4抑制劑不會發生低血 糖 , 對 體 重 也 沒 有 影 響 。2012年Dr. Monami 針對DPP-4抑制劑與血脂的整合分析發現46, 總膽固醇指數的顯著下降 ,可能有助於心血管 危險的減少 。這些藥物都有心血管疾病危險性 評估的縱向性研究 ,如TECOS研究(Sitagliptin, NCT00790205) 、SAVOR-TIMI研究(Saxagliptin, NCT01107886) 和 EXSCEL 研究 (Exenatide, NCT01144338)等。雖然這些大型的心血管疾病 安全性研究都還在進行中 ,但是近期小型的研 究顯示DPP-4與GLP-1類可能能夠改善一些心 血管疾病的監測指標(surrogate markers)47,然而 有結論性的證據仍然須等大型研究的發表 。
結論
至於到底血糖要降到多低才對冠狀動脈疾 病最好 ,近期ACCORD與VADT研究都不支持 要將HbA1C降至6.5%以下,這些積極降血糖 的研究都發現病人心血管疾病沒下降 ,但低血 糖的危險性與死亡率會上升16,18。ACCORD研
究的事後分析 ,甚至顯示有心血管疾病的病人 並沒有從積極降血糖獲得好處16,48;另一發現是 血糖控制越不好的糖尿病病患越容易發生低血 糖 ,與有無接受積極血糖控制無關 ,HbA1c降 幅較大並不會增加低血糖的風險 ,而可能增加 低血糖的危險因子包括:女性、高齡、低教育水 準 、週邊神經病變病史 、身體質量指數(Body Mass Index, BMI)較高、罹患糖尿病時間較長以 及使用胰島素 。
美 國 糖 尿 病 協 會(American Diabetes Association) 以及歐洲糖尿病研究協會 (European Association for the Study of Diabetes) 在 2012 年 提 出 第 二 型 糖 尿 病 血 糖 控 制 的 共 同 聲 明49, 強 調 以 病 人 為 中 心 的 方 式(A Patient-Centered Approach),著重個別化的治療 (individualization of treatment);對大多數的糖尿病病人來說,控 制HbA1c < 7%,空腹血糖值 < 130 mg/dl,餐 後血糖值< 180 mg/dl 即可;對於較年輕、糖尿 病罹病時間短或沒有心血管疾病的族群,若沒 有低血糖或其他副作用,則嚴格控制HbA1c 為 6.0-6.5%;對於有嚴重低血糖病史、高齡、嚴重 併發症、積極控制仍血糖控制不佳( 使用多重降 血糖藥物及胰島素) 或多重慢性疾病族群,則寬 容控制 HbA1c 為 7.5-8.0% 或稍高 ( 圖一 )。
由於HbA1c降到6.5%只對小血管病變有幫 忙 ,降低大血管病變的好處有限 ,因此以心臟 科醫師的立場處方降血糖藥物 ,就降低心血管

疾病的目的來看 ,對高心血管風險或已經確診 心血管疾病的糖尿病病患 ,採取傳統標準治療 至HbA1C < 7%即可。藥物的建議使用仍然是以 雙胍類為首選13,50,至於DPP-4抑制劑與GLP-1 類對心血管疾病是否有所助益 ,可能須看未來 幾年的研究而定了 。

Only limited studies have shown that anti-diabetic drugs could reduce the mortality and morbidity associ- ated with coronary artery disease (CAD), of which is the United Kingdom Prospective Diabetes Study (UKPDS) regarding with the use of biguanides, sulfonylurea, and insulin in the long-term follow-up. Thiazolidinedione (TZD) is controversial especially when Rosiglitazone associated with an increase in the risk of cardiovascular deaths. There is a lack of consistent conclusions on the cardiovascular outcomes for α-glucosidase inhibitors and further studies are needed. Dipeptidyl Peptidase-IV Inhibitor (DPP-4 inhibitor) and Glucagon-like peptide-1 (GLP-1) are developed recently, which are capable of improving some cardiovascular surrogate markers. ACCORD and VADT studies do not support the intensive sugar control with a target HbA1c of 6.5% or less, which may increase the risk of hypogly- cemia and mortality without any decrease in cardiovascular disease (CVD). Management of hyperglycemia should be based on a patient-centered approach with emphasis on individualization of treatment. To reduce CVD from a cardiologist's point of view, a simply target HbA1c of 7.0% is adequate for patients with high cardiovascular risk or diagnosed CVD and the biguanides is preferred. For DPP-4 and GLP-1, further research is required to evaluate the potential benefit in CVD.
