Endometriosis is high risk factor to female infertility, and these patients are usually the candidate of in vitro fertilization and embryo transfer (IVF-ET). Leuplin is GnRH agonist which is clinically used to treat endometriosis, central precocious puberty, prostate cancer as well as premenopausal breast cancer, and Leuplin is X class drug to pregnancy women. A 31-years-old female with severe dysmenorrhea was diagnosed with bilateral endometrioma in the gynecologic department. She accepted laparoscopic enucleation and started with leuplin injection for 6 doses for endometriosis recurrent prevention. After leuplin injection, the patient got hot flash, amenorrhea, and acne. Therefore, she went to our traditional Chinese medicine outpatient department for treatment for side effects accompanied with leuplin therapy. We performed syndrome differential diagnosis with blood stasis syndrome for core pathogenesis, and accompanied syndrome like dampness-heat syndrome or asthenis heat syndrome was changed with time. We treated this patient with Chinese herbal medicine major with Gui-Zhi-Fu-Ling-Wan powder for whole progress. After leuplin treatment course stopped, we changed the main treatment to Xiao-yao-San and prepared for pregnancy. And then the patient got pregnancy half a year later. We revealed this case with thinking process about traditional Chinese medicine pathogenesis of endometriosis, and discussed the treatment strategy among the difference between recurrence prevention by leuplin with fertility conservation. In conclusion, we try to manage a paradigm of translational medicine in traditional Chinese medicine with endometriosis.
