前言

自 十 九 世 紀 末 期 , 就 有 人 注 意 到 高 尿 酸 與高血壓、糖尿病、腎臟病與心血管疾病的相 關性。當時有些學者,如Frederick Mahomed, Alexander Haig, Nathan Smith Davis等,也曾假 設尿酸本身是高血壓與腎臟病的致病因子;不 過由於那時降尿酸藥物尚未問世,所以也沒有 相關研究來證實這樣的假設1

直到二十世紀中,尿酸與這些心血管疾病 的相關性才又再度被重視,同時也發現這樣的 相關性並不只是在高尿酸的情況下才存在(定義 為男性尿酸值大於7 mg/dL,女性尿酸值大於 6 mg/dL),甚至在那些尿酸值在正常範圍內但

稍高者(大於5.2-5.5 mg/dL)也有這樣的相關性。

但這相關性的臨床意義仍舊存在著爭議,例如 Framingham Heart Study Group便認為尿酸與心 血管疾病的相關性並不能與傳統危險因子相提 並論1

同樣地,在近10年內,高尿酸與腎臟病的 相關性也被廣泛地探討,高尿酸與腎功能惡化 的相關研究更如雨後春筍般的出現2,3。由於尿酸 會隨著腎功能下降而累積,臨床上此兩者的相 關性,究竟是高尿酸導致腎功能下降,抑或是 腎功能下降引起的尿酸升高,似乎是個「雞生 蛋或蛋生雞」的問題。而高尿酸到底是慢性腎 臟病的原因或是結果呢?到目前為止,已發表

摘 要

尿酸除了大家所熟知地會導致痛風外,近來的研究顯示它同時也與高血壓、糖尿病、腎 臟病與心血管疾病相關。而針對尿酸與腎臟病的研究指出,在腎功能正常的族群中,尿酸與 之後發生慢性腎臟病與進入透析的機率增加有關。但在已存在慢性腎臟病的族群,尿酸是否 會加速腎功能的惡化,尚待進一步的研究報告。本文就尿酸的代謝、生物特性、動物實驗及 流行病學,來探討尿酸與腎臟病之間的相關性。

的研究論文似乎仍存在著爭論性,在此,就將 相關研究做一整理,以釐清尿酸與慢性腎臟病 的關係。

尿酸的代謝

「尿酸」(uric acid)是一種弱酸,在人體生 理的酸鹼值下,是以離子化形式,尿酸鹽(urate),

而存在。現代人類血中尿酸值較其他哺乳類高 出許多,這是因為人類腎臟能非常有效率地再 吸收尿酸;而且,在演化的過程中,人類肝臟 中一種能代謝尿酸的氧化 ,uricase,逐漸失 去它的功能。隨著尿酸濃度在體液中的增加,

它會過度飽和而形成結晶。造成高尿酸的原因 主要有︰尿酸過度製造與尿酸排泄過少;前者 約占10%,後者約占90%;少數可能兩種情況都 有4

尿酸主要是由嘌呤代謝而來,如圖一,而

嘌呤的前驅物不外乎是外源性,由飲食而來;

或是內源性,由細胞代謝而來。每天飲食中嘌 呤的攝取量與血中尿酸濃度有很大的相關性;

譬如說,只要幾天完全不吃含嘌呤的食物,血 液尿酸值就可以從5.0 mg/dl降至3.0 mg/dL5,6;的 確,有研究資料指出,若將男性依每天肉類與 海鮮的攝取量分成四組,肉類攝取量最高的那 一組較肉類攝取量最低的那一組,增加了41%痛 風的機會,而海鮮類攝取量最高的那一組較海 鮮類攝取量最低的那一組,增加了51%痛風的機 會7。而內源性的嘌呤代謝產生的尿酸,約90%

發生於一些增生性或發炎性疾病,譬如血液學 的癌症或牛皮癬等,或使用一些化療藥物,這 些情況都會造成細胞代謝速率增快而過度製造 尿酸,其他少部分(約10%)是因為嘌呤代謝過程 中的酵素活性異常所造成。

在腎臟尿酸的運送分成四部分︰由於尿酸

是小分子,所以先在腎絲球被百分之百完全地 濾過,接著在近端腎小管幾乎完全地被吸收,最 後在較末端的近端腎小管被分泌與分泌後被再 吸收。之前提過,人類腎臟再吸收尿酸是非常 有效率的,在腎絲球被濾出的尿酸中,約有90%

再吸收,最後從尿液中排出的尿酸,只有10%。

而 在 近 端 腎 小 管 中 , 有 許 多 的 輸 送 體(tran- sporter)與尿酸的再吸收有關;其中最重要的是 位在腎小管細胞頂側的urate-anion exchanger (URAT1),大約回收了近50%的尿酸,它同時也 是我們熟知的促尿酸排泄藥物,如probenecid、

benzbromarone等作用的地方8

尿酸同時有抗氧化與促氧化的特性

遠在中新世的人類祖先就喪失了自行合成 維生素C的能力,而幾乎也在這段時間,人類的 uricase也失去了它的功能9。在1970年時Proctor 就曾提出假設認為尿酸與維生素C一樣具有抗氧 化的功能,在演化的過程中,尿酸可能用以代 替維生素C的抗氧化作用10。而在1980年之後,

陸續有許多研究確定了尿酸抗氧化的作用,包 括了它能清除hydroxyl radicals, singlet oxygen11與 peroxynitrite12等,同時並能箝合鐵並阻斷鐵所催 化的氧化反應13。此外,尿酸可以維持體內許多 抗氧化系統的功能,譬如它可以防止由過氧化 氫導致的細胞外superoxide dismutase (SOD)不活 化14,預防維生素C的氧化15;在暴露於peroxynit- rite的內皮細胞中,阻止tetrahydrobiopterin的 氧 化 反 應16。此外,尿酸可回饋抑制xanthine oxidase的活性17。Wayner比較了人類血漿抗氧化 系統中,尿酸、血漿蛋白、維生素C、維生素E 等的抗氧化能力,發現尿酸就負責了約35-65%

的抗氧化作用18。而這樣抗氧化的保護作用也在 一些神經科的疾病被提出,例如多發性硬化症 與帕金森氏症1

但很有趣地,尿酸在某些情況下反而成了 前氧化劑(pro-oxidant)。例如,尿酸在與pe- roxynitrite作用時,會產生許多自由基19,20。尿酸 在有氧化劑存在的狀況下,會刺激NADPH氧

21,22。所以,尿酸的氧化或抗氧化作用的表

現,取決於周遭環境的不同。而Sautin與Johnson

認為在血漿中的尿酸主要是抗氧化劑,而在細 胞中的尿酸則大部分是前氧化劑22

尿酸在動物實驗中對腎臟的影響

在 腎 功 能 正 常 的 大 鼠 , 尿 酸 會 造 成 腎 臟 病的形成;而在腎功能異常的大鼠,尿酸會促 進腎臟病的惡化。在病理切片方面,高尿酸造 成的損害主要有腎絲球硬化、腎間質纖維化、

腎小動脈的變化;這些變化與「痛風腎病變」

(gouty nephropathy)類似,唯一不同的是,在切 片上看不到尿酸結晶的形成。

而造成這種腎病變的機制是甚麼呢?在腎 功能正常,給予餵食oxonic acid (uricase的抑制 劑)以抑制uricase而使血中尿酸升高的大白鼠,

較沒有餵食oxonic acid或餵食oxonic acid但同時 投與allopurinol來降尿酸的大白鼠,前者的收縮 壓與腎絲球微血管壓較後兩者來得高,而且腎 小動脈壁也較厚23

另外,Kang DH則是在實施腎臟六分之五 切除的慢性腎衰竭模式大白鼠上研究尿酸的影 響,而投與oxonic acid的高尿酸組別較對照組而 言,有較高的血壓、較嚴重的蛋白尿與較高的 肌酸酐值。同樣地,高尿酸組的腎絲球硬化與 腎間質纖維化都較嚴重,同時腎絲球前小動脈 也有動脈壁增厚並伴隨著平滑肌細胞增生的現 象。此外,大白鼠血中尿酸值的高低與腎臟腎 素(renin)與cyclooxygenase-2 (COX-2)的表現量 成正比,之前已有實驗證實經由angiotensin II所 引 起 的 血 管 平 滑 肌 增 生 是 由C O X - 2 產 生 的 thromboxane (TX-A2)所媒介,而Kang DH將血 管平滑肌細胞與尿酸一起培養,也證實了以上 的結論。所以,Kang DH認為在慢性腎衰竭的大 白鼠模式上,高尿酸引起的腎功能惡化,可能 是經由高血壓與COX-2媒介的血管增生24

Nakagawa T也發現類似的結果,在高尿酸 血症的腎功能正常大鼠,尿酸在不形成結晶沉 澱的情況下,本身就能造成高血壓、腎絲球肥 大 與 腎 絲 球 高 壓 。 而 在 腎 臟 六 分 之 五 切 除 與 cyclosporine腎病變的大鼠,高尿酸者會有較嚴 重的病理表現25,26。而Sanchez-Lozada LG更針對 尿酸對單一腎元腎絲球過濾速率 (single nephron

glomerular filtration rate)的影響做出量化,在正 常腎功能與實施六分之五切除的高尿酸血症大 鼠,都出現腎皮質血管收縮與腎絲球高壓,而 相較於正常尿酸的大鼠,其單一腎元腎絲球過 濾速率下降的幅度,在前者是35%,而在後者是 40%27

但是也有報告指出,高尿酸血症在腎臟小 動脈造成類似高血壓時的硬化表現,是與血壓 無關的;在高尿酸血症大鼠,用利尿劑將血壓 降至與對照組血壓相同,仍會出現腎臟小動脈 中層增生,有趣的是,如果用血管收縮素轉化 抑制劑 (angiotensin converting enzyme inhibitor, ACE inhibitor)來降血壓,就不會出現上述狀況,

所以,尿酸本身可能可以不經由高血壓的影響,

直接經由腎素-血管收縮素系統(renin-angiotensin system)來導致腎小動脈病變28

除了上述的腎素-血管收縮素系統,尿酸也 可導致內皮細胞的功能失常而使一氧化氮(nitrite oxide, NO)的產量下降,因一氧化氮本身對血管 平滑肌細胞有抗增生的作用,所以尿酸也可經 由使一氧化氮的產量下降而造成小動脈血管病 變。的確,Sanchez-Lozada LG在高尿酸血症的 大鼠投予五周的L-arginine (substrate for en- dothelial nitric synthase)使尿液中的NO2-/NO3-上 升,成功地使血壓下降,並使腎皮質血管收縮 減緩29

尿酸與慢性腎臟病危險因子的關係

目前已知,尿酸與許多慢性腎臟病危險因 子都有相關,如高血壓、心血管疾病、代謝症 候群與糖尿病等;其中研究最多,證據最充分 的便是高血壓。根據大型的回顧性研究指出,

高尿酸血症的高血壓,在早期是與尿酸有關而 與鈉鹽較無關,但一旦在小動脈病變出現之後,

高 血 壓 便 與 尿 酸 無 關 而 與 鈉 鹽 相 關(sodium- dependent)了1,30。而許多流行病學的研究也暗示 高尿酸與早期高血壓的形成有關。其中一篇包 括30位高血壓與高尿酸成人的雙盲研究指出,

使用allopurinol降低尿酸的確可使血壓降低,其 降幅與降壓藥相等;而在尿酸值降至5mg/dL以 下後,約有86%的受試者血壓恢復正常,而安慰

劑組只有3%的受試者血壓恢復正常31

高尿酸導致腎臟傷害的假說

由目前已知的證據顯示,尿酸可能經由直 接或間接的方式傷害腎臟,如上述動物實驗,尿 酸升高可能直接造成腎臟的腎素上升,一氧化 氮下降,腎間質及入球小動脈的病變,而這些 又會惡化血壓。再者,尿酸升高也與高血壓、

糖尿病、心血管疾病等慢性腎臟病的危險因子 有關,可間接經由這些危險因子導致腎功能的 惡化1,32(如圖二)。

尿酸在正常腎功能族群中與慢性腎 臟病相關

在 針 對 正 常 腎 功 能 族 群 的 流 行 病 學 研 究 顯示,高尿酸是造成慢性腎臟病的危險因子之 一33。它可以在高血壓前期但腎功能正常的患者 引發微蛋白尿34,或在無蛋白尿的第一型糖尿病 患者造成腎功能惡化35;而在腎功能正常的一般 族群則與慢性腎臟病相關,且增加末期腎衰竭 的機率。

Iseki在琉球一個包括6403人的研究發現,

在一開始有正常肌酸酐的族群,在追蹤2年後,

上升≧1.4 mg/dL,女性上升≧1.2 mg/dL);尿酸 值高於8 mg/dL較低於5 mg/dL者,於男性和女性 族群分別增加了2.91與10.39的機率使肌酸酐上 升36。而Iseki另一個包括48177人的研究中指出,

血中尿酸值大於6.0mg/dL的女性較小於6.0mg/dL 的女性,多出5.77倍進入末期腎衰竭的機率。但 在男性則無顯著意義37

另一個針對49413個日本男性鐵路工人的研 究發現,高尿酸者(≧8.5 mg/dL)較正常尿酸 (5-6.4 mg/dL)者,除了有較高的機率造成腎衰竭 外,也有較高的總死亡率、與心血管疾病、腦 中風、及肝病的機會38。而在北加州一個包括17 萬人左右的研究發現,除了傳統上用來預測進 入末期腎衰竭的因子,譬如男性、老年、蛋白 尿、糖尿病等等,非傳統性因子,如尿酸、貧 血、夜尿、家族史等也可用來預測進入末期腎 衰竭;若在一般族群中依尿酸值的高低將族群 分成四等份,尿酸最高組較最低組多出2.14倍進 入末期腎衰竭的機率39。另一個在泰國針對某電 力公司員工的研究也指出高尿酸(>6.29 mg/dL) 是腎功能降低(GFR<60 ml/min)的危險因子40

尿酸在慢性腎臟病族群是否能造成 腎功能惡化

由 於 隨 著 腎 功 能 惡 化 , 尿 酸 像 尿 毒 素 或 鉀、磷等電解質一樣會累積,所以究竟是腎功 能惡化導致尿酸累積,抑或是高尿酸引起腎功 能惡化,一直頗受爭議。Madero M針對838個第 三、四期的慢性腎臟病患者,平均追蹤了約10 年,發現若將這些患者依血中尿酸值分成三等 份之族群,尿酸最高組較最低組增加了各種原 因造成死亡(all cause mortality)的風險,而且有 增加心血管疾病死亡率(cardiovascular disease mortality)的傾向,但是並不增加需要洗腎或腎 移植的機率。Madero M認為這是因為在慢性腎 臟病患者,腎臟本身已有了一些結構性的損害,

這時候尿酸的致病性就沒那麼重要了41

但是,有些研究指出,治療慢性腎臟病患 者無症狀的高尿酸血症,可以延緩腎臟病的惡 化。Siu YP在第三期慢性腎臟病的患者用allo- purinol降低尿酸,追蹤了12個月,發現使用

allopurinol降尿酸的那組,肌酸酐惡化的速度較 不使用組來得較慢。但值得注意的是,這樣的 結果究竟是使用了allopurinol降低了尿酸所導致 的結果抑或是減少了xanthine oxidase相關的氧化 物,仍待進一步的研究3。此外,在芬蘭針對IgA nephropathy的慢性腎衰竭族群,追蹤了0.2至17 年的報告指出,高尿酸和高三酸甘油脂與腎功 能的惡化有關42

綜合以上所述,高尿酸是否會使慢性腎臟 病患者的腎功能加速惡化,似乎仍未有一致的 結論。

結論

高尿酸與高血壓、糖尿病、心血管疾病等,

已成為現今社會的文明病之一。而慢性腎臟病 的預防與治療亦是已開發國家關心的課題。根 據上述已發表的研究顯示,高尿酸在正常腎功 能的一般族群,的確增加了之後發生慢性腎臟 病與進入透析的機會,但是目前並沒有研究顯 示在腎功能正常族群,積極使用藥物降低尿酸 是否有所助益,基於降尿酸藥物的副作用如史 蒂芬強生症候群,導致的臨床症狀較嚴重,所 以,對於無症狀的高尿酸血症患者是否需投與 藥物,仍待進一步的研究報告。而尿酸在慢性 腎臟病族群究竟是否有加速腎功能惡化的作用,

到目前已發表的論文仍未有一致的結論,尿酸 在已發生結構性變化的腎臟,是否像對健康腎 臟一樣具有傷害,可能需要更多證據證明。

Uric acid is well-known for its causality of gout. Recent researches also show it relates to hypertension, diabetes mellitus, renal disease and cardiovascular disease. Studies focused on uric acid and renal disease point out that uric acid does increase risk of chronic kidney disease and end stage renal disease in general population.

However, in CKD group, it needs further report to see if uric acid deteriorates renal function. This article explores relation between uric acid and renal disease in view of its metabolism, biologic characters, animal studies and epidemiology. ( J Intern Med Taiwan 2010; 21: 197-203 )
