前言

第九單元主題為精神醫學和行為科學 (Psychiatry and Behavioral Science Unit),主要是企圖運用整合性的教案及演講課程,讓同學能對 精神醫學和行為 科學其主要的生物性(Biological)、心理性(Psychological)、以及社會性(Social)評 估、處置、甚至預防精神疾病的重要原則有一初步性的完整了解。由於教案和演 講課程各有其重要性與特色,所以在此單元中將教案及演講課程整體規劃設計、

交互進行,有其互補、增強學習效果的考量。

在結束四年級的課程後,各位即將進入臨床實習,面對求診病患的精神狀 態評估、精神疾病診斷、以及完整的治療計劃,不僅需要系統性的臨床知識及技 巧,也要藉由查閱書籍、文獻,掌握最新的醫學正確概念;當然,對於腦科學和 行為科學的認識也是不可少的。所以本單元就是在如此的考量下,藉由臨床案例 和主題式演講,企圖架構起同學對於「基礎醫學」、「臨床醫學」的並重研習契機

「心理影響生理,生理影響心理」,精神醫學和行為科學不僅是學習豐富的 腦部知識,更是學習用敏銳、同理的觀察與思維,去體會病患以及家屬「身心之 苦」!願各位同學藉由此單元,開啟對「既是科學、又是藝術」的精神醫學的窺門 之妙感受!

課程目標 其內容如下:

Biological Perspective

1. Brain structures related to the control of autonomic function

2. Basic science of sleep and its roles in different psychiatric disorders.

3. Neurotransmitters associated with mental disorders 4. Lesions in the brain related to the mental disorders 5. Mechanisms and etiologies of auditory hallucination 6. Mechanism of common mental disorders

7. Genetic roles associated with mental disorders

8. Pharmacology of anti-psychotic drugs and drugs used in mental diseases 9. Biology of addiction (such as neuroanatomy of drug reward, molecular

targets of abused drugs, mechanism of adaptation to chronic drug use) 10.The pharmacology of CNS-stimulants and related drugs. The neural basis

and development of attention

11.Define the milestone on social development

12.The etiologies of autistic disorder (psychological, biological, genetic, neurochemistry…etc)

13.Describe normally psychosexual development

Clinical Perspective

1. Cognitive function

2. The diagnosis and differential diagnosis of common mental disorders (such as anxiety disorders, mood disorders, personality disorders, schizophrenia, pervasive developmental disorders, somatoform disorders, and gender identity disorders)

3. The measurement of attention and the definition of attention deficit.

4. The measures of intelligence quotient (IQ) and its interpretation

5. Describe the development of social skills, communicative skills in a child.

6. Emotional and behavioral changes during substance abuse and dependence

7. Medical complications and sequences of substance abuse

8. Manage substance (including morphine and CNS stimulants) intoxication and substance withdrawal.

9. The treatment for patients with schizophrenia and other psychotic disorders

10.The treatment for patients with common non-psychotic disorders

11.The treatment for patients with cognitive disorders( dementia, delirium, amnesia)

12.The role of biofeedback and cognitive-behavior therapy in treating mental disorders

Population and Community Health Perspective

1. The incidence and prevalence of common mental disorders 2. The risk factors of mental disorders

3. Prevalence of substance abuse disorder and related criminal incidence 4. Social complication of substance abuse disorder and prevention strategy.

5. Community health strategy to prevent substance abuse disorders.

6. The conditions causing learning disability.

7. Family and community attitude toward mental disorders and suicide

Psychosocial and Ethical Perspective

1. Ethical an law issues of managing a patient with mental disorder

2. The influence of mental disorder on one’s life quality and interpersonal interaction

3. The method to build up the patient’s insight about his psychotic disorder 4. Discuss the both psychological and social impact of a patient with a mental

disorder and his (her) family.
