壹、前言

第 九 單 元 主 題 為 精 神 行 為 及 重 症 單 元 (Unit of Psychiatry, Behavioral Science, and Critical Diseases)。精神行為的部份,主 要是企圖運用整合性的教案及演講課程,讓同學能對精神醫學和行 為科學其主要的生物性(Biological)、心理性(Psychological)、以及 社會性(Social)評估、處置、甚至預防精神疾病的重要原則有一初 步性的完整了解。重症單元的部份,教學目的在於讓正式進入臨床 前之同學們,提早接觸急重症個案,著重於生命徵象之評估以及臨 床上對於重症病患之處理步驟和邏輯。由於教案和演講課程各有其 重要性與特色,所以在此單元中將教案及演講課程整體規劃設計、

交互進行,有其互補、增強學習效果的考量。

在結束四年級的課程後,各位即將進入臨床實習,面對求診病 患的疾病嚴重度、處理之急迫性、精神狀態評估、精神疾病診斷、

以及完整的治療計劃,不僅需要系統性的臨床知識及技巧,也要藉 由查閱書籍、文獻,掌握最新的醫學正確概念;當然,對於腦科學 和行為科學的認識也是不可少的。所以本單元就是在如此的考量 下,藉由臨床案例和主題式演講,企圖架構起同學對於「基礎醫 學」、「臨床醫學」的並重研習契機!

「心理影響生理,生理影響心理」,精神醫學和行為科學不僅 是學習豐富的腦部知識,更是學習用敏銳、同理的觀察與思維,去 體會病患以及家屬「身心之苦」!另外,重症單元的教學也會讓同 學體會到,急重症病患可能會出現在任何科別,是不分專科的,每 一位醫師都必須有評估與處置重症病患之基本觀念。願各位同學藉 由此單元,開啟對「既是科學、又是藝術」的臨床醫學的窺門之妙

參、課程目標

精神行為和重症單元的學習目標分四類,其內容如下:

Biological Perspective

1. Brain structures related to the control of autonomic function 2. Basic science of sleep and its roles in different psychiatric

disorders.

3. Neurotransmitters associated with mental disorders 4. Lesions in the brain related to the mental disorders 5. Mechanisms and etiologies of auditory hallucination 6. Mechanism of common mental disorders

7. Pharmacology of anti-psychotic drugs and drugs used in mental diseases

8. Biology of addiction (such as neuroanatomy of drug reward, molecular targets of abused drugs, mechanism of adaptation to chronic drug use)

9. The pharmacology of CNS-stimulants and related drugs.

10. The nervous systems effects of altered calcium concentration 11. The mechanisms of calcium regulation

Clinical Perspective

1. Cognitive function

2. The diagnosis and differential diagnosis of common mental disorders (such as schizophrenia, mood disorders, substance dependence, personality disorders, and anxiety disorder) 3. Psychiatry of infancy, child, aldolescence, and late-life

4. Emotional and behavioral changes during substance abuse and dependence

5. Medical complications and sequences of substance abuse 6. Manage substance (including morphine and CNS stimulants)

intoxication and substance withdrawal.

7. The treatment for patients with schizophrenia and other psychotic disorders

8. The treatment for patients with common non-psychotic disorders

1. Ethical and law issues of managing a patient with mental
