本研究顯示在四個肩關節方向的最大伸展角度時加上局部震動訓練
(40Hz,每個方向一分鐘重複三次,共 12 分鐘的訓練),可以立即提升肩
關節活動度,並降低休息時及動作時疼痛。然而肩周圍肌肉的張力及肩 關節位置的本體感覺並無明顯變化,顯示局部震動訓練增加五十肩患者 的肩關節柔軟度的機轉可能主要為減少疼痛閾值。而五十肩的關節活動 度受限主要病因仍為關節囊攣縮,肩周圍肌肉緊繃的肌肉機械性質影響 不大。至於震動可能刺激高爾基氏腱(Golgi tendon organs)促進肌肉放 鬆,或經由 Ia 抑制神經元(Ia-inhibitory neuron)抑制拮抗肌收縮等影 響肌肉神經控制的效果,本研究設計未做這方面的資料收集,後續研究 可以加上肌肉活性的測量,以了解震動增加柔軟度的神經控制機轉。
本研究顯示局部震動伸展訓練對五十肩關節攣縮患者有立即而顯著
的幫助,提供臨床工作者一個方便省時也節省人力的新治療方法。震動
訓練的長期效果也令人期待,有待進一步的後續研究。
