摘 要
本 通 函 旨 在 通 知 各 中 學 日 校 、 小 學 、 特 殊 學 校 和 幼 稚 園 有 關 在 2 0 2 2 / 2 3 學 年 為 日 校 學 生 提 供 學 生 津 貼 的 安 排 。
背 景
2 . 財 政 司 司 長 於 2 0 1 9 年 8 月 公 布 的 1 9 1 億 元 紓 困 措 施 當 中 ， 建 議 在 2 0 1 9 / 2 0 學 年 為 中 學 日 校 、 小 學 和 幼 稚 園 學 生 提 供 一 次 性 2 , 5 0 0 元 的 學 生 津 貼 ， 以 減 輕 家 長 在 教 育 支 出 方 面 的 財 政 負 擔 。 這 項 措 施 已 於 2 0 1 9 年 1 2 月 6 日 獲 得 立 法 會 財 務 委 員 會 批 准 。 根 據 行 政 長 官 2 0 1 9 年 《 施 政 報 告 》 ， 這 項 津 貼 已 由 2 0 2 0 / 2 1 學 年 起 恆 常 化 。
資 格 準 則
3 . 學 生 津 貼 不 設 資 產 審 查 。 所 有 於 申 請 日 在 香 港 提 供 本 地 或 非 本 地 課 程 的 中 學 日 校 、 小 學 和 特 殊 學 校（ 包 括 公 營 學 校 、 直 接 資 助 計 劃 學 校 、 英 基 學 校 及 私 立 學 校 ）及 幼 稚 園（ 不 論 是 否 參 加 幼 稚 園 教 育 計 劃 ）就 讀 的 學 生 1 皆 可 獲 得 有 關 津 貼 ， 但 夜 校 學 生 、 自 修 生 、 持 學 生 簽 證 來 港 就 學 及 持 有 入 境 事 務 處 發 出 擔 保 書 （ 坊 間 一 般 稱 為 「 行 街 紙 」 ） 的 學 生 則 不 屬 資 助 範 圍 。
4 . 考 慮 到 部 分 學 生 理 應 符 合 上 述 第 3 段 的 條 件 但 因 特 殊 情 況 而

( b ) 就 讀 於 由 教 育 局 資 助 的 全 日 制 啟 動 課 程 的 新 來 港 兒 童 ； 以 及
( c ) 就 讀 職 業 訓 練 局 為 中 三 離 校 生 而 設 的 全 日 制 課 程 的 學 生 。
派 發 申 請 表 格 及 相 關 安 排
5 . 教 育 局 會 透 過 學 校 派 發 「 學 生 津 貼 」 申 請 表 格 及 核 實 學 生 身 份 。 申 請 表 格 分 為 表 格 B （ 樣 本 見 附 錄 一 ） 和 表 格 A （ 樣 本 見 附 錄 二 ） 。 表 格 B 已 預 印 學 生 及 申 請 人 的 基 本 資 料 （ 原 校 就 讀 的 學 生 適 用 ） ； 表 格 A 屬 空 白 表 格 （ 本 學 年 取 錄 的 新 生 或 轉 讀 他 校 等 學 生 適 用 ） 。 學 校 請 提 示 家 長 ／ 監 護 人 按 照 下 列 情 況 填 寫 申 請 表 －
( 甲 ) 表 格 B ：
一 般 情 況 下 ， 家 長 ／ 監 護 人 只 須 核 對 所 載 資 料 正 確 無 誤 。 如 預 印 資 料 沒 有 需 要 更 改 ， 便 可 在 表 格 底 部 的 確 認 方 格 加 上
「 ✓ 」 號 ， 並 簽 署 確 認 ， 透 過 學 校 送 交 本 局 辦 理 。
如 表 格 B 所 列 部 分 資 料 需 要 更 新 （ 學 生 英 文 姓 名 、 日 校 名 稱 及 學 校 類 別 以 外 的 資 料 ）， 家 長 ／ 監 護 人 請 在 有 關 資 料 的 上 方 位 置 用 黑 色 或 藍 色 原 子 筆 以 正 楷 作 出 修 正 （ 請 勿 使 用 塗 改 液 或 改 錯 帶 ）， 並 留 空 表 格 底 部 的 確 認 方 格 ， 再 透 過 學 校 送 交 本 局 辦 理 。

新 入 學 學 生、轉 讀 他 校 的 學 生、個 別 沒 有 獲 教 育 局 提 供 表 格 B 的 學 生 ， 或 需 要 修 改 主 要 資 料 （ 即 學 生 英 文 姓 名 、 日 校 名 稱 或 學 校 類 別 ） 的 學 生 ， 家 長 ／ 監 護 人 需 填 寫 表 格 A 申 請 。

6 . 請 學 校 於 2 0 2 2 年 9 月 2 0 日 至 1 0 月 7 日 派 員 帶 同 填 妥 的 「 領 取 表 格 」（ 附 錄 三 ） 到 所 屬 區 域 教 育 服 務 處 領 取 「 學 生 津 貼 」 申 請 表 格 和 「 通 知 書 」 （ 用 以 顯 示 交 回 的 申 請 表 格 數 目 ， 樣 本 見 附 錄 四 ） ， 並 於 2 0 2 2 年 9 月 2 6 日 起 將 申 請 表 格 派 發 予 合 資 格 學 生 的 家 長 。 表 格 A 亦 可 於 教 育 局 網 頁 下 載 （ 主 頁 > 學 生 及 家 長 相 關 > 支 援 及 資 助 > 學 生 津 貼 ） 或 在 各 區 域 教 育 服 務 處 索 取 。 請 注 意 ， 填 妥 的 申 請 表 格 必 須 透 過 有 關 學 生 就 讀 的 學 校 提 交 ， 而 每 名 合 資 格 學 生 只 可 提 交 一 份 申 請 。 申 請 人 在 填 寫 「 學 生 津 貼 」 申 請 表 前 ， 請 先 細 閱 表 格 內 的 「 須 知 事 項 」 及 「 聲 明 」 。
7 . 學 校 須 於 每 份 填 妥 的 申 請 表 格 蓋 上 學 校 印 鑑 ， 以 確 認 該 學 生 為 貴 校 學 生 ， 並 於 2 0 2 2 年 1 0 月 6 日 至 1 1 月 4 日 期 間 ，將 申 請 表 格 連 同
「 通 知 書 」， 一 併 送 交 教 育 局 特 別 職 務 辦 事 處 學 生 特 別 支 援 組 觀 塘 辦 事 處 （ 地 址 ： 九 龍 觀 塘 觀 塘 道 4 1 0 號 觀 點 中 心 1 0 樓 1 0 0 1 - 3 室 ） 。 如 個 別 學 生 未 能 在 指 定 限 期 前 提 交 申 請 ， 學 校 可 分 批 將 已 填 妥 的 申 請 表 格 及 「 通 知 書 」 交 回 教 育 局 。 如 學 生 在 2 0 2 2 年 1 1 月 4 日 以 後 入 學（ 例 如 新 來 港 兒 童 ）， 而 入 學 日 期 在 2 0 2 3 年 6 月 3 0 日 或 以 前 ， 仍 可 申 請 這 項 資 助 ， 如 有 關 學 生 是 轉 校 生 ， 他 們 可 透 過 原 校 或 新 入 讀 學 校 申 請 這 項 津 貼 ， 但 不 可 重 複 申 請 。
8 . 申 請 人 在 現 階 段 無 須 提 交 其 他 文 件 ， 我 們 會 儘 量 利 用 現 有 學 校 、 學 生 及 申 請 人 的 資 料 進 行 核 實 工 作 ， 有 需 要 時 才 要 求 申 請 人 提 交 其 他 文 件 。 教 育 局 會 適 時 透 過 短 訊 和 電 郵 ， 讓 家 長 知 悉 申 請 的 進 度 。 由 於 家 長 一 般 擁 有 本 地 銀 行 戶 口 ， 我 們 會 透 過 銀 行 轉 帳 向 家 長 發 放 津 貼 。
9 . 為 方 便 學 校 就 學 生 津 貼 事 宜 與 學 生 家 長 ／ 監 護 人 溝 通 ， 教 育 局 會 透 過「 學 生 津 貼 系 統 」（ h t t p s : / / s t g s f i l e . e d b . g o v . h k ）定 期 向 學 校 發 放 學 生 津 貼 申 請 的 進 度 報 告 。 如 申 請 表 格 內 的 資 料 不 完 整 或 不 正 確 ， 或 本 局 需 要 其 他 資 料 以 核 實 學 生 的 申 請 資 格 ， 我 們 會 透 過 學 校 要 求 申 請 人 提 交 所 需 資 料 。 學 校 須 以「 電 子 化 服 務 入 門 網 站 」 學 校 行 政 主 戶 戶 口 或 「 統 一 登 入 系 統 」 學 校 行 政 戶 口 ， 經 「 統 一 登 入 系 統 」 進 入 「 學 生 津 貼 系 統 」 獲 取 進 度 報 告 及 其 他 文 件 。 學 校 亦 可 透 過「 統 一 登 入 系 統 」的 功 能 ， 委 派 校 內 教 職 員 協 助 處 理 有 關 津 貼 申 請 的 事 宜 。

1. 本表格用作申請 2022/23 學年「學生津貼」。學生必須在 2022/23 學年就讀中學日校、小學、特殊學校或幼稚園。
This application form (“Form”) is for application for Student Grant for the 2022/23 school year. The student must attend secondary day school, primary school, special school or kindergarten in the 2022/23 school year.
2. 學生必須是香港居民。只持有學生簽證或入境事務處發出的擔保書（坊間一般稱為「行街紙」）的學生均不符合資格。
The student must be a Hong Kong resident. Students holding student visas only or recognizance forms issued by the Immigration Department are not eligible.
3. 申請人必須是學生的父母或監護人。
The applicant must be the parent or guardian of the student.
4. 如本表格所列資料需要更新（學生英文姓名、日校名稱及學校類別除外），請在有關資料的上方位置用黑色或藍色筆以正楷作出修正。請勿使用塗改液或改錯帶。如學
生英文姓名、日校名稱或學校類別需作更改，請使用表格A。建議申請人備存一份填妥的表格以供日後參考。
If the pre-printed information on this Form requires updating (except Student’s Name in English, Name of Day-school and School Type), please provide the amendment in the space above the relevant information in BLOCK letters using black or blue pen. Please do not use correction fluid or tapes. If Student’s Name in English, Name of Day-school or School Type requires amendment, please use Form A. It is advisable for the applicant to keep a copy of the completed Form for future reference.

You have the right to request access to and correction of your personal data held by EDB. I agree that the payment under such scheme(s) be effected directly into the bank account as specified in this Form. I also agree and undertake to notify the Government forthwith of any overpayment or any payment by mistake to me under such scheme(s) and refund the same to the Government. I declare that the student is enrolled in the day-school as specified in Part I of this Form; the student satisfies the eligibility criteria for such scheme(s), and he/she is neither a holder of student visa for entry into Hong Kong for studies, nor a holder of recognizance form issued by the Immigration Department. I understand that I will not receive payment under such scheme(s) in case the student is ineligible.
8. 本人明白只有其中一位父母或監護人可為每一名合資格學生提交一份申請。本人聲明本人已與本人的配偶及其他監護人（如適用）核實，並沒有就填報的學生重複遞交申請；亦未曾在有關的學年在該等計劃
下就填報的學生遞交申請或領取款項。如申請人為同一名學生重複遞交申請，其後重複申請將不獲處理。

1. 本表格用作申請 2022/23 學年「學生津貼」。學生必須在 2022/23 學年就讀中學日校、小學、特殊學校或幼稚園。
This application form (“Form”) is for application for Student Grant for the 2022/23 school year. The student must attend secondary day school, primary school, special school or kindergarten in the 2022/23 school year.
2. 學生必須是香港居民。只持有學生簽證或入境事務處發出的擔保書（坊間一般稱為「行街紙」）的學生均不符合資格。
The student must be a Hong Kong resident. Students holding student visas only or recognizance forms issued by the Immigration Department are not eligible.
3. 申請人必須是學生的父母或監護人。
The applicant must be the parent or guardian of the student.
4. 請用黑色或藍色筆以正楷填寫本表格。請勿使用塗改液或改錯帶。建議申請人備存一份填妥的表格以供日後參考。
Please complete this Form in BLOCK letters using black or blue pen. Please do not use correction fluid or tapes. It is advisable for the applicant to keep a copy of the completed Form for future reference.
5. 申請人於填寫及簽署本表格時，即表示申請人同意提供第 I 和第 II 部分所需資料以及第 III 部分的聲明。如申請人未能提供所需資料，可能會令其申請延遲或無法處理。

The personal data provided by you in this form will be used by the EDB for one or more of the following purposes:
(a) 處理、核實、審核資格及查證各項補助和津貼，以及由教育局提供的教育服務的申請；
Activities relating to the processing, authentication and assessment on eligibility and counter-checking of the application for individual grant and subsidy as well as education service provided by the EDB;
(b) 就上文(a)項所述申請的處理、核實、審核資格及查證，將個人資料與政府相關政策局／部門資料庫進行核對；
Activities relating to matching of the personal data with the database of other relevant Government bureaux / departments in connection with the processing, authentication, assessment on eligibility and counter-checking of the application mentioned in (a) above;

Activities relating to matching of the personal data within the database of EDB for purposes of verifying / updating records of the EDB; and (d) 編製統計資料、研究及政府刊物。

I, and on behalf of the student specified in this Form, consent to the matching of the Data with the personal data held by the Education Bureau and/or the Immigration Department and/or other bureaux/departments and/or their agents/contractors in relation to my application and receipt of payment under such scheme(s), and understand that the matching procedure is conducted for ascertaining whether the student is eligible under such scheme(s) or not, and therefore I will not receive payment under such scheme(s) in case the student is ineligible.
6. 本人同意政府把該等計劃的款項直接存入本人在表格內指定的銀行帳戶。本人亦同意並承諾，如政府就該等計劃向本人支付的款項超過既定款額，或誤向本人支付任何款項，本人定當立即通知政府並退回任

I agree that the payment under such scheme(s) be effected directly into the bank account as specified in this Form. I also agree and undertake to notify the Government forthwith of any overpayment or any payment by mistake to me under such scheme(s) and refund the same to the Government. I declare that the student is enrolled in the day-school as specified in Part I of this Form; the student satisfies the eligibility criteria for such scheme(s), and he/she is neither a holder of student visa for entry into Hong Kong for studies, nor a holder of recognizance form issued by the Immigration Department. I understand that I will not receive payment under such scheme(s) in case the student is ineligible.
8. 本人明白只有其中一位父母或監護人可為每一名合資格學生提交一份申請。本人聲明本人已與本人的配偶及其他監護人（如適用）核實，並沒有就填報的學生重複遞交申請；亦未曾在有關的學年在該等計劃

I understand that only one of the parents or guardians can submit an application for each eligible student once only.
