本研究以兩大部分,分別探討人工牙根與支台體之 I.接合寬度、

接合深度、接合角度,與 II.接合位置與骨脊之相對位置,其結論如 下:

1. 植體與支台體接合因素中,接合寬度對周圍骨應力之影響性高於 接合深度, 而接合角度的影響性最小。

2. 減少接合寬度明顯降低骨質應力值,其原因在於接合寬度減少可 增加植體壁厚度,較大之植體璧的厚度,提供較大的應力分散空 間,有效降低植體應力質,其降低植體應力值的效果,連帶降低 了周圍骨質的應力值,此為平台移轉接合造成骨應力值較低的主 因;然而,縮減接合寬度將直接造成支台體本身之應力值增加,

可能會提高支台體疲勞斷裂的危險性。

3. 錐狀支台體與植體間之接合深度增加,亦有助降低骨質應力值,

其原因在於 增加了支台體與植體間之接合面積,並有助於將應力 向植體壁深處分散。

4. 植體與支台體間錐狀接合角度對周圍骨應力影響不大,僅在植體 受垂直力時,較小的接合角度可些微降低骨質應力值。

為最直接的骨應力承擔部位,其降低骨質應力值的效果顯著。骨 質應力值降低之效果為非線性,並以接觸皮質骨為2.5 mm 為一分 水嶺,低於 2.5 mm,增加接觸皮質骨厚度,可快速降低骨質應力 值,高於 2.5 mm,增加接觸皮質骨厚度,骨質應力值並無顯著降 低。

6. 在骨脊下植入植體時,骨質與人工牙根未接觸的厚度(未接觸皮質 骨),提供較大的應力分散空間,亦具有降低骨質應力值的效果,

但其降低的效果有限。適當厚度之未接觸皮質骨(≧0.6 mm)可 降低骨應力值;厚度不足(<0.6 mm)會阻礙應力分散,造成應 力提高。
