Introduction

Transmigration 就是阻生牙的位移跨越了中線。未萌發的牙齒產生這樣的移動是不 常見而且值得去研究的。很少發現下顎犬齒在下顎是成一水平位置的，而且 transmigration 也很少見。Joshi 認為犬齒跨越mildline suture的傾向比他在越過中線後移動的距離更直得去思考。

阻生牙在牙科而言是重要的，特別是矯正科(尤其是當這顆牙是犬齒時)。下顎阻生犬齒發生率較上顎來的少，而有transmigration現象的更是少見。本篇的目的就是進行6個case report，和回顧文獻。

Case report

Case 1:

一位28歲男性主訴其下顎兩顆殘留的乳犬齒引響美觀。X ray的檢查發現下顎兩側犬齒是阻生的。右下顎犬齒雖然未萌發，但是在一個有利的位置。左下顎犬齒則是跨越中線位移到右側。

Case 2:

一位22歲女性被轉診到口腔外科進行上顎第三大臼齒的移除。X ray顯示左下犬齒是水平阻生的情形，而且其牙根是跨越中線的。

Case 3:

一位54歲男性被轉診去進行補綴物的重建。X ray顯示右下犬齒是阻生的，而且其牙冠是往近心傾斜並跨越中線。牙冠部分被dentigerous cyst 包圍。

Case 4:

一位14歲男生被轉診去處理左下殘留的乳犬齒。X ray顯示左下犬齒是阻生的，而且位置是在正中門牙和侧門牙之間。下顎正中門齒被犬齒往近心推擠，造成下顎中線往右偏移。另外，側門牙的牙根呈現dilaceration的情形，也是阻生犬齒造成的。

Case 5:

一位23歲女性被轉診去進行牙周病治療。X ray顯示右下犬齒水平阻生，並且跨越中線到達左側的小臼齒根尖之下。

Discussion

回顧文獻可以發現下顎犬齒transmigration的情形真的很少見，被提出的病例報告也只有154個。Transmigration 大部分是單側性的，只有16 cases是雙側性 (both)。左側犬牙發生率也較右側高 (both)。女性發生率也較男性高 (both)。

造成transmigration的特殊原因，目前仍是未知。有可能是之前受過傷，遺傳，牙胚萌出路徑太長，乳牙提早喪失，空間不足……等都有可能會造成transmigration。伴隨著dentigerous cyst的犬齒可能是囊腫內的壓力造成transmigration，然而我們無法得知究竟是transmigration前或後才病變成dentigerous cyst。

Mupparapu 提出以下五個分類：Type1~近心傾斜，牙冠跨越中線。Type2~水平阻生

，在門牙根尖之下。 Type3~跑到對侧犬齒區附近。 Type4~水平阻生，在對侧小臼齒根尖之下。 Type5~垂直阻生，長軸跨越中線。

文獻中大多是Type 1，在本篇則是Type 1：2，Type 2：2，Type 4：1，Type 5：1。

在Type 2中要注意的是牙冠還是牙根在對侧。

Howard 認為年紀越大的病人阻生牙移動的距離會更長。Ando和其工作團隊觀察他們的病人六年，發現阻生牙真的會移動。所以早期X ray的發現真的很重要。

這六位病人平均年齡是32歲，如果能早期發現，也許它們會在較有利的位置，讓治療更好進行。一旦transmigration發生，可能就只有進行手術拔除。

Summary

下顎犬齒發生transmigration是很少見的。早期的X ray檢察對治療方式是很重要的。

更進一步的研究也是需要的。
