依據 2019 美國糖尿病學會與其它文獻 探討住院病人血糖控制的監測與治療建議

摘 要

住院病人血糖過高或是過低都會造成併發症的增加,更嚴重甚至會造成病人死亡。因 此住院病人血糖控制的核心精神就是避免高血糖與低血糖。目前認為平均血糖 140mg/dL 至 180mg/dL 是可以接受的範圍,當然依據病況不同是可以適當調整的。筆者依照 2019 美國糖 尿病學會與其它文獻整理出此篇文章,如何依據不同的住院病患狀況選擇適當的處方,以期 將血糖控制到目標範圍內,希望讓所有醫師都可以簡單的應用。

前言

衛生福利部公布台灣 106 年十大死因統計 中,糖尿病排名第五位1。住院的病患中超過 20% 有糖尿病,其中有高達 38% 至 40% 在住院 期間曾經發生高血糖的情況2。依據國外的研究 知道糖尿病患住院率是非糖尿病患的三倍3-4 而且住院時的併發症及死亡率都比非糖尿病患 者高5。住院病人血糖過高或是過低都會造成併 發症的增加,更嚴重甚至會造成病人死亡6-9

因此筆者依照 2019 美國糖尿病學會10與其

它文獻11-13整理出此篇文章,如何依據不同的

住院病患狀況選擇適當的處方,控制住院病患 的血糖並進而降低其併發症是此篇文章的探討 重點。

住院病人血糖浮動的致病機轉

生病時,體內的壓力性荷爾蒙分泌增加造 成脂肪分解、蛋白質分解以及組織細胞利用葡 萄糖能力下降,進而使體內的血糖上升。高血 糖會造成利尿與組織脫水情形使粒線體受損,

進而推升氧化壓力,發炎感染反應因而加劇形 成惡性循環,最終使得內皮細胞損傷並引起多 重器官衰竭14-15

住院病人的血糖監控與控制目標

一、住院病人的血糖監控

所有住院的糖尿病患者或是住院當中量測 血糖高於 140mg/dLl 患者,若三個月內未有糖 化血色素檢測數據者,建議檢測以利評估及藥 物的開立6, 16-18

住院中的血糖監測分三類型病患,其一是 可以進食者建議監測餐前血糖以調整藥物;其 二是無法進食者建議每 4 至 6 小時監測血糖;

最後一種是靜脈注射胰島素患者則建議 30 分鐘 至 2 小時需監測血糖6

二、血糖的治療目標值

當血糖數值超過 180mg/dL 建議開始施打胰 島素,針對大多數的病患目標值為 140mg/dL 至 180mg/dL6。但是少數心臟手術的病患在沒有 低血糖的情況下可設立較嚴格的目標至 110mg/

dL ∼ 140mg/dL 之間19-21

三、低血糖的定義

第 一 級 低 血 糖 為 血 糖 介 於 70mg/dL 至 54mg/dL 之間,第二級低血糖為血糖低於 54mg/

dL,第三級低血糖為低血糖伴隨意識狀態改變 或需他人協助22。當病患出現第一級低血糖時 就應當立即介入處理並調整胰島素或藥物的劑 量,以避免更嚴重的低血糖造成不可逆的併發 症甚至死亡。

低 血 糖 的 處 置 流 程: 意 識 清 楚 的 患 者 可 給 予 15-20g 口 服 醣 類, 若 是 意 識 不 清 楚 者,

則 給 予 靜 脈 注 射 20-50ml 的 50% 葡 萄 糖 水 (Dextrose50), 並 適 時 調 整 藥 物 與 持 續 監 測 血 糖,避免再次發生16

住院病人控糖藥物的選擇與處置

一、如何處理住院時高血糖?一般習慣使 用的滑尺量度胰島素注射法 (sliding scale insulin) 並不建議常規使用21

可以依據病患的疾病嚴重度及營養攝取的 需求,做以下調整:

(ㄧ)針對非重症的病患 1. 胃口良好

建議基礎胰島素 (basal insulin) 加上三餐餐 前胰島素 (nutritional insulin) 與矯正劑量胰島素 (correction insulin)。

2. 胃口不佳

建 議 基 礎 胰 島 素 (basal insulin) 再 依 據 餐 前 血 糖 數 值 給 予 矯 正 劑 量 胰 島 素 (correction insulin)。

劑量的選擇與調整:之前已經有使用胰島 素的病患,可以之前使用的劑量作為每天的總 胰島素總量,再依照不同的胰島素種類分配劑 量。如果之前從未使用過胰島素的病患,可以 依 據 體 重 (kg) 乘 於 0.3 或 是 0.6。 年 紀 大 於 70 歲或是腎功能 (eGFR) 小於 60ml/min/1.73m2 病患以 0.3units/kg/day 作為起始劑量。如果血糖

<198mg/dL 則以 0.4units/kg/day 作為起始劑量,

如果血糖 >198mg/dL 則以 0.5units/kg/day 作為 起始劑量。其中約 40%~50% 的總劑量為基礎胰 島素。筆者認為總劑量 40% 的基礎胰島素加上 20% 餐前胰島素是相對安全的做法。矯正劑量 胰島素的給予則依據病人的餐前血糖數值以及 病人的胰島素敏感性再給予調整,如表一13,23

(二)針對重症的病患

連續靜脈胰島素輸注 (continuous intravenous insulin infusion) 是加護病房患者較好的降血糖方 式。

不同加護病房有不同的擬定的方法,甚至 有電腦程式可以依據前次與當下的血糖數值計 算血糖增減速率得出後續胰島素的劑量。目標 值為 140mg/dL 至 180mg/dL 且避免低血糖低於 70mg/dL。目前大型的臨床試驗均指出過度嚴格 控制血糖反而增加低血糖發生率,產生不必要

的併發症19, 24-27

舉例:利用改良型耶魯大學胰島素輸注策 略 (The Modified Yale Insulin Infusion Protocol) 來 調 整 血 糖28。 此 策 略 的 血 糖 控 制 目 標 值 是 140mg/dL~180mg/dL。 以 起 始 血 糖 數 值 除 以 100,四捨五入取整數值作為起始的輸注速度:

“Δ”胰島素流速調整是必須依照目前的胰島素流速不同而不同,請依以下建議調整。

如 起 始 值 為 320mg/dL,320/100=3.2, 則 給 予 3units/hr 的輸注速度。經過 1 小時後根據血糖增 減數值來調整後續劑量,如表 228。舉例來說:

如果起始值為 320 mg/dL,以 3units/hr 的輸注速 度給予胰島素一小時後血糖下降為 220 mg/dL,

兩者相差下降 100mg/dL,則依據圖表第三行血 糖介於 180-249 mg/dL 而第五列血糖下降超過 80mg/dL,因此胰島素要暫停 30 分鐘然後減少 2Δ,目前流速是 3units/hr,所以 30 分鐘後要減 少至 1 units/hr。

使用連續靜脈胰島素輸注之後必須每小時 監測血糖,直到連續三次的血糖值都在目標範 圍內,才可以改為每 2 小時監測一次;24 小時 都在目標值,就可以改至每 3 到 4 小時再量測 一次。

當病人的血糖穩定時可以考量從連續靜脈 胰島素輸注治療轉換成皮下胰島素注射。以連 續靜脈輸注總劑量的 60%~80% 當作皮下注射的 起始總劑量,且施打基礎胰島素後 2-4 小時才可 停止連續靜脈胰島素輸注。

如果治療過程中遇到低血糖 (50-69mg/dL) 且伴隨低血糖症狀,則停止胰島素靜脈輸注治 療並給與一支 25g 50% 葡萄糖水 (Dextrose50) 注 射,十五分鐘後再測一次血糖。若無低血糖症 狀,則停止胰島素治療並給予半支 25g 50% 葡 萄糖水 (Dextrose50) 注射,然後 30 分鐘後再測 一次血糖。當血糖恢復且大於 100mg/dL 約 1 小 時後,再以 75% 的原胰島素注射劑量重新恢復 治療。若血糖低於 50mg/dL,除立即給予一支 25g 50% 葡萄糖水 (Dextrose50) 注射,15 分鐘後 需再測一次血糖,直到血糖大於 100mg/dL 約 1 小時後,再以 50% 的原胰島素注射劑量重新恢

復治療13, 16

(三)特殊飲食的病患 1. 連續管灌病患

基礎胰島素:以平日的基礎胰島素劑量或 由每日胰島素總劑量之 30-50% 轉換。未使用過 胰島素者,可選擇每 12 小時使用 5 單位的 NPH 或是 insulin detemir、亦可每 24 小時使用 10 單 位的 insulin glargine29

餐前胰島素:每 10-15g 碳水化合物需給予 1 單位胰島素。若使用短效胰島素每 6 小時給 予,若使用速效胰島素則每 4 小時給予。依血 糖高低再給予矯正胰島素29-32

2. 間歇性管灌病患

基礎胰島素:以平日的基礎胰島素劑量或 由每日胰島素劑量之 30-50% 轉換。未使用過胰 島素者,可選擇每 12 小時使用 5 單位的 NPH 或是 insulin detemir、亦可每 24 小時使用 10 單 位的 insulin glargine29

餐前胰島素:每 10-15g 碳水化合物需給予 1 單位胰島素,在每次管灌前給予短效或速效胰 島素。隨著血糖高低再給予矯正胰島素30

3. 全靜脈營養病患

將短效胰島素依每 10g 碳水化合物給予 1 單位劑量的方式加入於全靜脈營養的溶液中。

隨著血糖高低再給予矯正胰島素。

(四)針對糖尿病酮酸中毒及高滲透壓高血糖狀 態病患

此類型的病患治療重點在於找出潛在性感 染 或 發 炎 因 素, 校 正 體 液 與 組 織 灌 流 不 足,

維持電解質平衡以及控制高血糖或酮酸中毒狀 態。嚴重的病患中,以連續靜脈輸注胰島素為 標準治療方式;對於中度及輕度的病患,皮下 胰島素注射也是可以替代的方式。

酮酸中毒及高滲透壓高血糖狀態病患通常 伴隨嚴重的水份缺失。在酮酸中毒患者其體液 缺損約 100ml/kg,而高滲透壓高血糖狀態更可 以高達 100ml/kg 至 200ml/kg。嚴重休克的病患 應盡快以每小時一到兩公升的速度輸液來矯正 休克狀態,接著四小時以每小時 0.5 公升的速率 輸液,最後四小時再改成每小時 250 毫升。過 程中應隨時注意是否有心臟或腎臟衰竭的情況 來調整輸液的速度。病患若合併有低血鉀則必 須先矯正後才可以給予胰島素連續靜脈輸液以 避免致命性的心律不整。胰島素建議一開始以 0.1 units/kg/hr 給予,當血糖低於 250 mg/dL,但 酸中毒尚未校正完全時,應給於 5% 葡萄糖水並

將胰島素劑量減至 0.05 units/kg/hr 直至酸中毒校 正完全。此時可以將連續靜脈胰島素輸注總劑 量的 60-80% 當作皮下注射的起始總量,且應在 施打基礎胰島素後二至四小時才停止連續靜脈 胰島素治療11, 12, 32

(五)類固醇治療病患

針 對 類 固 醇 治 療 患 者, 應 該 考 量 類 固 醇 的種類與預計施打的時間長短來訂定適合的 胰 島 素 治 療 計 劃。 若 病 患 使 用 短 效 的 類 固 醇 (prednisolone),其最大作用時間約為使用後 4-8 小時,此時可使用中長效型胰島素 NPH 來減 少類固醇造成的高血糖。若是使用長效型類固 醇 (dexamethasone),則可以選用長效型胰島素 如 glargine16, 18。若病情需要使用高劑量的類固 醇,則必須考慮連續靜脈輸注,或是選用長效 基礎胰島素再加上餐前胰島素及矯正胰島素來

控制血糖34, 35

二、住院病患使用口服降血糖藥物的時機與建議 研究指出使用 dipeptidyl peptidase 4 inhibitor 併用基礎胰島素的效果與基礎胰島素加上餐前 胰島素的效果相當,但這類藥物中 saxagliptin 和 alogliptin 這兩種藥物對於心衰竭患者,有 增加心臟衰竭及再住院率的疑慮36。Glucagon like peptide 1 receptor agonist 有些病患有腸胃道 的副作用,如噁心嘔吐 …等,對於使用住院病 患也是一大考驗。Sodium-glucose cotransporter 2(SGLT2) inhibitor 容易增加急性腎衰竭與酮酸 中毒的風險,住院中病患若有脫水狀態與或需 要禁食,可能會惡化這些副作用37, 38。而使用 SGLT2 的酮酸中毒很容易血糖並不高不像常見 的高血糖酮酸中毒,很容易誤診39, 40。由此看 來,選擇胰島素可以說是相對簡單且有效的治 療方式。

住院病人血糖控制其它注意事項

一、手術前後應當注意的事項41

(一) 開刀前血糖目標訂定為 80-180mg/dL。

(二) 術前需評估病人使否有血管及其他器 官併發症如:心肌梗塞,自主神經病

變或是腎臟衰竭。

(三) 手術當天暫停使用 metformin。

(四) 手術當天早上停用降血糖藥物,但如 果是中長效胰島素 NPH 則可以給予一 半劑量,長效基礎胰島素則可以給予 60-80% 劑量。

(五) 手術當天每 4-6 小時監測血糖。

結論

住院中無論感染、疾病壓力、飲食、與藥 物的改變都會造成血糖的浮動性增高,將血糖 控制在目標值 140-180 mg/dL 的範圍內且避免低 血糖的發生是所有醫護人員的目標。住院期間 良好的血糖控制,完善的出院嫁接與準備都是 減少再次住院的重要環節,端賴臨床醫師、護 理人員、及衛教團隊的通力合作,才能提高糖 尿病照護的水平,並降低併發症的發生率。

Diabetes Care in the Hospital: New ADA 2019 Standards of Medical Care in Diabetes and

Literature Review

Both hyperglycemia and hypoglycemia are known be associated with an increased risk of complications and mortality in patients during their hospitalization. The essence of blood glucose control is to prevent hyperglycemia and hypoglycemia and their associated complications. A target glucose range of 140mg/dL–180 mg/dL is recommended, however this range can be adjusted according to different clinical condition. Here, we present a review article referenced from the latest guideline from the American Diabetes Association, 2019 Standards of Medical Care in Diabetes, and other references to summarize a comprehensive diabetic management plan in various inpatient settings in hope to derive an intuitive universal treatment regimen for maintaining blood glucose in the desired target range. We hope that this review article can guide the physicians for patients’ glucose control in their daily practice.
