建構多層次超媒體醫療系統

放射科報告系統是放射科資訊系統的一個部分,對於作成醫療上的決定及治 療計畫有關鍵性的影響。但是它是單方面而缺乏互動性的一個系統,意即報 告的文字內容與影像本身是分開的個體,而無法聯想在一起,使得臨床醫師 因為誤認病變的位置,而作了錯誤的決策或手術,以致嚴重的影響到整體的 醫療品質。在中山醫學大學附設醫院完成網路影像傳輸及拮取系統之後,院 內資訊系統的的整合,以及無片化的環境都不是問題,所以我們嘗試建立一 個生動活潑又具互動性的一個報告系統系統。場景為基礎的多層次的結構,用 NAVI-Tag , PIN-Tag 及 Stack-Tag 來定義整它們在系統中扮演的角色以及 如何整合各種醫學資源。

在實施 NAVI 之後,臨床醫師對於疾病的診斷充滿信心,醫學生也對於能整 合各種超媒體醫學資源非常滿意。 NAVI 本身易於叫出資料,也因此而有助 於線上會診及遠距放射線診斷。 NAVI 在臨床應用及電腦為基礎的互動性學 習,都有一個光明的遠景。Multi-layer Hypermedia Medical System

Radiology report system is part of radiology information system (RIS) and it is crucial for med ical decision-making and/or treatment planning. However, it is unidirectional and not interacti ve. It means there is no link about the texture and the image. Therefore, the clinicians bewilder or get confused about the exact location about the lesion and make a wrong decision or even o perate incorrectly, and it also deteriorates the quality of medical care severely. After setup and refinement of our PACS in Chung San Medical College Hospital, its almost complete filmless now and integration of RIS, HIS and PACS is not a problem to us. We wish to implement a sy stem about a live and interactive report system, named NAVI-system.
The NAVI-system is a Scenario-based multi-layer structure for integration of various medical resources which use NAVI-Tag, PIN Tag, and STACK Tag to link medical medias (including text, images, video and voices) and defined their patterns of display. It will help the clinicians to detect the lesion, provide a tool for medical students as computer-based learning and it is als o the backbone of telemedicine or teleradiology.
After implementation of the NAVI-system, the clinicians are more confident about the lesion d etection and will have less chance of misdiagnosis due to the close relationship between clinici ans and the radiologists. The clinician and medical students are all satisfied and appreciate of t he system for integrating the text, image and video and audio together. It is not only easy for r etrieval but contributes a lot to on-line consultation and teleradiography or telemedicine. The s ystem is promising for both clinical use and interactive computer-based learning.
