本研究之結果顯示,具水平平台介面之植體其周邊骨吸收量在支 柱接出第一年內似乎和不具水平平台介面的植體表現相同。隨著使用 時間增加,超過第一年後具水平平台介面之植體的骨吸收量似乎較控 制組來得少,至第 36 個月時達到統計上有明顯差異的程度;惟超過 第 48 個月時,控制組的骨高度突然上升,導致趨勢改變。同時比較 兩組在植體接出第一年後的年平均骨吸收量,兩者亦無顯著差異,因 此無法對水平平台介面在晚期骨吸收的影響下達肯定結論。
由於本研究並非在第二階段手術植體接出時馬上轉換成水平平 台介面,可能因此減弱水平平台介面對早期骨吸收的影響。然而 PS 組似乎在接出第一年後的骨高度表現較佳則是意外的發現,促使我們 注意水平平台介面是否也有利於減少植體周邊的晚期骨吸收。因此,
提早轉換水平平台介面的時間點、延長追蹤年限、以及增加樣本數為 未來相關研究的方向。
關於植體周邊骨變化的議題方興未艾,除了長期成功率的考量 外,穩定的植體周邊軟、硬組織更與贋復物美觀的維持息息相關。水 平平台介面是兩段式植體克服早期骨吸收現象的一線曙光,若其效果 屬實,不僅在臨床上對植體支持贋復物的表現大有幫助,學理上則對 植體周邊骨質的生理學有更深的認識。因此,進一步以前瞻式隨機臨 床試驗(prospective randomized clinical trial)來釐清水平平台介面的效 應實屬必要。
