2.5 頸部的活動
大部分頸部和頭部運動由頸部上面的三個關節活動所控制，頭部支 撐的結構由頸部下面的關節及上背所控制。但頸部的活動實際是由頸部 肌肉所控制，頸部的活動可分為幾個動作，包括：前彎、後伸、雙側旋 轉及側彎。藉由對頸部活動的認識，可以了解頸部疼痛或損傷可能造成 活動的限制，因而找出適當的治療穴位，使本研究可以達到更好的療效。
2.5.1 頸部活動的主要肌肉
頸部的活動可分為幾個動作，包括：前彎、後伸、雙側旋轉及側彎。
而這些動作分別為數個肌肉所控制，包括主要的肌肉和次要的肌肉。

2.5.2 觸發點和反射痛[23]
頸部疼痛引起的原因很多，常以肌肉疼痛的型態出現，而肌肉的疼 痛有所謂的觸發點(trigger points)及反射痛(referred pain)，而這些痛點通 常在中醫的經絡及穴位上，透過對觸發點(trigger points) 及反射痛 (referred pain)的了解，可以更快速及方便的檢查出患者疼痛的位置及情 形，並且掌握患者的預後情形。
觸發點(trigger points)[23]
觸發點被用來描述為在骨骼肌的 hyperirritable spots，和肌纖維的拉 緊帶(taut bands)可觸摸結節(palpable nodules)相關連。臨床醫師相信可觸 摸結節是小的收縮結節並且會造成疼痛，壓縮觸發點(trigger points)可能
2. 這疼痛的點能夠被感覺如腫塊(tumor)或帶(band)在肌肉上，並 且抽搐(twitch)反應可被觸發點的刺激所引發。
3. 可觸摸的觸發點造成患者疼痛的困擾。
4. 這疼痛無法被神經學的檢查(neurological examination)所解 釋。
反射痛(referred pain) [25]
反射痛，也稱為擴散痛，是相對於局部疼痛而言。局部疼痛是指病 變局部的疼痛，當局部肌肉、韌帶或器官等組織受到損害在局部引起的 疼痛。反射痛是指一個組織或器官損傷後，疼痛向臨近或遠端的組織放 射而產生的疼痛。International Association for the Study of Pain, 2001，並 未正式定義這個術語，所以許多作者有自己不同的定義。
