影響台灣地區醫師實施臨床指引意願之因素探討

(1) 臨床指引對於提昇醫療品質應有所助益，本研究初步得到醫療團隊主導者醫師願意實施臨床治
療指引的比例為 87% 。
(2) 醫師是否願意執行臨床治療指引與下列因素有關：
1. 是否曾經使用過臨床治療指引。
2. 是否曾經有實施臨床路徑經驗。
3. 執業科別中有否論病例計酬案件。
4. 醫院或基層診所醫師。
5. 公立醫院或基層診所醫師。
(3) 指引之特性亦影響醫師實施臨床指引，整體來看，在台灣地區的臨床指引，最為醫師接受的是
：符合學理、證據醫學之學術性，由專科醫學會制定、在運用時必須在法律事件上對醫師有保護作
用、使用過程中必須可以依情況修正而且可以由期刊及刊物中獲得，並應由公正、自主機構監測及
全國性之適用。
(4) 醫師對於新事物的經驗與熟悉度是是否願意實施指引的重要因素。推動之策略以短程現階段來
看，繼續推廣臨床路徑及結合健保局支付制度，促使醫師有更足夠的使用及接觸經驗，以提升對臨
床指引之接受及認同。而中程之推動策略宜藉由醫院中較為完善之品管功能及醫院醫師對於臨床指
引接受程度相對較高之情況，應可由醫院開始來推動，尤其是意願性較高之公立醫院。而加強對於
尚未接觸過臨床路徑的科別、醫師之宣導均是導入臨床指引之重要策略。長程之推動策略宜由政府
成立國家級之獨立機構 ( 類似美國 National Guideline Clearing House) ，結合實證醫學之運用 ( 類似
國外 Cochrane Library) ，研發、制訂符合證據醫學、具有彈性、容易取得之全國性臨床指引並包括
對於法律層面之探討，以推廣及監測臨床指引之實行與運用。
綜合上述，本研究提供醫師對實施臨床指引意願性觀點之表達，亦可明白醫師族群對臨床指引之期
盼。期盼可提供相關單位參考。
Exploring Factors of Physicians’ Willingness on Im
plementation of Clinical Practice Guidelines in Tai
wan
A study on factors that influence physicians’ willingness for implementatio n of clinical practice guidelines was performed through a structured questio nnaire. In Taiwan ， physicians who had the experience on clinical practic e guidelines or clinical path will have more intense to perform the clinical practice guidelines.
The other factors are those who work in hospitals especial in public hospita ls and who face the 「 case payment 」 during their practice. The characte ristics of clinical practice guidelines that will be favored by physicians in T aiwan are 「 match the evidence-based medicine 」、「 made by associat ion of specialist 」、「 national level 」、「 flexible in use 」、「 prote ctive role in legal events 」、「 available on journals 」 and 「 monitori ng by an independent organization 」 .
Suggestions to policy makers 、 hospital administrators and physicians are described.
