糖尿病腎病變治療新進展

導致末期腎臟病變的原因眾多,其中糖尿病腎病變佔 4 成 5 且持續上升中。近來,許多 大型研究提供臨床治療更多證據,本篇以實證醫學為觀點,提供更貼近臨床照護的準則,讓 第一線照護醫師面對糖尿病腎病變有更好的依循,旨在探討糖尿病腎病變之最新治療原則。
文中以血糖控制出發,繼之以致病機轉、治療選擇、可能有害、無效或待研究之治療為軸,
提供適切建議,以達最佳治療建議。近來,針對 SGLT-2 抑制劑 (Sodium-Glucose Cotransporter 2 Inhibitors, SGLT-2 Inhibitors) 研究,發現其卓著的心腎保護效果亦詳加著墨,而其潛在治療 風險也加強提醒,誠為本文重點。

引言
糖 尿 病 腎 病 變 的 典 型 症 狀 為 巨 蛋 白 尿 (macroalbuminuria),常見於血糖控制不佳合併 高血壓導致腎絲球灌流過盛 (hyperfiltration) 的 患者。不論第一型與第二型糖尿病患者,其一 生得到糖尿病腎病變的風險是相同的1。微量蛋 白尿 (microalbuminuria) 是糖尿病影響腎臟的初 期臨床表癥,於此同時,部分腎臟組織可能已 經發生腎絲球硬化現象2( 表一 )。然而,並非所 有蛋白尿發生皆肇因於糖尿病腎病變,以第一 型糖尿病為例,確診五年內即發生蛋白尿、未 合併視網膜或周邊神經病變、腎功能退化速度
過快、尿液檢體出現異常沉澱或血尿等情形,
皆需探查其他非糖尿病腎病變之原因3。本篇旨 在探討糖尿病腎病變之最新治療原則。

血糖控制與糖尿病腎病變
目前已知糖尿病腎病變可藉由嚴格的血糖 控制來改善,此效果在第一型糖尿病患者最為 顯著,且與起始控制之腎功能及血糖代謝情形 有關5。研究顯示,以胰島素長期嚴格控制血糖 ( 部分報告需維持二年以上正常血糖值 ) 可獲得 下列效果:部分改善腎絲球肥大 (hypertrophy) 及灌流過盛 (hyperfiltration)、穩定或減少腎絲 球過濾尿蛋白之量、延緩腎絲球過濾率下降速 度6。另有研究指出,接受胰臟移植之第一型 糖尿病患者,不論移植前蛋白尿嚴重度,移植 後維持正常血糖值達十年時,經由腎臟切片發 現,因灌流過盛 (hyperfiltration) 導致擴張之腎 絲球及腎小管間質 (mesangial) 體積產生回縮,
部分腎絲球結節狀病灶消失,以及腎小管萎縮 獲得改善7。
糖尿病腎病變致病機轉
除了前述血糖因素,尚有許多其他致病因 子存在。歷經二十多年的研究,糖尿病導致腎 病變的機轉在細胞及分子生物層次已有長足進
展。目前認為高血糖與血行動力學改變之交互 作用,產生代謝中間產物及刺激生長因子及細 胞激素,最終造成蛋白尿、發炎反應及腎絲球 纖維化8 ( 附圖一 )。在由眾多動物實驗發現,
腎絲球內部壓力上升與腎絲球擴大 (hypertrophy) 是其中關鍵。在糖尿病初期,擴張的腎臟血管 造成腎絲球過濾率增加,腎絲球壓力上升伴隨 腎 元 受 損, 進 一 步 代 償 增 加 腎 絲 球 血 流, 最 終造成腎絲球擴大。而蛋白尿的嚴重度直接反 應出腎絲球內部高壓,為了獲得良好預後,治 療重點在於減少尿蛋白排出,維持足夠腎絲球 過濾率 (GFR)。因此,為了減低腎絲球內部壓 力, 可 藉 由 低 蛋 白 質 飲 食 以 減 少 尿 蛋 白 排 出 達 到 減 壓, 或 使 用 放 鬆 出 球 小 動 脈 的 藥 物 如 Angiotensin-converting enzyme (ACE) inhibitors 或 Angiotensin receptor blockers (ARBs) 可改善 及預防腎絲球壓力上升。曾經有證據認為第一 型糖 尿 病 患 者使 用 ACE inhibitors、第二型糖 尿病患者使用 ARBs 的效果較佳,但目前公認 此兩種藥物皆有保存腎功能及改善蛋白尿的功 效9。

第一型糖尿病患者治療選擇
ACE inhibitors
的護腎機制糖尿病腎變病初期僅有微量蛋白尿時,給 予 ACE inhibitors 即有明顯效果。一項為期 2 年 包含 92 位第一型糖尿病患者的隨機分配研究指 出,對於血壓正常但患有中等程度蛋白尿的病 人投予 ACE inhibitor,相較於使用安慰劑的對 照組,可減少尿蛋白排出且延緩兩年之內糖尿 病腎病變病程的進展10。另一項有關第一型糖 尿病的最大規模試驗共收案 490 位患者,血清 肌酸酐濃度少於 2.5mg/dL 但伴隨明顯蛋白尿,
給予 Captopril 治療比較使用安慰劑合併其他降 血壓藥物治療之對照組,追蹤 4 年,兩組患者 的血壓控制相當,但是發現對於血清肌酸酐大 於 1.5mg/dL 的患者,使用 Captopril 組別之血清 肌酸酐上升速度減低 50%11。
陸續有許多研究證實,第一型糖尿病患者 長期使用 ACE inhibitors 皆有良好的效果,維持 血壓控制,減少尿蛋白排出,同時減緩腎功能 衰退12。
若第一型糖尿病患者使用單一 ACE inhibi- tors 療效不佳時,合併使用 ARBs 與 ACE inhibi- tors,可減少更多的蛋白尿排出,這可能與更佳 的血壓控制有關13。
第二型糖尿病患者治療選擇
一、ARBs護腎機制
根 據 RENAAL 隨 機 分 佈 試 驗, 共 收 案 1513 位第二型糖尿病患者 ( 平均血清肌酸酐為 1.9 mg/dL), 分 別 給 予 Losartan 與 安 慰 劑, 追 蹤 3.4 年後發現,在不影響血壓情形下,使用 Losartan 治療可減少 25% 患者血清肌酸酐上升 (2 倍 ),且減少 28% 患者進展為末期腎病14。 後續關於 RENAAL 的分析發現,開始 ARBs 治 療後半年內,與腎臟病進展相關度最高的風險 因子為蛋白尿之多寡15,每減少 50% 蛋白尿排 出,心血管風險便減少 18%16。
二、
ACE inhibitors
護腎機制ADVANCE 試驗比較 11000 位第二型糖尿
病患者使用 Perindopril-indapamide 與安慰劑的 效果,患者初期平均血清肌酸酐為 1mg/dL 合併 中度蛋白尿17。追蹤 4.3 年後發現,血壓平均值 明顯下降 (-5.6/-2.2 mmHg),使蛋白尿增加的比 例下降。但是對於血清肌酸酐數值上升或進展 至末期腎臟病的速率,使用 ACE inhibitors 並無 法獲得明顯改善18。
DETAIL 試 驗 則 比 較 使 用 Enalapril (ACE inhibitors) 與 Telmisartan (ARBs) 在 250 位 第 二 型 糖 尿 病 合 併 初 期 腎 病 變 的 患 者 的 效 果, 觀 察 5 年 後 發 現, 兩 種 藥 物 對 於 腎 絲 球 過 濾 率 (GFR)、血壓、血清肌酸酐、尿蛋白排出、末期 腎病發生率、心血管事件機率及死亡率等,結 果並無明顯差異,證明 ACE inhibitos 與 ARBs 對於糖尿病腎病變皆能減少蛋白尿排出量及延 緩腎功能退化的速度,但無法改變糖尿病患者 最終將進入末期腎病的事實9。
三、鈣離子阻斷劑
針對糖尿病腎病變患者,除了 ACE inhibi- tors 或 ARBs 外,唯有 Diltiazem 與 Verapamil 具 備減少蛋白尿的功效19。另外,與 ACE inhibi- tors 或 ARBs 併用時還能產生加乘效果。一篇 關於 30 位第二型糖尿病患者的研究顯示,單獨 使用 Lisinopril 或 Verapamil 皆能減輕排出蛋白 尿 (5.8 g/ 天降至 2.7 g/ 天 ),若兩者合併使用則 發現,尿蛋白改善幅度增加 (6.8 g/ 天降至 1.7 g/
天 ),且劑量較低,副作用 (Verapamil:便秘、
Lisinopril:眩暈 ) 也較少發生20。目前對於鈣離 子阻斷劑 (non-dihydropyridine) 改善蛋白尿的機 制並不完全了解,部分認為 non-dihydropyridine 鈣離子阻斷劑除了可降低腎絲球內部壓力,於 動物試驗發現可能藉由回縮增生的腎絲球達到 減輕蛋白尿的效果21,而另一項人體試驗則認 為 Diltiazem 可改變腎絲球體積而有助於減少蛋 白尿22。
四、血壓控制目標
嚴 格 的 血 壓 控 制 能 夠 防 止 第 二 型 糖 尿 病 腎病變及其他併發症的產生,但最佳的收縮壓 控制目標依然沒有定論。英國前瞻性糖尿病研
究 (UKPDS) 報 告, 收 縮 壓 每 下 降 10mmHg,
則 減 少 12% 糖尿病併發症風險,當收縮壓降 至 120mmHg 時的風險值最低23。另一項關於 Irbesartan 的研究 (Irbesartan Diabetic Nephropathy Trial) 也 得 到 相 似 的 結 論, 將 收 縮 壓 控 制 在 120mmHg 的患者有較低心血管及腎臟病的風 險指數24。然而,更低的收縮壓卻可能使有心 臟病史或心衰竭患者的風險增加25。目前對於 有 蛋 白 尿 的 病 患, 要 求 血 壓 控 制 目 標 需 低 於 130/80mmHg;若單次尿液 protein-to-creatinine ratio (UPCR) 在 1000mg/g 以上 ( 代表每日尿液 排 出 蛋 白 質 約 大 於 1000mg) 或 有 糖 尿 病 腎 病 變,考慮將收縮壓控制更低26。但要注意,所 有患者的收縮壓應避免低於 110mmHg;有冠狀 動脈疾病患者之舒張壓不應低於 75mmHg27。
五、體重控制
肥胖的糖尿病患者進行減重之後,可觀察 到蛋白尿明顯改善。一項針對 30 位體重過重病 患 ( 身體質量指數 >27 kg/m2), 合 併 蛋 白 尿 的 研究顯示,其中 14 位患有第二型糖尿病,進行 飲食控制組在觀察五個月之後,平均體重減少 4%,蛋白尿排出量減少 31%,雖然最終對於腎 功能無明顯影響,但若持續追蹤應能發現對於 腎臟的益處28。
六、低蛋白飲食
糖 尿 病 腎 病 變 的 患 者 想 藉 限 制 蛋 白 質 攝 取以減緩腎功能惡化之理論仍未被證實。兩篇 有 關 第 一 型 糖 尿 病 患 者 的 小 型 試 驗 得 知, 低 蛋白飲食 ( 每日 0.6 g/kg) 可延緩 eGFR 下降速
率29,30。但其他試驗卻顯示低蛋白飲食與正常飲
食之 eGFR 下降速率並無差異31。對此不一致 的結果有兩個解釋,(1) 限制蛋白每日攝取量依 然偏高 (2) 受到調控的血壓、蛋白尿及血糖等因 子同時也是影響腎絲球過濾率的獨立因子,可 能遮蔽限蛋白飲食的效果。但是糖尿病患者若 嚴格限制蛋白質則可能衍生其他問題,在相對 缺乏胰島素情況下,會加速代謝蛋白質產生能 量,造成蛋白質缺乏之營養不良32。事實上,
許多病患經常無法遵循嚴格限制蛋白飲食,目
前主要建議患者避免攝取過多蛋白質,每日約 1 g/kg 是較為可行的方式。
七、鹽份攝取與蛋白尿之關係
部 分 研 究 顯 示, 糖 尿 病 患 者 採 取 高 鈉 飲 食 已 被 證 實 與 減 弱 ACE inhibitors/ARBs 調 節 蛋 白 尿 效 力 有 關33。 因 此, 若 病 患 使 用 ACE inhibitors 或 ARBs 時, 血 壓 獲 得 初 步 控 制,
而蛋白尿未明顯改善時,建議患者採取低鈉飲 食34。一項研究顯示,嚴格限制鹽份攝取 (≤ 70 meq/ 天 ) 可 加 強 ARBs 降 低 尿 蛋 白 排 出 的 效 果35。然而,一般患者通常無法接受嚴格的低 鈉飲食,目前建議每日限鈉≤ 100 meq/ 天,並 檢驗 24 小時尿液之鈉離子與肌酸酐濃度,計算 尿液鈉離子清除率,來追蹤患者是否遵循低鈉 飲食。若飲食無法改變,可考慮使用利尿劑排 出鈉離子,增強腎素系統抑制劑抗蛋白尿的效 果33。
八、
SGLT-2
抑制劑(Sodium-Glucose Cotrans- porter 2 Inhibitors)
目前已知,第二型糖尿病患者使用 Sodium- glucose cotransporter 2 (SGLT-2) inhibitors, 如 Canagliflozin、Empagliflozin 可 幫 助 控 制 血 糖 值、降低心血管疾病風險及減輕體重36,37。而 糖尿病腎病變屬於微小血管的病變,一項比較 使用 Empagliflozin 與安慰劑的臨床試驗顯示可 減少蛋白尿排出、延緩血清肌酸酐上升速率、
延後患者進入透析時間23。確切機轉仍未完全 了解,可能與血糖降低以外的多重因素有關。
若 合 併 使 用 NSAIDs、ACE inhibitors、ARBs 時,需注意可能加重病患發生急性腎損傷之風 險,部分使用 Canagliflozin 或 Dapagliflozin 患 者發生急性腎損傷而導致住院或需要透析。由 於 SGLT-2 inhibitors 可增加尿糖排出,因此部 分研究顯示患者發生泌尿生殖器感染的機率增 加38。另外,對於骨質疏鬆的患者,曾有報告 顯示使用 12 週後發生骨質流失且骨折機率增 加39。目前若患者使用此類藥物,需注意禁忌 症 為: 第 一 型 糖 尿 病、 第 二 型 糖 尿 病 合 併 腎 功 能 不 全 (Dapagliflozin:eGFR<60 ml/min,
Canagliflozin/Empagliflozin:eGFR<45 ml/min)、
第二型糖尿病合併酮酸中毒。
九、PPAR agonists
Peroxisome proliferator-activated receptors (PPAR) 為過氧化體轉錄因子的活化調節受器,
其 作 用 在 於 調 控 脂 質 合 成 代 謝、 胰 島 素 耐 受 性、發炎反應、及血壓40。使 PPAR 配體活化 的藥物已被認為可用來治療高血壓、粥狀動脈 硬化、及糖尿病腎病變41。目前發現 PPAR 有 三 個 分 型:PPAR-α,PPAR-β/δ,PPAR-γ。 其 中,PPAR-α 活化劑 Fenofibrate 與 Clofibrate 目 前 廣 泛 應 用 於 降 血 脂 治 療, 而 腎 臟 近 曲 小 管 及 腎 絲 球 基 質 細 胞 (mesangial cell) 表 現 大 量 PPAR-α 受器,部分研究顯示,使用 clofibrate 活 化 PPAR-α,可抑制 TGF-β 引發的腎絲球間質細 胞氧化反應,進而減少間質細胞增生42。另外 有研究顯示,加強 PPAR-α 活性還可促進腎小管 細胞對於尿蛋白的回收及分解,減少蛋白尿排 出43。目前眾多糖尿病患者使用的 PPAR-γ 活化 劑為 Thiazolidinediones 類藥物 (Troglitazone、
Rosiglitazone、Pioglitazone),除了控制血糖效果 之外,藉由抑制腎臟間質細胞 TGF-β 的作用,
達到降低蛋白尿、改善腎絲球灌流過度,避免 腎絲球擴張。臨床應用上,由於過度活化 PPAR 受器可能導致心肌細胞脂質代謝異常而產生心 肌病變44,因此,對於心衰竭或心肌病變患者 為使用之禁忌症。
可能有害、無效或待研究之治療
一、合併
ACE inhibitors
與ARBs
治療在 第 一 型 或 第 二 型 糖 尿 病 患 者 合 併 使 用 ACE inhibitors 及 ARBs 比較單一藥物治療,可 降低更多蛋白尿。然而,也增加不良事件的發 生率。一項美國退伍軍人的隨機分配雙盲研究 (VA NEPHRON-D) 顯示,1448 位男性居多的糖 尿病腎病變患者,在已使用 Losartan 情形下,
隨機分為併用 Lisinopril 與安慰劑兩組,但此試 驗進行 2.2 年時旋及中止,原因是併用兩項藥物 群體發生急性腎損傷 (18% vs 11%) 及高血鉀機 率 (9.9% vs 4.4%) 明顯增加45。
另一項關於單用 Ramipril 與 Ramipril 併用 Telmisartan 的研究46也有相似的結果,3163 位 患有糖尿病腎病變的病人併用藥物並無法改善 腎功能,反而增加需要透析的急性腎損傷 (1.4%
vs 0.8%)、高血鉀 (11.3 vs 7.8%)、低血壓 (2.8 vs 1.9%) 機率46。
二、Aliskiren合併
Angiotensin inhibition
Aliskiren 作用為直接抑制腎素 (renin),合 併 ACE inhibitor 使用並無法保存腎臟功能,反 而 有 害。 在 一 項 Aliskiren 與 第 二 型 糖 尿 病 患 者 的 跨 國 研 究47顯 示,8561 位已經長期使用 ARBs 或 ACE inhibitors 的第二型糖尿病腎病變 患者,分別隨機給予 Aliskiren 或安慰劑,平均 追蹤 32.9 個月後,結果顯示綜合腎臟及心血管 相關的死亡及高血鉀發生率在 Aliskiren 治療組 較高,相較於對照組缺乏其他優點,試驗因而 被迫提早終止47。雖有研究顯示,Aliskiren 合 併使用 Losartan 較單用 Losartan 而言,降低蛋 白尿效果更佳48,但考量明顯副作用,目前仍 不建議使用 Aliskiren 治療糖尿病腎病變。
三、
Bardoxolone methyl
Bardoxolone methyl 與 prostaglandin 相 似 具 有 抗 氧 化 的 發 炎 抑 制 劑。 曾 在 動 物 試 驗 中 觀察到有益於缺血性或藥物引起的急性腎衰 竭, 但 在 人 類 卻 無 法 獲 得 相 同 的 結 果。 一 項 關 於 227 位 平 均 腎 絲 球 過 濾 率 (eGFR) 介 於 20-45 ml/min/1.73m2的 第 二 型 糖 尿 病 患 者,
使用 Bardoxolone methyl 與安慰劑的隨機分配 試 驗 (BEAM trial)49顯 示, 投 藥 後 52 週, 使 用 Bardoxolone 患者的 eGFR 明顯增加 6-10 ml/
min/1.73m2。但是後續試驗 (BEACON trial) 針 對 服 用 ACE inhibitors 或 ARBs 的 患 者, 比 較 併 用 Bardoxolone methyl 與 安 慰 劑50, 卻 因 Bardoxolone methyl 組的患者發生心血管不良事 件 ( 心因性死亡、心衰竭住院率、中風、心肌 梗塞 ) 比例增加 (12.8% vs 7.8%) 而提前終止試 驗。另外,雖然 Bardoxolone methyl 顯著增加 eGFR,但同時導致鈉水滯留,造成血壓上升及 形成蛋白尿51。
四、Pentoxifylline
Pentoxifylline 為非專一性 Phosphodiesterase inhibitor 具抗發炎反應,臨床上常被用在跛行 及 酒 精 性 肝 炎 的 患 者。 關 於 糖 尿 病 腎 病 變 的 小 型 研 究 發 現 pentoxifylline 能 夠 改 善 或 穩 定 eGFR52,而一項薈萃分析 (meta-analysis) 甚至顯 示,pentoxifylline 降低蛋白尿的效果近似使用 ACE inhibitors53。但是相關研究規模過小,尚待 更多大型臨床試驗證實 pentoxifylline 作用。
五、鹽皮質激素受體拮抗作用
利尿劑雖能降低血壓,但一般並不認為具 有減少尿蛋白排出的功效54。然而,不論第一 型或第二型糖尿病合併蛋白尿患者,單獨使用 鹽皮質激素受體抑制劑即有降低排出尿蛋白的 功效,若與 ACE inhibitors 或 ARBs 併用時更有 加乘效果55。一項針對 81 位患有糖尿病、高血 壓、蛋白尿的患者研究顯示,在使用 Linsinopril (80 mg/ 天 ) 的 基 礎 下, 將 患 者 分 為 三 組 分 別 合併使用 Spironolactone (25 mg/ 天 )、Losartan (100 mg/ 天 ) 與安慰劑,在第 48 週時觀察到相 較於安慰劑,併用 Spironolactone 的族群 UACR (urine albumin-to-creatinine ratio) 降低效果增加 34%,併用 Losartan 的族群僅增加 17%,而各 組血壓並無明顯差異,顯示與血壓的改變並無 關係56。目前進行研究中的藥物為 Eplerenone57 與 Finerenone56,仍未有足夠證據顯示長期併 用鹽皮質激素受體抑制劑與 ACE inhibitors 或 ARBs 能夠降低 GFR 退化或防止進展至末期腎 病或死亡,相反地,由於血鉀上升及 GFR 下降 的風險,限制了此類藥物的臨床運用。
總結
綜 合 以 上 研 究 結 論, 目 前 建 議 用 於 糖 尿 病腎病變 ( 蛋白尿 ) 之藥物以 ACE inhibitors、
ARBs、Diltiazem 與 Verapamil 為 主54, 藉 由 降低腎絲球內部高壓及改變腎絲球微血管增生 等機制,減少蛋白尿排出。蛋白尿程度向來被 視為是糖尿病腎病變的重要指標,眾多類型研 究也顯示降低蛋白尿程度與延緩腎病變及減低 末期腎病之風險有高度相關,但糖尿病患者常
有諸多共病現象,如:高血壓、體重過重、血 脂代謝異常、胰島素阻抗、發炎反應、氧化壓 力等因素。單獨治療蛋白尿並無法反轉糖尿病 腎病變之病程,且對於長期腎臟預後並無直接 明確的因果關係,因此,尚需合併血壓控制、
穩定血糖、減重、低鈉飲食等多面向治療。此 外,其他新興的治療標的仍有待更多糖尿病腎 病變相關之基礎病生理研究及臨床試驗證明。
