第八節 本研究貢獻及未來方向
本研究的貢獻如下：（1）建立國內阿茲海默型失智症的「知感」資料，（2）
以臨床失智量表（CDR, Clinical Dementia, Rating scale）對阿茲海默型失智患者
進行失智嚴重度的分組，以清楚比較不同嚴重度之失智患者的「知感」表現，增
進臨床的實用性，(3)發現疑似及輕度阿茲海默型失智症的記憶、執行功能及「知
感」表現皆出現缺損，(4)推論「知感」的心理機制與記憶及執行功能兩者有關，
（5）在臨床上藉由「知感」評估，有助於醫療人員了解患者對自己病情的覺知
情形是否良好，進而判斷是否可根據患者的病情陳述來做為診斷的依據；當發現
患者知感判斷良好卻出現記憶抱怨，醫療人員則可早期偵測患者罹患阿茲海默型
失智症的可能性，以達到早期發現，早期治療。（6）整合國內正常老年人的「知
感」研究，以瞭解台灣正常老年人的「知感」表現。（7）發現疑似失智症患者及
輕度失智症患者出現主觀記憶抱怨，則可以推測客觀記憶有缺損，但無法推測其
「知感」是否缺損；若上述兩種的患者沒有出現主觀記憶抱怨，則無法預測客觀
記憶是否缺損，但可推測「知感」判斷缺損。
未來的研究方向，由於本研究工具為文字，所以未來在研究工具方面希望可
以發展出以圖片的方式呈現，以消除受試者在文字閱讀方面的限制，以期方便更
為廣泛地應用於臨床上。另外，未來可加入神經影像學的研究，以結合本「知感」
研究結果和大腦功能定位的關係。
