呼吸器依賴患者整合性照護轉介資源平台使用成效

摘要 經由建構呼吸器依賴患者整合性照護轉介資源平台，提供各單位轉介資訊的需求以達垂直性及水平性的服務整 合，且針對轉介個案相關轉介原因與轉介結果的資料提供資料分析，以做為臨床分析改善之參考。研究結果顯示 ，轉介資源平台使用者使用成效分析中，顯示使用轉介資源平台後使用者滿意度整體提昇 34.8%，達總滿意度 92% 。轉介資源平台之案例資料統計成效分析結果顯示，於 97.01~97.07 共 87 例個案經由轉介資源平台順利進入第二 階段呼吸照護中心，針對此 87 例個案資料詳細分析，男性 57 例佔 65.5%，女性 30 例佔 34.5%；平均年齡為 74.6 歲，男性平均年齡 73 歲，女性平均年齡為 77.6 歲。提早進入 RCC 之個案，女性比男性多；病情不穩延後進入 RCC 之個案，男性比女性多；第二階段呼吸器脫離成功轉入病房之個案，女性比男性多；於第二階段中死亡之個 案，男性比女性多；第二階段進入第三階段之個案，女性比男性多。提早進入 RCC 者其呼吸器脫離成功轉入病 房與轉入第三階段 RCW 佔最多，病情不穩延後進入 RCC 者較多於 RCC 中死亡。於 RCC 中死亡或病情變化回 轉 ICU 者其訂床至第一階段期滿或訂床至轉入時間皆比平均時間長。 關鍵字：整合性照護、轉介資源平台 Abstract
According to patients relied on constructive mechanical ventilator, incorporated care transfer platform recourses provided each unit the requirement of transfer recourses in order to achieve vertical and horizontal incorporated services, and to use individual case related transfer reasons and results to generate clinical improvement analysis for reference. Transfer resource platform user research showed a 34.8% satisfaction increase reaching total satisfaction index of 92%. Transfer resource platform case data statistically showed there were 87 cases in total during January 2008 to July 2008 successfully entered into second stage of respiratory care center through transfer resource platform. Detailed analysis was carried out for these 87 cases with an average age of 74.6. 57 cases (65.5%) were males aged around 73 and 30 cases (34.5%) were females aged around 77.6. There were more female patients in the case of early RCC entry, successful mechanical ventilator weaning in the second stage, and development into third stage. Meanwhile, more male patients were found in the case of delay entry to the RCC due to unstable conditions and death in the second stage. Most of these cases were transfered to the general ward after mechanical ventilator weaning was successful and transfer into third stage of RCW due to early RCC entry. Patients delayed entering RCC due to unstable condition tended to die in the process of RCC. In addition, patients sufferred from death or transferred to ICU because of unstable condition during RCC tended to take longer than usual in the period of initial bed ordering to first stage expiration or initial bed ordering to entering time.
