(1)酒精影響運動免疫功能. 運動後攝取含酒精飲料對生理及免疫功能的影響 1. 2. 洪寶蓮 、方世華 1 中國醫藥大學運動醫學系 2 國立臺灣體育運動大學競技運動學系 摘要 運動員飲酒被認為有助於團隊向心力的凝聚及壓力的釋放,雖然過去研究顯 示適量的酒精不影響選手的運動表現,但因研究對象的運動訓練量與強度不同, 以及對於酒精的吸收與代謝能力有明顯的個體差異,劇烈運動後攝取酒精是否會 影響生理功能及免疫功能的恢復是一個重要的議題,本篇文章探討最近有關酒精 影響骨骼肌、體溫及水份調節、醣類及脂肪代謝、神經系統、賀爾蒙系統、免疫 細胞及細胞激素的研究結果,發現低劑量酒精不會對肝醣恢復、復水及肌肉損傷 造成危害,然而激烈運動後攝取較高劑量酒精則會出現較大的肌肉損傷、降低中 樞神經系統的功能、降低睪固酮的分泌量及功能、降低吞噬細胞移動、吞噬、產 生細胞激素與殺菌的能力並降低自然殺手細胞抗腫瘤及抗病毒的能力,導致整體 免疫功能下降。因此,建議有飲酒習慣的運動員宜控制酒精攝取量並充分休息, 以利運動後各種生理功能及免疫系統之恢復。. 關鍵詞:酒精、運動後恢復、生理功能、免疫細胞、細胞激素. 通訊作者:方世華 E-mail:shfang@ntupes.edu.tw. (2) 國立臺灣體育運動大學學報 第 4 卷第 2 期(104.06),51 – 66. 壹、前言 飲酒與團隊性的運動項目有較密切的相關性,特別是以男性為主的運動項目, 如:美式足球,因為在飲酒的情境中被認為與團隊向心力的凝聚及壓力的釋放有 關。過去已了解酒精在人體中的代謝途徑及是否影響選手的運動表現,唯酒精對 於 運動後 恢復 期的影 響是 近年來 重要 議題 (Barnes,2014), 由於其 影響 因素相當 複雜,目前細胞研究及動物實驗的結果顯示運動員在高強度運動後仍可以適量地 攝取含酒精飲料,但須更注意配合哪些飲食策略將有助於運動後恢復。特別是酒 精的吸收與排除在人跟人之間有很大的差異,酒精動力學的影響因子包括:年齡、 性別、基因、身體組成及所攝取的酒精量、濃度、酒的種類以及是否伴隨著其他 食物及這些食物的種類等 (Irwin, Goodwin, Leveritt, Davey, & Desbrow, 2012), 本文旨在探討近期有關運動後攝取酒精影響各個生理層面包括:骨骼肌、代謝及 體溫調節、神經系統、內分泌系統及免疫功能恢復的研究結果,提供運動員飲酒 時之參考原則。. 貳、酒精攝取對健康的影響 酒類飲料是全球最常被使用的休閒飲品之一,依據國際標準定義一單位的標 準飲酒 (standard drink, StD)約是 360 毫升的一般啤酒或是 150 毫升的紅酒,其 中約含 15 公克的酒精。世界健康組織定義過度飲酒為在單一次的飲酒狀況下飲 用超過 60 公克的酒精 (Prentice, Stannard, & Barnes, 2014a)。由於飲酒除了酒精 的部分之外,非酒精的部份大都含有較多的多酚類 (polyphenol) ,而這些成分 已被許多研究發現具有抗氧化、抗發炎、抗病毒、保護神經、降低血小板凝集及 低密度脂蛋白膽固醇 (low-density lipoprotein cholesterol)及抗癌等功效 (Arranz et al.,2012)。因此,若是適度的飲酒則有益於健康,對於改善細胞老化、認知功 能等有正面的功效,如:男性每天 2 個標準飲酒單位及女性 1 個標準飲酒單位, 經調查性研究結果顯示反而比不飲酒者罹患心血管疾病的風險較低,其可能的保. 52.

(3) 酒精影響運動免疫功能. 護機轉是透過增加高密度脂蛋白膽固醇 (high-density lipoprotein cholesterol) 、 降低血小板凝集及纖維蛋白原 (fibrinogen) 的濃度,進而促進內皮細胞功能及減 少全身性發炎 (Arranz et al., 2012)。相反的若是超量飲酒則會造成急性或是慢性 的身心傷害,目前已有足夠的證據支持規律及適度的飲酒與得到心血管疾病的風 險有顯著的負相關性,將增加心血管疾病造成的死亡率 (Prentice et al., 2014a)。 另外有研究指出適度的飲酒可以降低許多腫瘤,如:大腸癌、乳癌及前列腺 癌的發生率(Bianchini & Vainio, 2003),主要是因為酒中含有多種不同的多酚成 分,例如:紅酒成分中含有白藜蘆醇 (resveratrol)、花青素 (anthocyanins)、黃烷 3醇類(flavan-3-ols)等,除了具有抗氧化作用外還被發現具有抗癌化、抗細胞增生、 抗癌細胞轉移等功用(Guerrero, Garcia-Parrilla, Puertas, & Cantos-Villar, 2009)。啤 酒 中 的 黃 腐 酚 (xanthohumol) 及 黃 酮 (flavone) 等 多 酚 成 分 也 被 研 究 證 實 為 很 好的抗癌化劑,可以透過抑制血管新生與發炎的訊息傳遞而抑制早期癌細胞生長 及致癌化的過程 (Gerhauser et al., 2002)。近年來這個議題逐漸受到重視,然而因 為造成癌症發生的因素相當複雜,包括:基因、飲食等,因此未來還需要更深入 的研究。 然而攝取過多的酒精除了造成肝臟的破壞之外,也發現容易增加感染的機率, 因為酒精性肝疾病 (alcoholics liver disease)患者比非酒精性肝疾病患者的血液中 有較高的免疫球蛋白,這些免疫球蛋白是因不正常的調控而導致免疫球蛋白過量 產生,所以並不具保護作用反而增加自體免疫 (autoimmunity)症狀的風險(Cook, 1998)。同時,也發現酒精性肝疾病患者容易出現淋巴細胞 (lymphocyte) 數目減 少、自然殺手細胞 (natural killer cell)功能下降等而造成病患的免疫力降低,因 而受到細菌及病毒感染的機會增加及症狀較為嚴重,酒精與肝功能對於免疫功能 影響的機轉也是值得進一步探討的課題。. 53.

(4) 國立臺灣體育運動大學學報 第 4 卷第 2 期(104.06),51 – 66. 参、運動後攝取酒精對生理功能的影響 許多運動員喜歡在運動後攝取酒精飲品,然而並不清楚酒精對於運動後恢復 的影響,所謂運動後的恢復包括了肌肉肝醣的恢復、補充流汗所失去的水分及電 解質、肌肉適應或是修補所需的蛋白質合成、免疫及抗氧化反應等,各方面都與 促進運動員的恢復及健康有關 (Burke, 2010)。其中肌肉肝醣的恢復,除了運動訓 練及比賽的課程外,每天需要攝取每公斤體重 3-10 公克的碳水化合物,尤其是 升糖指數中或高的碳水化合物比升糖指數低的碳水化合物更能促進肝醣的再儲 存 (Burke, Collier, & Hargreaves, 1993),而且在運動後 2-4 小時進行補充的效果 最好。此外,若要成功排除脫水的現象,則決定在運動員喝了多少液體及有多少 可以留在體內,一般需要補充的液體量大約是流失體積的 125-150%,雖然冰水 (0ºC) 最 受 歡 迎 的 , 但 是 冷 水 (15ºC) 比 較 能 被 大 量 且 快 速 的 攝 取 (Hubbard, Szlyk, &Armstrong, 1990)。另外,阻力訓練後補充 20-25 公克蛋白質則會獲得最 大的蛋白質合成反應 (Moore et al., 2009),將有助於肌肉組織的增大及修補。至 於酒精對於運動後生理恢復的影響則分述如下:. 酒精對骨骼肌功能的影響 酒精對於骨骼肌造成許多不好的作用,例如:酒精會抑制鈣離子進入肌細胞, 這將造成無法順利伸展與收縮而降低了肌力的產生 (Cofan et al., 2000)。另外, 酒精會加劇運動誘發肌肉損傷而導致血漿中肌酸激酶 (creatine kinase, CK)濃度 增加並造成力量流失(Spargo, 1984)。臨床上發現出現骨骼肌肌肉病變 (myopathy) 患者中大約 1/3-2/3 是酒精濫用者,尤其以第二型的肌纖維 (又稱快縮肌)最易受 到影響 (Martin & Peters, 1985)。肌肉的束縛感、疼痛及失去本體感覺都是過度 使用酒精的症狀(Prat, Adan, & Sanchez-Turet, 2009)。雖然有些研究指出在進行完 300 次的股四頭肌最大離心收縮之後,攝取每公斤體重 0.5 公克酒精連續兩週後, 不影響後來所進行高強度的離心收縮 (Barnes, Mundel, & Stannard, 2011)。另外 的研究發現在一場美式足球後的幾小時內飲用大量的酒精 (20 標準飲酒單位)不 影響比賽後幾天所測量的肌力、爆發力、肌肉損傷指標及反覆爆發性運動的能力. 54.

(5) 酒精影響運動免疫功能. (Prentice et al., 2014a)。雖然橄欖球員在大量飲酒 (6-20 標準飲酒單位)之後顯著 減少睡眠時間及降低身體動能輸出,但不影響隔天早上無氧運動表現的測試結果, 並於兩天後就完全恢復 (Prentice, Stannard, & Barnes, 2014b)。但有研究發現一般 人在阻力運動訓練後若攝取每公斤體重 1 公克酒精則出現較大的運動誘發肌肉 損傷. (Barnes, Mundel, & Stannard, 2010a) 並 降 低 運 動 肢 的 運 動 表 現. (Barnes,Mundel, & Stannard, 2010b) ,因此,較高劑量 (約每公斤體重 1.0 公克) 在肌肉有損傷的情況下應避免。過去研究也指出比賽時有肢體接觸的運動項目, 肌肉損傷是相當常見的 (Gill, Beaven, & Cook, 2006),由運動表現的觀點而言, 肌肉損傷代表著肌肉的功能下降,表示攝取酒精的時機及劑量都可能會影響到對 肌肉損傷及運動表現的作用,未來需要更進一步的分析與評估。. 酒精對代謝及體溫調節的影響 已 知 酒 精 可 以 藉 由 抑 制 血 管 加 壓 素 (vasopressin)的 分 泌 造 成 周 邊 血 管 擴 張 的作用而產生利尿的效果 (Shirreffs & Maughan, 1997),此作用將促進液體的蒸 發而加速脫水,同時作用在中樞體溫調節而使核心溫度下降。過去研究顯示攝取 每 公 斤 體 重 0.92 公 克 酒 精 會 抑 制 運 動 後 脫 水 恢 復 的 能 力 , 但 若 是 每 公 斤 體 重 0.49 公克酒精則不影響復水 (rehydration) 能力(Shirreffs & Maughan, 1997)。另 一方面,雖然一般的啤酒中含有約每公升 3-5 毫莫耳 (mmol/l)的鈉及每公升 810 毫 莫 耳 的 鉀 , 但 因 濃 度 太 低 所 以 不 具 恢 復 流 汗 所 流 失 的 電 解 質 (Hobson & Maughan, 2010) 的顯著效果。 每公克的酒精約可以提供 7 大卡的熱量,因此,大量攝取酒精將降低後續 的葡萄糖輸出及肝醣形成,在高碳水化合物餐伴隨酒精攝取之後會加速胰島素的 分泌而導致低血糖的現象 (Siler, Neese, Christiansen, & Hellerstein, 1998)。由於 運動開始前幾分鐘的能量來源主要來自肌肉肝醣,當肝臟肝醣用完時,肌肉細胞 開始使用循環中游離脂肪酸 (free fatty acid),但因酒精會降低肝臟中游離脂肪酸 的氧化而干擾了脂肪的代謝,這將降低運動後恢復時所能得到的游離脂肪酸 (Heikkonen et al., 1998)。在連續、間歇高強度阻力運動後,肌纖維蛋白合成的速 率因恢復期攝取大量酒精 (每公斤體重 1.5 公克酒精)伴隨碳水化合物比單獨補. 55.

(6) 國立臺灣體育運動大學學報 第 4 卷第 2 期(104.06),51 – 66. 充蛋白質下降約 37%,若是同時攝取酒精及蛋白質則下降約 24%左右 (Parr et al.,2014)。因此,建議低劑量 (約每公斤體重 0.5 公克)不會對肝醣恢復及復水產 生負面的作用 (Barnes, 2014)。. 酒精對神經系統的影響 酒精會降低中樞神經系統的功能而降低運動的表現,隨著濃度的增加會增長 反應時間、破壞手眼協調性、平衡及較為複雜的技術及細微的動作控制等能力 (Suter & Schutz, 2008)。最近有關壓力與酒精濫用相關性的研究指出以酒精來降 低壓力者,雖然能改變與壓力反應系統有關的 24 小時日夜周期,並影響壓力反 應中促腎上腺皮質激素釋放因子 (corticotropin-releasing factor),進而影響與焦慮 及 壓 力 有 關 的 下 丘 腦 - 垂 體 - 腎 上 腺 (hypothalamic-pituitary-adrenal, HPA) 軸 (McKinney & Coyle, 2006; Sarkar, 2012),然而詳細的機轉仍待釐清。. 酒精對內分泌系統的影響 急性的酒精攝取後會增加胰島素 (insulin)的分泌造成低血糖,進而增加了皮 質 醇. (cortisol) 與 腎 上 腺 素. (epinephrine) 的 產 生. (Souissi, Sesboue,. Gauthier,Larue, & Davenne, 2003)。睪固酮 (testosterone)可以刺激肌肉細胞吸收 胺基酸,並促進蛋白質生成及骨骼肌的適應與再生,後續影響到身體組成改變, 進 而 影 響 到 運 動 後 的 恢 復 。 但 當 睪 固 酮 與 性 賀 爾 蒙 結 合 球 蛋 白 (sex hormonebinding globulin,SHBG)結合時就失去活性,在酗酒的人身上發現性賀爾蒙結合球 蛋白濃度較高 (Vingren et al., 2010),這將降低睪固酮的作用力。另一研究在激 烈 的 有 氧 運 動 後 攝 取 每 公 斤 體 重 1.5 公 克 酒 精 , 則 會 抑 制 睪 固 酮 的 作 用 (Heikkonen et al., 1996)。 不同的發現在阻力運動後攝取每公斤體重 1.09 公克酒精並不影響血漿中睪 固酮、性賀爾蒙結合球蛋白及皮質醇 (Vingren, Hill, Buddhadev, & Duplanty, 2013)。 但另一個研究又有不同的發現,阻力運動後攝取每公斤體重 0.83 公克酒精會導 致 運 動 後 60-120 分 鐘 休 息 期 間 較 高 的 睪 固 酮 及 皮 質 醇 濃 度 (Koziris, Kraemer, Gordon,Incledon, & Knuttgen, 2000)。這些結果的差異可能與攝取酒精的量、年齡、 運動強度等多項因素有關,但對其機轉仍不清楚。. 56.

(7) 酒精影響運動免疫功能. 綜合目前研究結果顯示劇烈的運動加上過多的酒精可能會造成睪固酮分泌 量下降或是功能降低,特別是針對長期過量攝取酒精的運動員更容易對於骨骼肌、 賀爾蒙平衡及身體組成直接或是間接造成負面的影響,將不利於運動後肌肉恢復 的現象。. 肆、運動後攝取酒精對免疫功能的影響 免疫系統擔任身體的防禦機制,主要包括先天性 (innate immune system) 與 適應性 (adaptive immune system) 免疫系統 (Medzhitov & Janeway, 1997; Szabo & Mandrekar, 2009)。先天性免疫系統的主要成員為吞噬細胞 (macrophage)、多 形核白血球 (polymorphonuclear leukocyte) 及自然殺手細胞,主要負責第一線的 防禦工作,引起發炎反應,包括:吞噬作用 (phagocytosis) 、細胞增生與分化, 並 可分泌細胞 激素 (cytokine) 以調 控免疫反應 ,同時吞噬 細胞也是抗 原呈現 細 胞 (antigen presenting cell) 具有呈現病原菌抗原的能力。適應性免疫系統的主要 成員為 T 淋巴細胞及 B 淋巴細胞,由抗原呈現細胞負責活化 T 淋巴細胞,產生 專一性的免疫反應。免疫功能會因為運動造成的能量或是營養不足而受到干擾, 同時酒精會使呼吸道的保護屏障功能不佳,所以劇烈運動後最常出現的症狀是上 呼吸道感染,當免疫系統及抗氧化系統被抑制時,也可能同時對於激烈運動所引 發的肌肉損傷及恢復造成不好的影響,相關的機轉需要進一步的探討。 目 前 由 臨 床 及 實 驗 室 的 數 據 已 顯 示 暴 露 於 酒 精 會 抑 制 免 疫 功 能 (Nelson & Kolls, 2002),包括對不同種免疫細胞及產生各種細胞激素的影響如下:. 酒精對細胞性免疫功能的影響 短時間攝取酒精會降低吞噬細胞移動、黏附、吞噬、產生過氧化物及殺菌的 能力(Zhang, Bagby, Happel, Raasch, & Nelson, 2008)。由過去的研究顯示酒精會 抑制在感染或是發炎期間吸引多形核白血球的能力 (Zhang et al., 2008),主要是 因酒精可透過抑制週邊多形核白血球上 CD18 分子的表現而降低其黏附到內皮細. 57.

(8) 國立臺灣體育運動大學學報 第 4 卷第 2 期(104.06),51 – 66. 胞層進行趨化(chemotaxis)過程,而降低其黏附、產生過氧化物及吞噬的能力。 長 時 間 攝 取 酒 精 會 降 低 自 然 殺 手 細 胞 所 產 生 的 穿 孔 素 (perforin) 及 顆 粒 酶 (granzyme)而 降 低 毒 殺 功 能 , 因 而 降 低 抗 腫 瘤 及 抗 病 毒 的 能 力 (Zhou&Meadows, 2003)。. 酒精對體液性免疫功能的影響 雖然在以酒精加入飲水中供小鼠飲用 8-32 週的動物實驗中發現對於血清中 免疫球蛋白的濃度沒有明顯的改變 (Cook et al., 2007),顯示對於 B 細胞的功能 無顯著之影響。然而過去研究曾指出長期攝取酒精會導致免疫球蛋白 E (immunoglobulin E) 升高,可能因為酒精的代謝物產生了類似過敏原刺激的效果 (Domı´nguez-Santalla, Vidal, Vinuela, Perez & Gonzalez-Quintela, 2001)。另有研究 指出長時間攝取酒精雖然不會明顯降低一般人 B 細胞的數目,但是對於有肝臟 疾病的人則會使其 B 細胞的數目明顯下降,甚至改變 B 細胞對於病原刺激後的 反應能力而對身體產生負面的影響 (Cook, 1998)。. 酒精對細胞激素的影響 長時間及短時間攝取酒精都會增加 T 淋巴細胞的細胞凋亡及降低介白素-17 的分泌(Shellito et al., 2001),介白素-17 扮演連結先天性免液系統與適應性免疫 系統的細胞激素產生,因此酒精將破壞了身體對抗病菌感染的能力。 短時間攝取酒精會減少吞噬細胞所產生的細胞激素腫瘤壞死因子(tumor necrosis factor-alpha, TNF-)、介白素-1(interleukin-1 beta, IL-1)、介白素-6、 顆粒球刺激因子(granulocyte-colony stimulating factor, G-CSF)的分泌(Bermudez, 1994; Goral, Choudhry, & Kovacs, 2004; Zhao et al., 2003),將影響前發炎(proinflammation)反應與發炎反應進而降低免疫反應的效率 (Szabo,Catalano,White,&Mandrekar,2004)。. 酒精對其他免疫因子的影響 短時間攝取酒精會降低抗原呈現細胞的能力。由體內及體外的研究 (Mandrekar, Catalano, Dolganiuc, Kodys, & Szabo, 2004; Szabo et al., 2004)顯示酒 精會 抑制樹突細 胞 (dendritic cell) 共 同刺激分子 (co-stimulatory molecules)的 表. 58.

(9) 酒精影響運動免疫功能. 現及干擾素(interferon-gamma, IFN-)和介白素-12 的表現,這將使得輔助型第一 型 T 細胞 (helper T cell type I) 的分化出現問題,也會對身體的防禦機制造成不 好的影響。 綜合以上之研究顯示過多的酒精會抑制先天性與適應性免疫細胞的功能而 導致受到病原菌感染的機會增加。. 伍、結語 雖然運動後適量的攝取酒精並不影響短時間內運動員的表現,且低劑量不會 對肝醣恢復、復水及肌肉損傷造成危害,然而較高劑量酒精則出現較大的運動誘 發肌肉損傷、影響與焦慮及壓力調控有關的神經系統及抑制免疫細胞的功能。本 文彙整近年來有關運動後攝取酒精對生理功能及免疫功能的文獻,將其影響整理 成圖 1。由上述研究顯示酒精會加劇運動誘發的肌肉損傷、降低睪固酮活性及抑 制多種免疫細胞的功能,可能會因為酒精而造成長期肌肉損傷發炎及免疫系統功 能不佳,進而使運動員陷入易受感染的危險中,影響到運動員的訓練與表現。同 時運動後飲用酒精的時間點、飲用量、距離下次比賽所剩的恢復時間及損傷的狀 況都會改變酒精對運動表現的影響。因此,還是建議運動員在賽季期間減少飲酒 量並充分休息以促進生理及免疫功能之恢復。未來研究方向應針對短期及長期攝 取酒精可能對運動員的運動表現及運動後生理與免疫功能恢復所造成的影響,以 提供運動員使用酒精時更安全且更具體的依據。. 59.

Abstract Alcohol drinking is known beneficial to team cohesion and stress release in athletes. Previous studies have shown that moderate alcohol consumption did not affect sport performance of athletes. However, the amount and intensity of exercise training among studies are different, moreover, there exists significant individual variation regarding the absorption and metabolism of alcohol. Whether alcohol would affect the physiological and immunological recovery after intensive exercise is an important issue. This article reviewed recent studies dealing with the effects of alcohol on skeletal muscle, temperature and moisture regulation, carbohydrate and fat metabolism, nervous system, hormone system, immune cells and cytokines. These results indicated that low dose of alcohol have no negative effect on glycogen resynthesis, rehydration, and recovery from muscle injury. However, high dose of alcohol after high intensity exercise will increase serious muscle damage, decrease the functions of central nerve system, reduce the levels and functions of testosterone, reduce the migration, phagocytosis, cytokine production and bactericidal abilities of macrophages, in addition, reduce the antitumor and antiviral activities of natural killer cells leading to the inhibited immune functions. In conclusion, we advise that athletes with alcohol drinking habit should reduce alcohol consumption to facilitate the restore of physiological and immunological functions after exercise..
