影響台灣地區醫師實施臨床指引意願之因素探討

(1) 臨床指引對於提昇醫療品質應有所助益，本研究初步得到醫療團隊主導者醫 師願意實施臨床治療指引的比例為87%。
(2) 醫師是否願意執行臨床治療指引與下列因素有關：
1. 是否曾經使用過臨床治療指引。
2. 是否曾經有實施臨床路徑經驗。
3. 執業科別中有否論病例計酬案件。
4. 醫院或基層診所醫師。
5. 公立醫院或基層診所醫師。
(3) 指引之特性亦影響醫師實施臨床指引，整體來看，在台灣地區的臨床指引，
最為醫師接受的是：符合學理、證據醫學之學術性，由專科醫學會制定、在運用 時必須在法律事件上對醫師有保護作用、使用過程中必須可以依情況修正而且可 以由期刊及刊物中獲得，並應由公正、自主機構監測及全國性之適用。
(4) 醫師對於新事物的經驗與熟悉度是是否願意實施指引的重要因素。推動之策 略以短程現階段來看，繼續推廣臨床路徑及結合健保局支付制度，促使醫師有 更足夠的使用及接觸經驗，以提升對臨床指引之接受及認同。而中程之推動策略 宜藉由醫院中較為完善之品管功能及醫院醫師對於臨床指引接受程度相對較高 之情況，應可由醫院開始來推動，尤其是意願性較高之公立醫院。而加強對於尚 未接觸過臨床路徑的科別、醫師之宣導均是導入臨床指引之重要策略。長程之推 動策略宜由政府成立國家級之獨立機構(類似美國 National Guideline Clearing House)，結合實證醫學之運用(類似國外 Cochrane Library)，研發、制訂符合證據 醫學、具有彈性、容易取得之全國性臨床指引並包括對於法律層面之探討，以推 廣及監測臨床指引之實行與運用。
綜合上述，本研究提供醫師對實施臨床指引意願性觀點之表達，亦可明白醫師 族群對臨床指引之期盼。期盼可提供相關單位參考。
英文摘要
A study on factors that influence physicians’ willingness for implementation of clinical practice guidelines was performed through a structured questionnaire. In Taiwan，physicians who had the experience on clinical practice guidelines or clinical path will have more intense to perform the clinical practice guidelines.
The other factors are those who work in hospitals especial in public hospitals and who face the 「case payment」during their practice. The characteristics of clinical practice
guidelines that will be favored by physicians in Taiwan are 「match the evidence- based medicine」、「made by association of specialist」、「national level」、「flexible in use」、「protective role in legal events」、「available on journals」 and 「monitoring by an independent organization」.
Suggestions to policy makers、hospital administrators and physicians are described.
