This study aims to investigate the home care nursing users' medical costs, and to explore the home care nursing payment system in Taiwan .
This study focuses on the following three aspects:(1). Analysing the users' medical costs during 2000 to 2004.(2).Evaluating the rationality of the payment system of the home care nursing in Taiwan.(3).
Exploring the coordination between National Health Insurance(NHI)and Long Term Care Insurance(LTCI).
Four major findings of this study are as following:(1)Most of 558 cases in the sample are women and the elderly.(2)Male, elderly ,and who died and those with CVA spent more medical expenses.(3)In the initial stage more home care nursing care should be given.(4)More nursing activity should be provided.
There are three aspects of suggestions can be derined from this study:
1.For the government:the earlier to intervene the cheaper the social costs. 2.For the NHI:(1)more the preventive service and health education should be provided .(2)more staffs should be added for
providing complete home nursing care.(3)more flexible payment system should be applied on contingency.
3.For Bureau of Nursing and Health Services Development:More action programs of preventing chronic diseases, specially CVA ,should implemented.
