High-glucose environment downregulate thrombomodulin expression that promotes diabetic wound healing via toll-like receptor 4 pathway

Diabetes causes the impairment of wound healing process which involves hemostasis, inflammation, and angiogenesis. Our previous studies demonstrated that membrane-bound thrombomodulin (TM) play a role on mediating keratinocyte differentiation, and the soluble TM (sTM) released by rhomboid serine protease has the effects on promoting cutaneous wound healing. However, the role of TM on diabetic skin remains unclear. In this study, we found the protein levels of TM and toll-like receptor 4 (TLR4) were significantly downregulated in high-glucose cultured human keratinocytes and diabetic patients’ primary keratinocytes.

Treatment of TNF-α antibody reversed the high-glucose caused TM downregulation;

furthermore treatment of TNF-α decreased the TM expression in a normal-glucose condition.

High-glucose environment also abolished the wound-triggered TM upregulation and sTM production in both HaCaT cells and streptozocin (STZ) diabetic mice. Notably, treatment of recombinant sTM could upregulate the TLR4 expression and promotes cutaneous wound healing in STZ mice, but these benefits were lost in Tlr4-deleted mice that with delayed healing rate in early inflammatory stage. Taken together, high-glucose environment may cause the inhibition of membrane-bound TM expression through the TNF-α pathway, and sTM should function as an inflammatory mediator on promoting diabetic wound healing through a TLR4-dependent pathway.
