第六節 影響乳癌術後患者執行功能性活動的相關因素之預測分析

有關影響乳癌術後患者執行功能性活動的相關因素,本研究對乳癌術後患者的年 齡、教育程度、腫瘤分期、施行腋下廓清術、放療有無、化療有無、居家施行運動次數 與癌因性疲憊程度進行相關及差異比較分析,結果已詳述於本章第三節,針對統計結果 呈有顯著的影響患者術後不同時期執行功能性活動的因素,以逐步複回歸分析法 Stepwise regression analysis),以預測影響對患者執行功能性活動的變項,結果分別說明 如下:

一、乳癌患者的相關變項對術後0~5週執行功能性活動之預測力

前述研究結果指出本項乳癌術後患者執行功能性活動與運動次數和FBI-T有顯 著的相關,詳見「表4-3-1」,由「表4-6-1」步迴歸分析結果,選入迴歸方程式的 顯著變項,顯示居家施行運動次數對術後0~5週執行功能性活動的解釋力為9%。

二、乳癌患者的相關變項對術後6~10週執行功能性活動之預測力

前述研究結果指出本項乳癌術後患者執行功能性活動與教育程度和化療有無 有顯著的相關,詳見「表4-3-1」,由「表4-6-1」逐步迴歸分析結果,選入迴歸方 程式的顯著變項,顯示教育程度對術後6~10週執行功能性活動的解釋力為10%。

三、乳癌患者的相關變項對術後0~10週執行功能性活動之預測力

前述研究結果指出本項乳癌術後患者執行功能性活動與運動次數和癌因性疲 憊程度有顯著的相關,詳見「表4-3-1」,由「表4-6-1」逐步迴歸分析結果,選入 迴歸方程式的顯著變項,顯示癌因性疲憊程度對術後0~10週執行功能性活動的解釋 力為12%。

第七節 影響乳癌術後患者運動次數的相關因素之預測分析

有關影響乳癌術後患者居家運動次數的相關因素,本研究對乳癌術後患者的年 齡、教育程度、腫瘤分期、手術方式、施行腋下廓清術、放療有無、化療有無、與 功能性活動進行相關及差異比較分析,結果已詳述於本章第四節,針對統計結果呈 有顯著的影響患者術後 不同時期居家運動次數的因素,以逐步複回歸分析法 (Stepwise regression analysis),以預測影響對患者居家運動次數的變項,結果分別說 明如下:

二、乳癌患者的相關變項對術後 0~10 週居家運動次數之預測力

前述研究結果指出本項乳癌術後患者居家運動次數與腫瘤分期和活動功能 有顯著的相關,詳見「表 4-4-1」,由「表 4-7-1」逐步迴歸分析結果,選入迴 歸方程式的顯著變項,顯示腫瘤分期對術後 0~10 週居家運動次數的解釋力為 8%;活動功能對居家運動次數的解釋力為 12%。

第五章 討論

使癌因性疲憊的發生(McAuley, White, Rogers, Motl, & Courneya, et al, 2010)。因此需要鼓 勵癌友於治療過程中或後,在身體許可能力範圍內,盡量還是要從事一般居家日常活 能有顯著進步(Morimoto, et al, 2003)。Daley針對乳癌術後執行復健運動之研究,也發現 執行復健運動組的功能活動得分比只接受常規護理組較高(Daley, et al, 2007)。另一項乳 癌術後執行徒手淋巴引流、運動、加壓治療以及皮膚護理之研究中,也發現患者上肢失 能狀況顯著改善(Do, et al, 2015)。

此研究結果得知術後患者疲憊分數越高時,術後十週內功能性活動進步就越多。

NCCN指出疲憊是最常見也最令患者苦惱的症狀,會嚴重影響到身體活動及日常生活。

作,因此患者自我意識到復健運動之重要性,並特別依照治療師建議施作運動次數,養 成自行在家做復健的習慣。

第六章 結論

本研究採縱貫式研究方式,瞭解乳癌患者施行居家復健運動對於癌因性疲憊之影 響,並針對乳癌患者術後,哪些因素會影響癌因性疲憊、居家運動次數以及功能性活動 做相關探討。茲將研究結果歸納以下結論:

ㄧ、乳癌術後患者癌因性疲憊與基本資料、居家運動次數之相關性

對乳癌術後患者的癌因性疲憊最具預測力的變項為居家運動次數。當乳癌患者 施行居家運動次數越多時,其疲憊減輕就越多。

二、乳癌術後患者功能性活動與基本資料、居家運動次數和癌因性疲憊之相關性 當乳癌患者術後施行居家運動次數越多、術後疲憊分數越高、教育程度越低、

無進行化學治療者,其功能性活動分數就進步越多。對乳癌術後患者的功能性活動 最具預測力的變項包括:教育程度、居家運動次數以及術後疲憊分數。

三、乳癌術後患者施行居家運動次數與基本資料、功能性活動之相關性

當乳癌患者腫瘤分期越大、手術範圍越大、有進行腋下淋巴廓清、術後執行功 能性活動越困難者,其執行居家運動的次數就越多。對乳癌術後患者執行居家運動 的次數最具預測力的變項包括:腫瘤分期、術後功能性活動。
