5.3 未來研究方向
本研究採用的健保資料庫資料有大樣本且具代表性的優點,但也如上述所提 出之研究限制,我們可以看到本研究仍有許多研究限制尚須改進,舉例來說,抽 菸、酗酒等相關風險因子若能一併考慮進本研究中,並在探討慢性腎臟病與梅尼 爾氏症之間的風險關係時,能夠予以校正,如此,所提出的研究結果會更有可性 力與精確性,再者,若能取得病患的臨床生化值,那麼對於進一步疾病探討的準 確度與關係機制可能有進一層探討的可能性,當然的在本研究中,慢性腎臟病引 發梅尼爾氏症的假設情況,雖有樣本統計數據的證明,而更明確的內在因素、機 制的探討上卻也有許多問題需要進一步被探討與了解。
在本研究中,未來尚需要進行考量的部分還有下列幾點:
一、 在合併症的校正上,於章節 1.5.2 中所蒐集的文獻提到梅尼爾氏症的風險 因子中還包含了過敏、病毒性感染在本研究中並未被考慮校正,原因在於 過敏、病毒性感染所包含的疾病範圍也相當的廣,因此在未來,關於這兩 項因子與梅尼爾氏症有所相關的文獻蒐集以及進一步校正也應該被包含 於此慢性腎臟病與梅尼爾氏症風險關係的研究中。
二、 於本研究中屬於自身免疫性疾病的皮肌炎以及多發性肌炎的人數有較少 的情況,人數較少的合併症是否應被考慮包含於本研究中且是否具有討論 統計分析的意義,在未來人數過少的合併症是否應該移除也是需要進一步 考慮的一項問題之一。
三、 在本研究 1.5 相關研究探討章節中,我們提到一些藥物的使用,像是利尿 劑使用於慢性腎臟病患用於治療水腫,慶大霉素用於腹膜透析病患的腹膜 炎感染問題等,這些藥物的使用可能會導致耳毒性,像是眩暈、聽力損失、
耳鳴等類似梅尼爾氏症的症狀,而這些藥物於腎病患中的使用是否有可能 是導致梅尼爾氏症的發生因素,在未來的研究中也可一併使用健保資料庫 所擁有的藥物檔進行分析討論。
