正位心臟移植手術病患其術後身體活動度與心率變異度相關性之探討

 本研究主旨在了解正位心臟移植病患與健康成人心率變異度之差異及探討正位心臟移植

病患身體活動度與心率變異度之相關。本研究為一描述性、相關性的横斷性研究，收集接受正位心臟移植手術後一年之個案及與其年齡、性別與身體活動度相配對之健康成人為對照，以比較兩組個案心率變異度之差異。使用七日身體活動回憶量表調查研究對象在過去一週的身體活動程度，以探討身體活動度與心率變異度的相關性。共收集正位心臟移植病患 60 名，健康成人 60 名。研究資料分析以次數、百分比、平均值、標準差呈現描述性統計，再以 t-test 、 Chi-square 、 Pearson correlation 進行檢定。

 結果發現心臟移植患者幾乎各個心率變異度之參數值包括 mean 、 sdr 、 TP （ ms2 ）

、 LF （ ms2 ）、 LF （ nu ）、 HF （ ms2 ）、 LF/HF 數值均小於健康成人，並達統計上之顯著差異（ p <.05 ），心臟移植病患之心率變異度中， TP （ ms2 ）、 LF （ ms 2 ）、 HF （ ms2 ）及 LF/HF 之變異度僅為健康人之 17.8% ， 2% ， 7.3% 和 44% 。可見心臟移植患者歷經手術去神經化過程後的心率變異度顯著低於健康成人。心臟移植患者週相對中度以上身體活動量與 sdr （ r=.409, p=.015 ）、 TP （ ms2 ）（ r=.428, p=.0 1 ）、 HF （ ms2 ）（ r=.422, p=.012 ）呈顯著正相關，卻與 LF （ nu ）呈負相關（ r=-.

337, p=.048 ）；同樣的週絕對中度以上身體活動量亦與 sdr （ r=.424, p=.011 ）、 TP

（ ms2 ）（ r=.453, p=.006 ）與 HF （ ms2 ）（ r=.466, p=.005 ）呈顯著正相關。研究結果顯示中度以上身體活動量愈大心率變異度愈佳。

 本研究結果可提供護理人員對於心臟移植患者照護之參考，以提升心臟移植病患照護之

品質。

The Relationship Between Physical Activity and Heart Rate Variability in Orthor opic Heart Transplant Recipients

 The purpose of this study was to investigate the relationship between physical activity and hea rt rate variability in orthoropic heart transplant recipients, the difference of heart rate variabilit y between the the client under orthoropic heart transplant for one year and the healthy matchin g age, gender and physical activity with orthoropic heart transplant recipients. Seven-day Phys ical Activity Recall questionnaire was used to collect the subject’s physical activity amount pe r week. The data were analyzed by percentage, mean, standard deviation, frequency, t-test, Ch i-Square, and pearson correlation.

The major findings of this study were the following: heart transplant recipient’s heart rate vari ability in terms of mean, sdr, TP(ms2), LF(ms2), LF(nu), HF(ms2), and LF/HF were significan tly less than the healthy

(p<.05). Heart transplant recipient’s heart rate variability parameters including TP(ms2), LF(m s2), HF(ms2)and LF/HF’s data were only at the healthy’s 17.8%, 2%, 7.3% and 44%; respecti vely in the case of heart transplantation, relative amount of moderate physical activity was pos itively related to the sdr (r=.409, p=.015), TP(ms2)(r=.428, p=.01) and HF(ms2) (r=.422, p=.0 12), but negatively related to the LF(nu) (r=-.337, p =.048). Absolute amount of moderate phy sical activity was also positively related to the sdr (r=.424, p=.011), TP(ms2)(r=.453, p=.006) and HF(ms2)(r=.466, p=.005). The result confirms the more amount of moderate physical acti vity, the better heart rate variability is.

The result of this study would be valuable for caring and enhancing the quality of health care of heart transplant recipients.
