cytokines)IL-6與腫瘤壞死因子(tumor necrosis factor),與憂鬱症患者身上所 發現的促炎性細胞因子IL-1、IL-6以及干擾素(interferons)相似(Chen et al., 2013)。此前瞻性議題亦為後續研究可予以探討的議題,而由本研究所得的結 果也提醒後續研究者需注意,並釐清促炎性細胞因子影響是否造成嗜睡症狀,

進一步影響憂鬱情緒。

第七節 臨床應用

本研究在於提醒臨床工作者對 OSA 患者進行憂鬱症臨床診斷時,需注意 與釐清 OSA 患者是否以白天嗜睡的情況,來回應憂鬱症狀相關的嗜睡表現,以 降低誤判 OSA 患者憂鬱情緒的可能性。

過去研究者指出 OSA 患者(每人 C$118)在憂鬱症診斷下所花費的醫療 費用比一般大眾(每人 C$224)來的低,卻在抗鬱劑上所花費的費用比一般大 眾來得高(OSA 患者:C$324.76、一般大眾:C$232.21),研究者將此現象解 釋為一般大眾較傾向於以參與心理治療的方式進行治療,而 OSA 患者嗜睡的情 況則可能影響心理治療取向的參與而採取藥物治療的方式(Smith et al., 2002)。

上述研究結果說明了,相對於一般大眾,OSA 患者更常使用藥物治療,而本研 究認為 OSA 患者使用在抗憂鬱劑上的花費較高,另一可能性來自於 OSA 患者 憂鬱症診斷被高估所致,此現象亦可能使得抗憂鬱劑的效果被降低。因此,釐 清 OSA 患者是否以白天嗜睡的情況,來回應憂鬱症狀相關的嗜睡表現,以降低 誤判 OSA 患者憂鬱情緒的可能性,亦會降低醫療成本。
