第五章 結論與展望

5.3 未來展望

物,會增加罹患慢性腎臟病的共病,如:糖尿病、高血壓、高血脂、心臟病的勝算,

是否因藥物關係先導致糖尿病、高血壓、高血脂、心臟病之慢性腎臟病共病,之後 才導致慢性腎臟病,其生物機制有待更進一步實驗確認。

5.2 研究限制

健保資料庫含有大量病人就診資料,促使近幾年大量被發表探勘,以流行病學 統計方式找出疾病的關聯性,提供臨床醫生或是生物學家做進一步實驗之先備知 識。健保資料庫百萬人口歸人檔其資料非常寶貴,但是並未提供對於精神分裂症與 慢性腎臟病之研究定義,甚至於慢性腎臟病的共病資料如何取得與定義,皆未明確 指示,而本研究之研究設計與統計方法,皆是參考大量健保資料庫已發表文獻,或 是流行病學相關書籍其做法而加以調整修改,其健保資料庫並未有生化值、抽菸習 慣、飲酒習慣之病人生活型態之紀錄,這些限制或許將來可以用問卷或是臨床實驗 加以分析探討。

5.3 未來展望

本研究是第一個利用台灣健保資料庫百萬歸人檔,使用流行病學數據統計分 析方式,指出精神分裂症病患有較高的風險罹患慢性腎臟病,並與精神分裂症病患 服用抗精神病藥物無統計上顯著相關。本研究希望提供臨床醫生或生物學家為一 研究新發現,是否有其生物機制可依循,有待醫生或生物學家進一步探討。
