◎ 報名注意事項
壹、報名方式及資料
一、報名資料請於報名截止日 2017 年 12 月 25 日前，以下述兩種方式，擇一方式進行報名：
1. 郵寄報名：請影印本報名表，並妥善填寫後，將報名表及所需文件資料，放入附件 之報名專用信封內，黏牢密封，郵寄至以下地址：

2. 線上報名：請輸入下方網址或掃描 QR Code，進入網頁後，點選「寒假營隊報名 系統」，依步驟完成報名表，列印並填妥所需文件資料，再將所需文件 資料掃描或拍照（請確保上傳的文件內容清晰可讀），壓縮成 .zip 檔，
上傳送出。

二、所需文件資料包含：
1. 營隊報名表
2. 家長（監護人）同意暨責任切結書 3. 2 吋大頭照兩張
請於照片背面填寫姓名，一張浮貼於報名表上做為申請之用，另一張以迴紋針 固定於報名表上方，用於製作營隊結業證書
4. 繳費收據證明影本
三、利用線上報名者，請於營隊活動報到當天，攜帶上述所需文件資料（營隊報名表除外）
紙本繳交。
四、營帽 T 尺寸請參考營帽 T 尺寸表（附錄），營隊期間將不提供更換尺寸服務，請務必 謹慎測量挑選。
五、主辦單位將以營隊報名表資料進行聯絡、辦理保險及製作結業證書等，請務必正確且完 整填寫。
六、報名表件只須寄回上述所需文件資料，其餘相關說明請留存以供學員隨時聯繫及備查之 用。
線上報名系統 附錄（一） 報名注意事項

貳、繳費說明
一、本營隊採用中華郵政「匯款」或「轉帳」方式進行營隊報名費用繳費，匯款人欄位請務 必填寫參加學員姓名（如例圖所示），以便主辦單位進行報名資料對帳之用。
二、繳費匯款帳戶
戶名：國立雲林科技大學工業工程與管理系系學會紀雅婷 帳號：03010080767133
三、繳費確認資訊 1. 匯款者：
須提供匯款人姓名、匯款日期時間、學員姓名 2. 轉帳者：
須提供轉帳帳戶後五碼、轉帳日期時間、學員姓名
四、請於繳交報名表前完成繳費，並附上繳費收據證明影本，未完成繳費者視同放棄報名。
參、團報優惠方案
一、為了鼓勵學員與其同學、朋友一同參與營隊活動，認識雲科大工管系，主辦單位設有團 體報名之優惠。
二、報名費用：
1. 一人單獨報名：新臺幣 3,200 元整
2. 二～四人團體報名：每人新臺幣 3,000 元整 3. 五人以上團體報名：每人新臺幣 2,800 元整
三、申請團報優惠者請務必在營隊報名表內清楚勾選，並註明所有團報學員姓名。
四、團報學員之間請儘量不要相隔太久才繳齊資料，以方便作業。

肆、退費規定
一、匯款繳費後，如因個人因素不克參加本營隊需退費者，依據提出退營申請時間，分為以 下三階段退費標準：
1. 2017 年 12 月 25 日 ( 一 ) 23 : 59 前，予全額退費 2. 2018 年 01 月 01 日 ( 一 ) 23 : 59 前，予退費五成 3. 2018 年 01 月 02 日 ( 二 ) 00 : 00 後，不予退費 二、學員未赴本營隊且未事前請假或通知主辦單位，不予退費。
三、營隊期間，中途有事必須離開，視為個人因素，不予退費。
四、營隊期間如違反營隊規定，而被迫退出營隊者，不予退費。
五、營期前如遇不可抗力之天然災害（參考雲林縣政府公告之停課標準），主辦單位有權決 定營隊是否如期舉辦，並公布於營隊臉書粉絲專頁，得視情況退費。營隊期間如遇不可 抗拒之天然災害亦同。
六、上述規定因關係到相關食宿、保險、營外套、講義製作等費用處理問題，主辦單位將扣 除已支付費用後再行退費，不便之處敬請見諒。
七、若退費者為團體報名，且退費後將影響其他學員團報資格，其他學員之報名費差價將由 退費者之退費中扣除支付，其剩餘金額依第肆節第一條規定退還。
八、營隊期間，因活動受傷，而被迫提前離開營隊，由主辦單位退還未使用之食宿費用。
九、若發生重大天災 ( 如：地震 ) 導致營隊取消，得全額退費。
伍、營隊錄取說明
一、本營隊最高人數為 80 人，如超過報名人數，篩選錄取學員名單將依據下列原則：
1. 報名表件填寫之完整性
2. 主辦單位收件之先後順序（以郵戳為憑）
二、主辦單位將在收到款項及繳費資料後五天內，以電子郵件通知繳費完成及錄取狀態。並 在 2018 年 01 月 01 日（星期一）於臉書粉絲專頁公告正備取名單後，再由主辦單位 透過電話連絡方式，通知正取學員以確認意願，其後依序完成備取學員之遞補工作。
三、營期前會以電子郵件寄送行前通知，並以電話聯絡確認重要事項。
四、為保障您的權益，如未在以上時限內收到通知，請主動聯絡我們。

柒、其他聲明
一、本營隊所有報名表件內所記載之個人資料，皆用於 2018 國立雲林科技大學工管系專業 人才培訓營「大鬧天工」之使用範圍內（含主辦單位聯繫確認、營隊資料郵寄、辦理保 險及營隊活動必需使用之等），主辦單位同意遵照中華民國法務部頒布之「個人資料保 護法」使用規範，並致力保護我們所持有之個人資料的隱私、保密性和安全性，不另作 其他用途。本營隊活動結束後主辦單位亦將所有個人資料依相關法規進行銷毀，敬請安 心填寫。
二、本營隊因不可抗力之特殊原因無法執行時，主辦單位有權決定取消、終止、修改或暫停 活動。
三、如有未盡事項，主辦單位得隨時修訂並於營隊臉書粉絲專頁公布之。

附錄（二） 報名表
2018 國立雲林科技大學工管系專業人才培訓營「大鬧天工」
家長（監護人）同意暨責任切結書
本人 茲同意子女 於 2017 年 01 月 25 日（四）至 2017 年 01 月 28 日 ( 日 ) 參加國立雲林科技大學工業工程與管理系系辦公室與國立雲林科技大學工業工程 與管理系系學會共同主辦之 2018 國立雲林科技大學工管系專業人才培訓營「大鬧天工」，並督促 子女全程參與課程及活動、遵守相關規定與安排，並委託主辦單位盡本次營隊照顧事宜。
若本人子女於營隊期間不遵守營隊之規定，而造成任何意外危險，及營隊結束後的任何行為，
願由本人承擔全部責任，概與主辦單位無關。
營隊相關規定
一、營隊活動期間，學員應確實遵守營隊規則，聽從主辦單位之指示，以安全為重要考量，學員不 擅自外出、不單獨行動。
二、若子女於營期間出現脫序行為，或足以影響其他學員權益之行為，主辦單位有權予以退營處理 並通知家長，且不退還任何費用。
三、本營隊期間不得擅自離營，若因故須離營時應填寫離營切結書，並由主辦單位向家長聯繫得完 成離營手續。
四、本營隊將於結業式頒發結業證書。未參與三分之二以上營期者（包含活動與課程），恕不頒發 結業證書。
五、家長應將學員之健康狀況及應注意事項預先告知主辦單位，如有私人藥品需自行攜帶並提供藥 物或器材之使用方法。
六、為配合政府規定得全部或部分停止本營隊活動（如發生各項天災），主辦單位將依實際狀況通 知家長活動停止辦理。
