原雇主有下列情事之ㄧ（請擇一勾選），本人（外國人）自 年 月 日起由新雇主接續聘 僱，並自接續聘僱起始日負雇主責任、繳納就業安定費及依相關規定辦理：

備註：1.本證明書 1 式 5 份（如原雇主死亡 1 式 4 份），由簽署人各收執 1 份，新雇主應檢附本證 明書及其他規定文件，依規定期限通知當地主管機關實施檢查及向中央主管機關申請接續 聘僱許可。
2. 如原雇主死亡，請勿於原雇主簽名欄位簽署任何文字。
3. 如原雇主於聘僱許可期間死亡，與原被看護者有符合規定親屬關係者，應於原雇主死亡日 起 60 日內通知當地主管機關實施檢查及向中央主管機關申請接續聘僱許可。
4. 如原雇主於取得招募許可後至外國人未入國前死亡，與原被看護者有符合規定親屬關係者，
應於外國人入國後 3 日內通知當地主管機關實施檢查及於外國人入國後 15 日內向中央主管機關申請 接續聘僱許可。
