社區型糖尿病照護與衛生教育資訊系統

國內一般民眾就醫時，普遍的經驗為醫師看病的時間短，對於病患疾病和用藥旳 說明不足。同時民眾對於個人切身的問題中以病患所罹患的疾病、所服用的藥品 以及完整的病歷說明最為重要，且應為民眾所瞭解。隨著人口的老化，糖尿病個 案正逐年增加，已高居國人十大死因之第五位，對國人健康威脅是相當嚴重。而 糖尿病的病患除了對服用的藥品瞭解之外，還需要透過衛生教育以達到病患自我 照顧的能力，故教導糖尿病病患正確的用藥和各種衛教項目是必需的。
本論文以醫院為主建立社區型糖尿病照護與衛生教育資訊系統輔助，對於就診的 糖尿病病患所提供的 e 化的系統。由醫療院所的衛教人員針對糖尿病所做的衛教 計劃加以電腦化。以個案管理的方法，落實個人化照護概念。透過醫院提供上網 的帳號與密碼，使病患或其家屬透過網際網路取得病患在醫療院所的衛教計劃內 容，使病患或其家屬容易取得病患所需的衛教項目和用藥的說明和針對病患設定 的短期目標。提供病患輸入血糖、體重、血壓等自我監測項目的介面，透過設定 的標準值做為監測數值的回傳時警示，以利回診或衛教人員追踪使用。給予用藥 說明衛教內容，指導病患正確服藥，以控制病情。提供日後研究分析，建立醫院 糖尿病照護衛教標準。
本研究的研究方法即將糖尿病治療方法加以分類、併發症與衛教文件相關性，建 立標準衛教項目的組合，以利衛教計劃的完成。再由醫療資訊系統取得病患就診 時的藥物和檢驗資料以供病患及其家屬查詢。由衛教人員依據轉介醫師的轉介單 得知病患治療方法和藥物記錄同時加入適當的衛教項目和設定短期目標和自我 監測項目
針對糖尿病的衛教計劃加以電腦化，助於病患資料的完整性與連續性及節省評估 和書寫時間。且透過個案管理的作業瞭解糖尿病病患個人資料、病史、健康資料 做為日後統計分析的依據。同時對自我監測項目的數值做為往後追踪的機制。使 醫療院所對病患的服務的窗口更加多元化。
對於糖尿病患本研究提供個人化的衛教資訊能提供持續的照護，同時對於衛教人 員來說將糖尿病衛教計劃電腦化，易於日後統計分析、節省衛教人員手寫的時 間。本研究未來的研究方向可結合設備儀器的運用，由病患在家執行血壓或血糖 等監測項目，將取得的數據透過設備儀器自動回傳醫療院所，減少人為的錯誤與 麻煩。
英文摘要
When internal ordinary people take medical treatment, their common experience is
seeing a doctor for a short time. As regards their disease and medication, they give insufficient explanation. At the same time, the most important thing is that patients explain diseases, medication and complete case history of their personal concern.
And they should understand it’s real significant. The coming of population ageing, Chronic illness is in vogue rate also year by year increases, namely Diabetes.
Diabetes is fifth of our compatriot ten big death causes; the threat of compatriots’
health is quite serious. Also diabetics should be enable to take care of self expect understanding of medicinal uses. So it’s necessary to teach diabetics correct medicinal uses and education of various health cares.
It takes hospitals as the principal part of community-based information system for diabetes care and education and it provides electronic department system for diabetics who come to see a doctor. Because staffs in hospitals are directed against Diabetes to make health care plans by computerization, diabetics and their family can get health care plans content through internet. And diabetics and their family also acquire patients who want items of hygiene education and short-term target easily. Then they will have more ability to care for one. Furthermore it provides medicinal uses and numerical analysis of selfhood monitor for patients to go back to a doctor or for staffs in hospitals to chase it. It is diversification of service windows for patients.
The study is mainly for diabetics care and education plans by computerization to keep patients’ data completeness and continuity, and to save time of evaluation and writing. Using assignment of case management, we can know patients’ personal data and history about diabetics. According to document management, it will help statistic analysis and ways of treatment for complications, and then it takes standard items of hygiene education.
