進言之,居家照護模式下的醫療人員,於浸入差異的地域文化與不同病家的家庭文 化間,若能與病家締結緊密的關係脈絡,融入棲居地域中形成整體的支持網絡,於這樣 深厚的基礎下,醫護人員更能針對病家及照顧者會經歷到的困難,提供及時與有力的支 持。居家療護小組亦可針對居家照顧者將可能面臨的醫療照護困難與臨終時刻醫療抉擇 的困境,啟動早一步、預防式的討論,試著減緩緊急時刻來臨時的艱難。面對疾病帶來 生命斷裂,召喚出家庭中每個人不同的情緒、想法、或擔心,醫療人員也能適時啟動家 庭成員間彼此開放坦承的對話,協助共同面對如此的不安處境。
對同為相伴生命末期患者(及照顧者)的醫療人員而言,如此伴行的經驗,也可能 產生自我的心理沈潛、召喚出自我主體經驗的回應。而如何從中學習轉向回到自身的生 活方式,形塑自我照顧的技藝,也是彼此需共同面對的生命課題。
第二節、研究限制與建議
本研究為對鄉村型態居家照顧者處境之初探,以研究訪談的受訪者而言,目前僅針 對單一鄉村型態田野之研究探究也許對居家照顧處境整體經驗的深化性尚不足,但於結 果分析後發現透過居家視域可窺見鄉村型態照顧者獨特的棲居狀態,且不同夙緣關係間
(帶有歷史性的心理時間差異)的主要照顧者,可能會經歷不同樣態的心理經驗。仍可 為與其相似之家屬照顧者及相關醫療人員提供參考與思考對其可能有效的幫助行動。
未來,若要針對社區安寧照顧者有更進一步的理解與探究,依據本研究結果帶出的 處境樣貌做進一步延伸,可針對不同夙緣角色的居家照顧者進行深度訪談,甚或可專注 於單一家庭中主要照顧者及協同照顧者的經驗探究,進而帶出更深層的照顧者棲居經驗,
及照顧事件於家庭中對不同位置個體角色的動力流動。甚至於不同田野的居家照顧樣態,
更廣度地、深度地探索居家照顧者的置身處境,進一步對鄉村居家照顧樣態有更深的理 解與對照。另,從本研究受訪者經驗中發現,於居家照顧處境中,外籍看護的存在,亦

成為居家照顧中特殊但普遍的現象,外籍看護的工作與其說像一種「良心工作」,其實 也像另一種倫理道德的呈現,其於居家照顧處境裡的位置與帶出的關係樣態,亦為未來 研究可進一步探究之處。
