2 摘 要 利用有色兔子於玻璃体內注射鹽基性芽細胞生長因子以引起眼內新生血管 形成,利用螢光眼底血管攝影檢查(Fluorescein angiography)顯示眼內新生血管形 成出現於鹽基性芽細胞生長因子注射後第 3 或第 4 週達成長旺盛期,而後逐漸退 縮。組織學上顯示出有纖維組織增生的變化。 關鍵字:實驗性眼內新生血管、鹽基性芽細胞生長因子
SUMMARY
Experimintal intraocular neovascularization was induced by intravitreal injection of basic fibroblast growth factor in the pigmented rabbit eyes. Fluorescein
angiography showed intraocular neovascularization fully developed 3 to 4 weeks after intravitreal injection of basic fibroblast growth factor and then gradually regressed. Histological examination showed fibrous tissue formation.

4 前言 多種視網膜疾病可造成眼內新生血管形成,引起視力的嚴重受損,其中 糖尿病視網膜病變在先進國家中為引起中老年人視力受損的主因。 目前的研究認為眼內新生血管形成和多種生長因子有密切的關係,最近 國外的研究於免眼玻璃体內置入血管內皮細胞生長因子 (vascular endothelial growth factor, 簡稱 VEGF) 可造成眼內新生血管形成。目前已知和眼內新生 血管形成相關的生長因子包括血管內皮細胞生長因子,鹽基性芽細胞生長因 子(basic fibroblast growth factor , 簡稱 bFGF ),表皮生長因子(epidermal growth factor , 簡稱 EGF ),類胰島素生長因子(insulin-like growth factor, 簡 稱 IGF )和多種其他的生長因子。
本研究主要目的為建立新的眼內新生血管形成的動物模式,使用生長因 子的 bFGF 注射於兔眼玻璃体腔中造成眼內新生血管,於此動物模式建立完 成後,可做為日後臨床上探討不同治療方法對其抑制作用之評估。
材料與方法 本研究動物實驗方面使用的動物為成熟有色兔子,体重為 2.5 至 3.5 公 斤,於 Katamine 和 Rompam 全身麻醉後進行實驗。眼內新生血管動物模式 的建立為將不同劑量的 bFGF 注射於兔眼視神經盤前方的玻璃体腔之中,而 產生眼內新生血管的形成。所有兔子皆選擇右眼做實驗,實驗組於兔眼注射 生長因子,而對照組則注射緩衝溶液。 所有兔子皆於注射生長因子之前,注射後第 1 週、第 2 週、第 3 週、第 4 週和第 6 週做眼底照相和螢光眼底血管攝影檢查,觀察眼內新生血管的形 成過程。兔子在適當的時段於麻醉作用後,做眼球摘除手術,將眼球置於固 定液之中,而後以角鞏膜剪刀去除眼球前部和玻璃体,再仔細取下必要的部 位,利用光學顯微鏡以觀察新生血管形成的情況。 對於眼內新生血管形成抑制作用的研究為使用成熟有色兔子於雷射光凝 固作用前先做螢光眼底血管攝影檢查,螢光眼底血管攝影檢查由兔子耳靜脈 注射 100mg/Kg 的螢光素溶液,觀察視網膜血流的情形。而後,使用氬雷射 於各兔子選擇右眼做光凝固作用,光凝固作用的時間為 0.1 秒,凝固斑的大 小為 200 微米,能量的選擇為 150 毫瓦特,於各眼網膜上做 400 個雷射斑。 做完雷射光凝固作用後,立即於玻璃体內注射 bFGF,而後依上述時間間隔, 每隻兔子皆檢查 6 週,觀察眼內新生血管形成的時間及狀態。
6 結果 對有色兔子做玻璃体內鹽基性芽細胞生長因子 2000ng 注射作用後,螢 光眼底血管攝影檢查顯示眼內新生血管出現於玻璃体內注射後第 3 或第 4 週 達成長旺盛期,而後逐漸退縮。組織學檢查顯示出纖維組織增生的變化。 對有色兔眼做全視網膜 光凝固作用後 , 再於玻璃体內注射 2000ng bFGF,結果顯示出半數的兔眼未形成眼內新生血管,其抑制作用為 50%。
討論 眼內新生血管於多眼底疾病扮演重要角色,其中糖尿病視網膜變性於開 發國家中為引起中老年人失明的四大主要原因之一。目前由於人類生存年齡 的增加,對此症的研究也更具重要性。此症引起視力受損最主要原因為眼內 新生血管形成所引起一系列的變化所造成。 引起眼內新生血管形成的原因仍未明,目前認為多種生長因子可能和它 的形成有關但仍未能完全明瞭,可能和沒有理想的動物模式有關。本研究為 使用兔子做研究是新發展出的模式。本研究顯示出兔子眼內新生血管形成和 鹽基性芽細胞生長因子呈明顯相關性,利用本研究所發展出來新的動物模式 可做為將來進一步研究及治療之用。 全視網膜光凝固治療為目前最常用於治療眼內新生血管的方法,通常分 成數次施行,而每次施行之間的間隔約為一星期,但最想的狀態應為每次光 凝固施行所引起的消炎狀態消失之後,再做第二次光凝固作用,對眼球所可 能引起的損害作用最小。光凝固作用後對視網膜新生血管的抑制作用機轉仍 未明,視網膜新生血管的形成目前認為和生長因子 VEGF 及 bFGF 息息相關, 先前的研究顯示出凝固作用後玻璃体內 bFGF 有上昇的現象,其和光凝固作 用機轉間的關係仍需將來進一步的研究始能確定。
