第四節 研究建議與未來發展
整體而言，本研究發現失能程度與復元結果具顯著相關，相信以
復元導向介入的治療或策略來協助思覺失調症者，進而提升個案的功 能在復元的結果表現上如：獲得社會角色、社會參與或生活品質必有 幫助。而現今精神障礙者介入的目標在於減少失能和改善功能性的結 果 (improving functional outcomes) ( Harvey, 2009)，復元導向的治療 或策略亦有實證基礎，包含以下：利用精神治療團隊提供支持性的治 療，包含藥物治療、諮詢等；協助個案投入日常生活中，包含教育、工作、社區生活；提供支持性就業服務 ；賦權與個體使其有選擇的權 利，但也同時必須對自己負起責任。其他協助思覺失調症患者復元之 策略如優勢觀點治療、同儕支持等亦可運用 (Anthony, 2000 ; Dunn
& Rogers, 2010 ； Farkas et al. , 2005)，專業人員皆可選擇適合的方案 運用，協助個案復元。
以下根據本研究的文獻回顧發現與研究結果對於研究方法和未 來研究方向作建議：
一、研究方法方面
本研究以 FUNDES7.1 量表來評量思覺失調症者的失能程度，其 內涵全面而廣泛。未來研究在功能量表的選擇方面也許可以使用特定
概念的功能量表，如：精神病患社會功能量表 (宋麗玉，2001) 、褚 氏日常生活評量表 (Daily Living Function Scale) (褚增輝、謝清麟，
2004)，或是與社區健康相關的量表如：台灣版社區健康評估量表 (RAI/MDS-CHA) (陽明大學整合性照顧研究中心，2014) 等量表來探 討與思覺失調症復元的關係，相信可以更精確地測量到特定的概念。
本研究量表評估的方式是以個體主觀 (personal subjective) 評量 的復元 、主觀評量的失能程度來進行相關性研究，未來也可運用客觀 的測量 (clinical objective) 來進行相關研究，藉此了解以不同角度觀 點所看到的「復元」與其他相關因素的狀況。
在思覺失調症的職業方面，Whitley (2014) 表示獨立的家庭生活 (independent housing) 也可以算是復元的結果，宋麗玉、徐淑婷 (2009) 在發展量表時表示工作也包含志工，本研究有職業的定義未包含志工 或是家庭主婦在內，未來復元相關職業定義上可再多加考量所涵蓋的 範圍。
二、研究主題方面
在文獻回顧及相關理論發現除了個人因素影響復元外，社會支 持、環境因素對思覺失調症者是否能夠復元也是一大關鍵，但在本研 究僅著重在個人的失能因素中，未有考量到環境因子，未來可運用與 精神障礙者環境相關的量表，如污名感受量表等 (韓德彥、陳淑惠，
2008) 來探討與復元之關係。
而在復元相關文獻中多提及就業與復元的關係，並有職業復元 (vocational recovery) 詞彙的出現 (Dunn, Wewiorski, & Rogers,
2008 ；Dunn, Wewiorski & Rogers, 2010； Terry, 2004；Woodside et al., 2006)，但多數為質性研究，本研究之有職業組與無就業組之復元狀 況也無明顯差異，也許未來的研究也可針對就業與復元的關係再進行 更多的量化研究，以獲得更多職業與復元的相關性，也可探討復元的 狀況可否預測是否有就業或就業穩定狀況。
本研究之文獻回顧中，發現大量文獻說明思覺失調症者達到復元 的情形，從發病前趨期、急性發作、到病情緩解後的確可稱為達到復 元，其實仍需要持續復健並保持穩定，否則思覺失調症患者病況可能 會重蹈覆轍產生旋轉門效應；本研究為橫斷式的研究僅探討特定一段 時間個體的狀態，至於達到復元的狀態的穩定度，如：多數思覺失調 症可以維持多久復元狀態等相關議題相信亦是未來可調查及研究的
方向。
