第五章 討論與建議

第五節 研究限制及建議

壹、 研究對象

本研究對象主要針對北部地區收案為主,收案人數為 35 人,

樣本數可能影響統計資料之分析之顯著。此次收案對象大部分為 精神科日間照護中心之個案,可能受限於某類型的思覺失調症者,

若能在其他外縣市收案,其可能拓展研究對象所處地區性的廣度,

能使樣本更具代表性,未來可多針對社區或是門診之個案收案,

擴大樣本代表性。

此次收案未能針對思覺失調症者之發病年齡以及服藥狀況來 作探討,而對於思覺失調症者來說,病程長短以及服藥狀況可能 影響可能影響復健成效、功能以及生活品質,此亦為未來可再探 討之部分。

貳、 研究工具

據本研究所知,目前台灣仍未有研究探討 DSM-5 精神病症狀 嚴重度臨床評分表在思覺失調症者之信效度,故本研究精神症狀 之評估其信效度仍有考量。但此工具強調全面性的評估症狀,對 於思覺失調症者之精神症狀嚴重度評估是重要的,故建議未來研 究除發展工具信效度以外,仍頇強調全面性評估精神症狀嚴重度

的觀點。

WHODAS 2.0 與 WHOQOL-BREF 目前在思覺失調症者使用 上皆有信效度之發展,顯示在思覺失調症者之應用是可行的,但 此兩者皆為主觀性評估工具,建議未來之研究可再加入其他客觀 性評估工具,如照顧者觀察或是臨床專業人員評估等。

參、 研究變項

本研究此次人口學變項為控制變項,但在生活品質的文獻探 討中,發現過去探討思覺失調症者生活品質之研究,對於性別、

教育程度、婚姻關係、居住狀況等人口學變項皆有琢磨,後續研 究可再對此深入探討。

由於本研究目的為探討思覺失調症者精神症狀嚴重度、失能 程度與生活品質的相關性,其因素之間是否有交互作用影響較難 排除、控制,為此次研究較不足之地方。

整體來說,影響思覺失調症者之生活品質相關因素相當廣泛,

而生活品質又為個人主觀感受,若將生活品質視為復健後的重要 指標,除了橫斷面的研究以外,縱貫面的後續追蹤研究可能為未 來研究之重點。

肆、 結語

根據此次文獻探討,此為第一篇以 FUNDES 7.1 和

WHOQOL-BREF 作為探討思覺失調症者失能程度和生活品質的 研究,同時以 DSM-5 的精神病症狀嚴重度臨床評分表作為評估 精神症狀嚴重度。

思覺失調症者之生活品質為重要的議題,而其影響因素十分 廣泛,本研究以探討精神症狀與失能程度為主。思覺失調者由於 疾病影響,造成各領域生活表現有不同的失能程度。故臨床專業 人員在協助思覺失調症者的過程中,應該全面性評估精神症狀表 現以及其失能程度,提供個別化的復健計劃,增進個人對於生活 品質之滿意度,且亦可將之作為復健成效及指標。
