第一節 結論
第一節 結論
本研究依研究目的、研究假設進行調查、介入措施分析其結果，歸 納出下列結論：
一、 本研究對象有 14.2%的長者屬於全口無牙，牙齦嚴重發炎的比率 47%，沒有雙邊功能的後牙咬合接觸者占 53%，經醫師評估須儘早牙科 處置者占 67%，牙菌斑控制指數達 72.6%，顯示高齡者口腔衛生、就醫 資源與功能維持的預防保健是重要的問題。
二、 進一步分析機構長者的口腔狀況，高達 76%住民超過 2 年沒有 看牙醫，四成的機構住民沒有每天刷牙，有使用牙刷者僅占 66%，咀嚼 力低於社區長者的 1.5 倍，顯示機構長者在口腔衛生、功能、潔牙行為 與就醫資源均較社區長者低。
三、 研究結果發現高齡者在牙菌斑控制指數，實驗組與對照組均有 顯著改善，改善幅度大約二成；顯示對照組再次接受牙醫師口腔檢 查，可能強化其口腔衛生行為，亦產生同步重視潔牙的行為效應。
四、 高齡者口腔衛生教育介入對長者口腔狀況自評、舌苔、牙菌 斑、吞嚥秒數、生活品質有改善；對咀嚼力之口腔機能可維持。整 體實驗組在舌苔及牙菌斑控制指數後測有顯著改善。整體及機構實 驗組在吞嚥秒數有改善，咀嚼力則發現整體及機構對照組後測較前 測差，但實驗組沒有差異；顯示在研究過程中長者仍逐漸老化，不

五、 口腔衛生教育(oral health education)是強化民眾口腔健康知能之 一種有效方式，也是口腔健康促進(oral health promotion)之重要環 節，高齡長者發生齲齒及牙周病的機率高，如果能夠有效且正確潔 牙，依照「早期發現、及早治療」的原則，藉由社區據點及機構提 供口腔健康的訊息並提升長者口腔保健知識，引導其趨向更健康之 行為模式，提早將口腔疾病防範於未然，以達到健康老化之目的。
六、 透過高齡者口腔衛生教育介入，結合衛生、社政及教育跨領域 團體建立整合性的合作模式，推動長者口腔健康促進課程，本研究 以實證基礎評估成效，可提供政策制度面的策略建議，擴大實施口 腔公共衛生政策。
第二節 建議
根據以上研究結論，就衛生教育、實務與未來研究三方面之應用，
分別提出以下建議：
一、 衛生教育應用之建議
口腔健康議題融入現有老年人衛教方案，口腔疾病防治工作的推行 涉及許多行為改變的策略，但是不論是長者的健康行為或照護系統的促 進輔助，其改變誠屬不易，通常需要經過長時間教育與環境支持才能見 效，建議持續發展多元衛教活動，改善老年人及照顧者口腔健康識能，
營造口腔健康之支持性環境，以強化國人重視口腔健康並採取具體行 動。
本研究自行設計臺北市的長者適用之口腔保健教材，透過「健口 操」及口腔衛教課程的實務推廣，培訓志工及安養機構照護種子訓練，
提供不同生理狀況的長者，學習進食與清潔的口腔功能維持，未來仍應 持續發展多元化、視覺化、體感化的教材，結合智慧科技提醒，運用於 不同生理功能之長者最適需求之口腔健康促進模式。建議社區及長照機 構應持續鼓勵長者進行口腔健康促進活動；編製適合不同次族群（含新 住民）閱讀之口腔健康促進衛教摺頁、海報等，持續發展出「英文版」
與「印尼文版」健口操教材，以提供多元語言照護者運用（附錄十 三）。
二、 實務應用上之建議
高齡者口腔照護可分為日常口腔照護與專業口腔照護（POHC：

增加研究對象的樣本數、納入家庭照顧者或機構照護者及督導 者等資源系統共同參與，擴大場域類型、增加介入時間及研究廣 度，以提供更完整的實證資料。對於老人的定義現今已無法僅用年 齡為指標，多輔以生活功能評量來區隔，增加不同年齡與功能的長 者參與研究，並進行不同期程衛教課程介入的效益評估，是未來研 究上之建議。
