所以，正式接受中西醫結合治療的患者，為數不多

(1)第一章. 前言. 第一節、研究背景現今台灣的醫療環境，中醫與西醫各自獨立運作，政府尚未明確訂定全面推動中西整合醫療的政策。民間雖有中西整合醫學會的成立，迄今仍未受到台灣醫界普遍重視。以西醫為主導的各個大型醫院，在公元 2000 年以來，如雨後春筍般陸續設立中醫部門，除了中醫診所之外，民眾可選擇到大醫院的中醫門診看病。設有『中西合作』、『中西整合』、『中西結合』等類似名稱的門診，僅出現在極少數醫院。西醫住院，會診中醫，也僅有在少數大型醫院進行，住院期間的中醫藥費用須由患者自行負擔，不在全民健康保險給付範圍。所以，正式接受中西醫結合治療的患者，為數不多。而實際上，有不少民眾在兩個不同處所，分別就診中醫及西醫，中醫師與西醫師沒有會診溝通，這不能稱為中西整合醫療。衛生署在 2004 年 4 月 16 日發布的『多重醫事人員資格執業管理辦法』(衛署醫字第Ο九三Ο二ΟΟ三Ο三號令)，具有醫師、中醫師、牙醫師等多重醫事人員資格者，僅能擇一資格作醫事人員執業登記，而在符合設置標準之執業處所，得執行他類資格業務。從此之後，一位具備條件的醫師進行中西結合醫療，便有法規可循，但是全民健康保險制度尚無中西醫整合的配套措施，民眾若給中西醫雙重身份的醫師看診，僅有中醫或西醫其中一項享有健保給付，另一項須自費。受限於現行法令規章及健保制度，中西醫結合的特色未能充分發揮。由於中醫、西醫的體系雙軌並行，而且就醫便利，民眾有可能在醫師不知道的情形下，同時使用中藥及西藥治療疾病。 •1•.

(2) 中西藥物的交互作用，可能提升療效或減少副作用反應，這是大家所樂見的。從另一方面來說，在某些情況可能降低療效或產生不良反應，是用藥安全極需注意的焦點。中藥與中藥之間的配伍禁忌，有『十八反』、『十九畏』的說法，中醫師皆熟知這些中藥不宜同時使用。西藥與西藥之間的交互作用，現代醫學已建立詳細的資料庫，醫師、藥師隨時可查閱。開立西藥的電腦程式，可以自動提醒某兩個藥物有交互作用，並列出交互作用的機轉與臨床表現，詢問醫師是否確定開立或者修改處方。中草藥與西藥間的交互作用，近年來漸漸受重視，國外已有一些論文提到西藥與草藥 (包含中藥及其他國家使用的草藥)之間的交互作用，稱為 drug-herb interaction 或 herb-drug interaction，陸續有論文及案例報告發表。目前被研究最多的是金絲桃草 (St. John’s wort) ，藥物動力學方面已漸漸清楚，而這個草藥只有在國外使用。我們常用的中藥，其與西藥之間的交互作用，目前文獻仍然不多。現行醫療制度下，中西藥併用情形： (1)設有中醫部門的西醫醫院病人在西醫部門住院，由主治醫師會診中醫，可以接受中醫及西醫合併治療。中醫師在開處方前，可參考西醫病歷，知道正在用的西藥名稱，避開不宜使用的中藥。但目前中西藥配伍禁忌資料庫尚未建立，中醫師知識裡的項目有限，例如知道使用 Warfarin 的病人要避免服用丹參以免發生出血 1，對於未被正式公開發表的中西藥交互作用則易忽略。. •2•.

(3) (2)設有中西醫聯合門診的醫院醫師評估病情需要，選擇單用中藥或西藥其中一類，在適當情況下可能建議中西醫結合治療，經由聯合門診模式，病人可一併領到中藥及西藥。聯合門診的醫師，多具有中西醫雙重資格，少數可能是單一中醫師或西醫師身份。儘管如此，在中西醫聯合門診對同一病人所開立的中藥與西藥，中醫師與西醫師可經由電腦連線互相看見西藥與中藥處方。中西藥之間是否存在不良交互作用，須有參考資料，發藥的藥師也應該即時查閱處方是否有中西藥之間的配伍禁忌，對聯合門診的中醫師與西醫師提供建議，但目前仍未建立正式的參考資料。 (3)患者自行在兩個地點就醫民眾自行分別就醫而領取中西藥的情形，可能潛在更大的問題，患者通常不會主動告訴西醫師正在服中藥，也很少對中醫師說正在服西藥，中醫師與西醫師很可能不知道患者總共吃幾種中西藥，對於中西藥間的交互作用也不甚明瞭。中醫師、西醫師、藥師及民眾，都需要吸收新知識，對中西藥間的交互作用要有所認識，避免中西藥併用而傷害身體。中國醫藥大學李珮端教授等. 2,3. 的研究顯示，台灣中部地區約. 有 32.5%民眾有中西藥物併用問題，其中衍生之相關問題值得重視。台灣地區對於中西藥物併用發生不良反應之正式報告極為有限，僅有少數之動物研究報告。醫事人員因中西藥物不當併用所產生之不良反應等概況與預防，缺乏相關文獻資料可供參考。在為數不多的醫學文獻中，談到的中西藥交互作用，幾乎皆是講不良反應。而實際上，中西醫適當的搭配，可以提升整體療效，降低副作用反應，需要足夠的時間與人力物力，才能建立新 •3•.

(4) 的治療原則。查詢資料費時，不易即時提供臨床應用若要查詢中藥與西藥併用方面的資訊，採用一般網路的搜尋引擎，可查到各篇零散的資料在不同的網站，需逐一點閱，甚為耗時。中文方面的資訊有限，有些資料來自中國大陸，但所使用的中藥注射液與中成藥製劑在台灣沒有，而且中文音譯的西藥名稱也讓慣用英文藥名的台灣醫師不易直接明瞭，另外，簡體字不是台灣民眾所習慣閱讀。英文網站可以查到 Herb (草藥，不只是中藥)與西藥的交互作用，但有些草藥(例如 St. John’s wort) 為歐美國家才有，台灣並未上市。想要查詢中西藥併用相關資料，耗費時日，且很難找到期望的內容。如果事先查好，作個整理，由單一網站便可快速查到。. •4•.

(5) 第二節、研究目的基於上述需求，本研究擬建構一個操作方便的網站，提供查詢中藥與西藥間的交互作用。中醫師開處方前，可以詢問病人正在服用什麼西藥，查網站看應避免什麼中藥，改用其他藥。西醫師開處方前，詢問病人正在服用什麼中藥，查網站看應避免什麼西藥。視病情需要，決定優先順序，是否停用中藥，或者不開立西藥，或改用別的西藥。藥師接到病患詢問中藥、西藥能不能合併使用，也可透過網站來參考。民眾若有中西藥併用的情形，可以上網查詢正在服用的中西藥，看看是否存在已知的交互作用，或者併用時應特別注意哪些症狀。網站未列出交互作用的中西藥，不表示沒有交互作用，可能尚未發表，或已發表而尚未整理到網站資料庫裡。本研究的目的在建立中西藥交互作用的快速參考資訊平台，即時提供資料，讓醫療相關人員及民眾皆能參考，避免中西藥併用帶來的不良反應，對於中西醫結合的優點，希望能加以發揮。. •5•.

(6) 第二章. 文獻探討. 第一節、中藥與中藥之間的交互作用在方劑之中，中藥以『君、臣、佐、使』的方式搭配使用。神農本草經提到中藥配伍之間的『七情』：單行、相須、相使、相畏、相惡、相反、相殺。4 單行：即是單用一種藥物以發揮效能，不須其他藥物輔助，例如獨參湯，以人參一味，大補氣血。相須：二種以上功用相同的藥物，合併使用，可互相增強作用，促進療效。例如知母與黃柏合用，滋陰降火的作用更強。相使：二種以上藥物作用類似，一種藥為主，其餘藥為輔，以提高療效。例如黃耆使茯苓，增強補氣利水的功能。相畏：即是受彼之制，一藥受另一藥的抑制，因而消除或減少烈性、毒性。例如生薑可抑制半夏的毒性，稱為半夏畏生薑。相惡：即是奪我之能，兩藥併用，一藥牽制或減弱另一藥的療效。例如黃芩能減弱生薑溫性袪寒宣肺氣之能，稱為生薑惡黃芩。相反：兩不相合，當二藥合用時，會產生劇烈不良反應。例如甘草反甘遂。相殺：制彼之毒，一藥能消除另一藥的毒，例如綠豆殺巴豆毒。在神農本草經之後，金元時期開始流傳十八反、十九畏的說法，分別如下：十九畏：硫黃畏朴硝，水銀畏砒霜，狼毒畏密陀僧，巴豆畏牽牛，丁香畏鬱金，川烏、草烏畏犀角，牙硝畏三稜，官桂畏石 •6•.

(7) 脂，人參畏五靈脂。十八反：甘草反甘遂、大戟、海藻、芫花；烏頭反貝母、瓜蔞、半夏、白蘞、白芨；藜蘆反人參、沙參、丹參、玄參、細辛、芍藥。歷代醫家在使用中藥的過程，會注意到配伍禁忌，避免不恰當的組合。. 第二節、西藥與西藥之間的交互作用西洋醫學對於單一藥物在人體的作用位置、接受器，均有藥理學研究。藥物動力學 (pharmacokinetics) 研究身體組織對藥物如何影響，包括藥物的吸收、分佈、代謝及排泄等過程。藥物效力學 (pharmacodynamics) 是研究藥物在身體內的作用，涵蓋生理學、生物化學、分子等層面。5 在藥物的吸收過程，Cholestyramine 及 Colestipol 這類的藥，會與其他藥結合，形成不可吸收的複合體，以致降低其他藥的療效。胃中的 pH 值受制酸劑以及 H2-blocker 之類藥物影響，變得鹼化，也會改變其他藥物的吸收量。酸性藥物如 Salicylic acid 的吸收會減少，鹼性藥物如 Diazepam 的吸收則會增加。5 腸道蠕動速率受 Metoclopramide 或 Cisapride6 之類藥物影響而變快，使其他藥物在腸道停留時間縮短，而減少吸收。反之， Haloperidol 等藥使腸道蠕動變慢，此期間所服的各種藥物皆會在腸內增加吸收。在藥物的分佈過程，某些藥例如 Warfarin 的安全範圍狹窄，而且與血中蛋白(albumin)結合度高，合併使用的藥物若會競爭蛋白結合部位，即會使游離態的 Warfarin 增加，造成過度作用，可 •7•.

(8) 能發生出血危險。大部份的藥是在肝臟代謝，肝內各種酵素對於各種藥物作代謝處理，主要的酵素是 cytochrome P450 (CYP450) 。人類的 CYP450 有超過 30 種同功(isoforms)，各有專一的受質及不同的機轉調控。舉例來說，Warfarin 包含 S-Warfarin 與 R-Warfarin 兩種立體結構物，S-Warfarin 主要經由 CYP2C9 代謝，R-Warfarin 則經由 CYP1A2 及 CYP3A4 代謝，不同的藥物會經由對 CYP450 不同受質的代謝而影響 Warfarin。7, 8 抗黴菌藥諸如 Fluconazole 與 Ketoconazole 等 9，會抑制黴菌的 CYP450，也抑制人類的 CYP450，減少 Warfarin 的清除，而造成出血時間延長。 Cimetidine10 和 Erythromycin11 以及 Ethanol12 等，也會抑制 CYP450 的作用，減少代謝而增加其他藥物的活性。 Phenytoin13 、 Carbamazepine14 、 Phenobarbital15 及 Rifampin16 等，會激活 CYP450 的作用，加速其他藥物的代謝，而降低療效。藥物的排泄主要經由腎臟與肝臟。 Amphotericin B17 、 Methotrexate18 與 Amikacin 以及 Gentamicin19 等藥可能影響腎功能，已知病人腎功能不佳，或預測將會受藥物影響，最好降低各種藥物劑量，以免副作用發生。以上西藥間的交互作用機轉，可以作為研究中西藥交互作用的參考。. •8•.

(9) 第三節、中藥與西藥之間的交互作用近年內，有日漸增加的醫學期刊提到中草藥與西藥之間的交互作用，英文稱為 drug-herb interaction 或 herb-drug interaction。從事相關研究者，有的以臨床病人資料為基礎，有的作動物實驗，以現代藥理的的模式探討交互作用。外國草藥 St. John’s wort 被探討最多，藥理學實驗報導資料也最豐富，因為這個藥在臨床上具有顯著交互作用，降低免疫抑制劑的效用，造成多例器官移植失敗 20, 21。國內常用的中藥，只有少數在歐美國家上市，因此，PubMed 網站上的相關中西藥交互作用，資料有限。例如丹參與 Warfarin 併用會造成過度抗凝血反應而容易發生出血. 22. ，當歸與 Warfarin. 併用會造成 INR (international normalized ratio)過度增加 23，麻黃與 Theophylline 併用會造成高血壓. 24. ，人參會造成類固醇的副作用加. 重 25, 26，甘草會降低血中鉀離子濃度而增加 Digoxin 的毒性 27。. •9•.

(10) 第三章. 材料與方法. 第一節、資料收集國際英文期刊 PubMed 網站：網址為 http://www.ncbi.nlm.nih.gov/entrez/query.fcgi，其資料庫收錄 1950 年代以來 MEDLINE 以及其他生命科學方面的期刊資料，以英語為主。使用關鍵字 herb、medicinal food 或 Chinese medication，與 drug (或 interaction)作交集條件，搜尋國際英文期刊並取得全文內容，至 2004 年底為止。對於期刊裡常出現的中西藥交互作用，例如人參(ginseng)、甘草(licorice)，另以中藥名稱、西藥名稱作關鍵字搜尋，比較能查到第一手資料，而不僅是回顧性質的文章。取得論文的基本資料，包含 Author、Title、Journal、Year、 Volume、Issue、Pages 等醫學期刊表頭基本要項，也有 Accession Number(每篇文章的識別編號)，以及 URL (網頁超連結，直接連到該篇文章)，而摘要部分的取得則視來源是否提供而定。初步篩選先看摘要內容，是否探討中草藥與西藥之間的交互作用。某些文獻雖以 drug herb interaction 等關鍵字搜尋到，內容對於中西藥交互作用卻沒有明確描述，此類文獻則不予納入本研究的範圍。取得全文經過摘要篩選之後，與本研究有相關的文獻，進一步取得全文詳細分析。區分為人體臨床資料或動物實驗資料，臨床資料註明案例數。 • 10 •.

(11) 台灣本土資料以下兩個資料來源，皆位於國家圖書館網站。全國博碩士論文資訊網：網址為 http://etds.ncl.edu.tw/theabs/index.jsp，其資料庫收錄 1956 年以來台灣的博碩士學位論文。以關鍵字「中西藥併用」、「中藥」、「西藥」、「交互作用」、「不良反應」等，查詢各大學院校研究所的博士與碩士研究報告論文，至 2004 年底為止。中文期刊篇目索引影像系統：網址為 http://readopac2.ncl.edu.tw/ncl3/index.jsp，其資料庫收錄台灣及部份港澳地區自 1994 年以來所出版的中文學術期刊、學報，以及讀者利用頻繁之各類期刊 4000 多種。本研究希望找到的中西藥交互作用相關文章，刊登在中醫藥委員會年報，以及醫學會、藥學會、公會刊物或一般雜誌，可由此網站以簡易方式查得。年代在 1994 年以前的文章，須靠人工翻閱，耗日費時，不列入本研究之範圍，後續研究若有需要，才考慮搜集早期資料。這部分的搜尋，採用與前述檢索博碩士論文相同之關鍵字，查詢在台灣發表的刊物，取得相關之研究報告、個案報告及其他類型的文章，至 2004 年底為止。. • 11 •.

(13) 數文獻有詳細說明，超出 VARCHAR 型態最大極限 254 bytes，所以採用 TEXT 型態，不須指定欄位長度，能夠儲存大量文字內容。交互作用編號的欄位 Serial，資料型態是整數(INT)，包含 4 個 bytes，儲存範圍達十位數字，對於目前資料庫內容數量的編號綽綽有餘。每一筆資料錄的編號是唯一的，不與其他資料錄重複，以便資料庫管理。相同的中西藥交互作用(例如人參與 Warfarin)可能出現在多篇文獻裡，由不同文獻所擷取的交互作用資訊，使用獨立的編號，由 1 開始累增。本研究由原始文獻內容，逐篇篩選中西藥交互作用的資料，文字填入上述資料庫欄位。. • 13 •.

(14) 第三節、網站資料庫建立及網頁設計申請網址名稱 http://dhi.cmu.edu.tw ，其中 dhi 是 drug-herb interaction 的縮寫，cmu.edu.tw 是中國醫藥大學的網域名稱，這個網址可謂簡短易記，方便使用。篩選後的中、西藥交互作用建立 MySQL 資料庫，網頁使用 PHP 語言撰寫搜尋程式。選用 PHP 及 MySQL 的優點為免費，不需耗用龐大預算，而且容易管理。查詢介面包含輸入中藥名稱、西藥名稱的欄位，按下查詢按鈕，系統會記錄中藥及西藥名稱與查詢日期、時間，及操作者所在的網路 IP 位址，可併日後統計系統使用狀況及中西藥名稱出現的次數。查詢完成的輸出結果，列出符合條件的中藥名稱、西藥名稱，相關文獻若超過十篇，一次顯示十篇，每筆交互作用資訊列出研究方式(個案報告、動物實驗等)，及原文篇名、期刊名，提供網頁超連結到原文出處。. • 14 •.

(15) 第四節、專家諮詢會議本研究在 2004 年 7 月至 12 月期間，獲得行政院衛生署中醫藥委員會補助，成為『建構中藥用藥安全環境五年計畫』的子計畫之一。因考量中西藥交互作用的觀念的宣導過程、網頁訊息如何表現、網站的定位等方面可能面臨多項問題，所以在執行研究計畫期間，召開三次專家會議，聽取各方意見。第一次專家會議，主要邀請中醫師、西醫師、藥師(中藥局主任、西藥局主任)、藥理研究學者等專業人士，提供意見。第二次專家會議，邀請中醫藥委員會的長官、中藥用藥安全五年計畫各項子計畫主持人、本校藥學系教授、本校附設醫院從事中西結合臨床醫療工作的醫師等專家參與座談。第三次專家會議，邀請從事中西醫結合研究的醫師、中醫師參與座談，提供建議。. • 15 •.

(16) 第四章. 結果. 第一節、資料收集初步篩選在 PubMed 的搜尋之中，若使用太多關鍵字的交集，會使得到的篇數變少，可能遺漏重要文獻。關鍵字少，得到篇數較多，但可能因此夾雜不相關資料一併收入。本研究的初步篩選，先用較少的關鍵字組合，擷取較多篇之摘要，由摘要先排除不相關之文獻，之後再取得全文詳細閱讀。例如：以關鍵字 ginseng 及 interaction 交集搜尋到的文獻之中，有一篇 Choi S 等人發表在 British Journal of Pharmacology (2001;132:641-648) ，題目是 A novel activation of Ca2+-activated Clchannel in Xenopus oocytes by Ginseng saponins: evidence for the involvement of phospholipase C and intracellular Ca2+ mobilization。其內容探討人參皂與細胞膜結構之交互作用，不是講中西藥之間的交互作用，因此排除此類文獻。又如：關鍵字 herb 及 interaction 交集，查得期刊之中，有一篇 Mills E 等人發表在 Nutrition Journal. (2005;4:19) ，題目是. African herbal medicines in the treatment of HIV: Hypoxis and Sutherlandia. An overview of evidence and pharmacology。其內容描述的是非洲草藥，這些資訊對於國內常用的中藥、西藥交互作用沒有助益，此類文獻皆排除。. • 16 •.

(17) 取得全文 (一) 取得 PubMed 英文期刊全文 100 篇，其中 73 篇明確提到中西藥交互作用(Mai I.等作者)28，3 篇有提到草藥對代謝酵素影響而未指明特定西藥 (Lo A.C. 等作者 )29 ， 9 篇雖提到 drug-herb interaction 但非文章主軸，15 篇由 drug-herb interaction 關鍵字搜尋得到，但內容無相關。剔除低度相關 9 篇及無相關 15 篇，上列文章可供列入網站資料庫的共有 76 篇，其中屬臨床試驗有 5 篇，臨床個案報告 12 篇，動物實驗資料 12 篇，其餘為回顧性質文章，或者純由理論去推測草藥與西藥間的交互作用。 (二) 台灣本土博碩士論文，葉怡柔發表的葛根之代謝動力學及其對環孢靈、地高辛動力學之影響. 30. 等，共 11 篇。在中文期刊. 篇目索引影像系統搜尋到的中文研究報告及刊物文章，游明謙發表的中藥麥門冬湯與西藥併用治療氣喘之交互作用研究 36 篇。. 國內外之原始篇目，總共分析 147 篇。. • 17 •. 31. 等，共.

(18) 第二節、資料擷取結果處理過程舉例說明，Gupta 等人的研究. 32. ，描述一種中藥與一. 種西藥之間的交互作用，摘要如下： ABSTRACT The general population of the U.S. uses over-the-counter herbal medicines. Danshen is a Chinese herbal product used for the treatment of cardiovascular disease. In a previous study we showed that Danshen has significant digoxin-like immunoreactivity, and we used this parameter to monitor total and free Danshen activities in sera. In this report we demonstrated strong protein binding of Danshen (50-70%), and we also identified albumin as the major serum protein that binds Danshen. Because salicylate, which is also strongly bound to albumin, is a widely used over-the-counter medicine in the U.S., we studied Danshen-salicylate interaction in vitro. We observed no significant change in free Danshen concentrations as measured by free-digoxinlike activity when salicylate concentrations were subtherapeutic (< or = 100 microg/mL). With therapeutic concentrations of salicylate (> or = 150 microg/mL), the free Danshen concentrations significantly decreased from the control. On the other hand, Danshen can displace salicylate from protein binding, thereby increasing the free salicylate concentration. (34) 第四節、網站資料庫的內容因時間與人力有限，分析的文獻到 2004 年底為止，後續研究將會加入新資料。在原始文獻擷取的文字，整理到網站資料庫中，建立多欄位的大型資料表，可在網頁搜尋結果顯現符合中西藥條件的部分。於本論文中，為縮減篇幅，表格每一筆資料皆省略中草藥英文名、中草藥學名、個案人數、病人情況描述(僅少數原始文獻提及)及網址連結等欄位。(表 4.6) 某些資料錄的欄位是空白，例如交互作用的機轉，在大部分原始文獻未提及。有些文獻僅建議中西藥不要併用(do not take together)，未明確指出交互作用。類似情況，在表中皆隱藏其欄位，而非採取原資料庫之型態，留下很多空格。中西藥交互作用的文字，出自中文期刊者，皆以中文描述。出自英文期刊者，大多直接使用原來英文字，待下一階段資料整理，將全面提供中文翻譯，以便國人直接閱讀。. • 34 •.
