第二節 研究限制與建議
本研究在模型內生性問題、價格資訊來源正確性、研究追蹤期間、所採用分 析模型、研究區域劃分方式等部分尚存在相關研究限制。在模型內生性問題部分，
雖然本研究第三章第二節已說明為降低因模型內解釋變數與殘差項有相關性所 產生內生性問題(endogeneity)對分析結果之影響，本研究方採用固定效果模型分 析，但該模型僅能排除部分固定因素之影響，尚可能存在其他未納入的因素，可 能使該模型產生內生性問題，進而造成最後研究結果產生偏誤，又現階段尚無法 找到合適的工具變數處理內生性問題，此實為本研究之一大限制。另一方面，本 研究所用解釋變數間的相關性大多呈現低度相關，故應不致存在顯著的共線性問 題(collinearity)。
在價格資訊來源之正確性部分，本研究所用以分析的塗藥支架價格資料來自 健保署全球資訊網頁對外公布之醫院申報價格，而 2007 年至 2009 年三年間，醫 院並非每月均會申報價格，每年度申報之頻率亦有些許不同，即有部分月份可能 醫院仍有提供病患使用特定支架，但卻未申報該支架價格之情形存在，本研究僅 得以相鄰月份該醫院對同型號支架之訂價取代;甚至健保資料庫內部分醫院其病 患有使用特定型號塗藥支架，該醫院卻從未申報該支架價格，以致須將此類樣本 排除在外。而各醫院所申報塗藥支架價格之基礎可能也有不一致之情形(周恬弘，
2014)，如有些醫院申報之支架價格可能包含其他周邊配備或服務，上揭情形均 可能造成研究資料存在一定誤差。此外，醫院在申報支架價格時亦可能產生人為 上之疏失，致影響資料之正確性，存在作業面風險。
在研究追蹤期間部分，健保署係自 2006 年底開始對塗藥支架開放差額負擔 制度，而本研究之研究追蹤期間為接續該年之 2007 年至 2009 年三年期間，處於 差額負擔制度實施之初始階段，尚未臻成熟，民眾可能尚未充分了解此制度之運 作機制，或並不知曉健保署有定期公布各特約醫院其塗藥支架之當期價格資訊，
以致未充分進行瞭解相關資訊，進一步影響本研究實證結果之客觀性。
在所採用分析模型及變數之設計部分，本研究將一醫院內所有關於血管支架 價格及使用量等資訊，以加權平均之方式整合為一，按醫院別為分析基礎，以固

定效果模型進行分析，造成無法具體觀察到每位植入支架病患之個人特質資訊，
諸如其職業、所得水準、疾病嚴重程度等，以及為病患進行手術的醫師之個人相 關資訊，諸如其年資、年齡等;且部分因素其取得程度亦屬困難，諸如醫師對於 支架之偏好、醫院就各塗藥支架之進貨成本、病患對血管支架之認識程度等;而 固定效果模型亦將大部分不易隨時間變動之內外生因素排除於分析模型之外，諸 如醫院之權屬別、層級別等因素，以致本研究無法進一步對上揭因素進行分析。
此外，在對支架價格及病患投保金額加權平均之過程中，因資料之不完整而必須 排除某些樣本，此點亦可能影響最後加權平均數值之客觀性。
在研究區域之劃分方式部分，在價格競爭效果變數的分析上，本研究採用虛 擬變項之設計，按健保署各分署所轄區域將全國劃分為六大醫療區域，並將醫院 之塗藥支架價格與同區域所有被觀察醫院之價格分布比較，視其價格落在何等級 價位區間內，決定虛擬變項，惟此種劃分區域之方式較為簡略，可能有失客觀性，
因部分區域範圍可能較大，涵蓋多個縣市，而各縣市之交通並非完全暢通，病患 在選擇就診醫院時，可能完全不會考量屬同一分署別之部分較偏遠醫院，或是逕 為前往支架價格相對其住所附近地區醫院較便宜的其他地區內醫院進行冠心病 治療手術，以致價格競爭效果等變數稍失客觀性。
最後，在對未來研究者的建議上，由於本研究係以固定效果模型分析，並按 醫院別為分析基礎，故建議未來研究者或可以病患別為分析基礎，使用二元選擇 模型如 Logit 模型等分析，設計出適合此類模型的塗藥支架價格變數，及將病患 的教育程度、疾病嚴重程度、醫師的技術、服務經驗與建議、醫院對塗藥支架的 進貨成本、支架品質等病患與醫師個人相關資訊，納為解釋變數，一併觀察此類 因素對病患選擇支架種類之影響，並設計出可有效解決前述模型內生性問題的工 具變數以進一步處理。此外，本研究也建議未來研究者可針對植入塗藥支架的病 患，相較其他類型血管支架使用者，於手術後一定觀察期間內產生支架血栓併發 症的機率差異與變化情形，以及支架血栓對於此類病患治療結果的影響情形進行 分析;或針對「生物活性塗層冠狀動脈支架」及「全吸收式生物血管模架」等技 術面優於塗藥支架的新種特材之治療結果、成本效益、與塗藥或傳統支架的差異
