實是有價值的,值得未來反應轉移研究採用並進一步驗證。

第七節 未來研究方向

雖然個人化生活品質的概念越來越受到重視,但 SEIQoL-DW 做為個人化生 活品質量表,過去的分析卻顯示,對不同疾病或嚴重程度的病人所做的評估,

似乎在總分或結果上未能呈現出什麼樣的不同,或兩個時間點間的評估也不見 得有所差異,反應轉移旨在說明,受試在不同的時間點進行生活品質的自我評 估時,也會因為他們對於所評估生活品質認識的改變,而影響了評估的結果,

Sprangers and Schwartz (1999)將反應轉移更細分成重新構念、重新校準、重新權 重,本研究主要在探索,SEIQoL-DW 是否能以反應轉移的概念來進行分析,初 步結果顯示,慢性腎臟病、血液透析、腹膜透析三組最明顯的差別仍然在重新 校準上,進一步以自我調節的概念來分析重新校準的結果發現,轉換重要的生 活領域可以提升生活品質,但兩次提名未改變的生活領域,卻沒有如理論預 期,會降低對這些重要領域的期待,這究竟是因為測量期待的工具不理想,或 是文化差異所導致,值得進一步研究。另一方面,受試轉換的生活領域是相對 權重較低的,而轉換過後的新領域,在滿意程度、期待、和相對權重都顯著提 高,過去認為權重對生活品質的評估不具獨特影響性,但從本研究的結果來 看,權重在轉換領域的過程中似乎扮演著重要角色,未來也值得進一步釐清。

在反應轉移指標方面,重新構念的兩次提名差異數、重新校準的兩次提名 領域期待平均差、then test 重新權重平均值,這些反應轉移指標或計算方式,

都是本研究第一次提出來,而兩次提名領域期待平均差和 then test 重新權重平 均值都在本研究中達顯著,值得後續進一步檢驗。另外,本研究發現,重新構 念、重新校準、和重新權重之間似乎有緊密的關聯,究竟是用一個指標來代

表,或者仍分成三個指標合適,仍待未來研究探索。

重大事件是反應轉移的刺激因素,本研究僅檢驗三個月內無健康相關重大 事件影響的受試,發現反應轉移會因為時間的推移而發生,若再加入重大事 件,在隨著時間推移的過程中是如何影響個人的生活品質,反應轉移是如何變 化,重大事件的性質(正向或負向)、程度(對個人影響幅度的大小)的關係又 為何?在研究反應轉移指標的時候,實在無法避免要先釐清這個部分,而這 些,都有待未來有興趣研究者的持續投入。
